J. Haley, Operator/Owner, assisted me with today’s visit. Your program currently operates with a four-star license, issued 8/3/2023 earning 7 points in the education component, 2 points in the program standards component and 1 quality point for infant capacity restricted to no more than two children under age one. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed during today’s visit. It is important to frequently check your email to remain informed about any new changes that have or may occur. The last annual compliance visit was conducted 5/6/25. The family childcare home’s compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent as of 4/2/26. The NC Secretary of State website was reviewed on 3/31/26 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding ownership of this facility. A walk-through of the family child care home was completed today; all indoor and outdoor areas were monitored. Children were engaged in making bunny baskets and wind socks. The children transitioned to outdoor free play. Activity plan was posted. The fenced outdoor play area in the back yard consisted of portable play equipment, ride-on toys, balls, stationary equipment, and an area for shade. Lunch was observed and consisted of Cheese pizza, tossed salad, apples, and milk. The following violations were documented: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. An aerosol can of Lysol disinfectant spray was located on an open shelf 5 feet above the floor in the bathroom. .1719 (a)(7) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The last documented drill was 12/29/25. .1719(a )(16) & .1721(e )(7) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The last update occurred January 6, 2025. .1714(e ) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 4/16/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance: Storage of Hazards Items - All aerosol cans are highly flammable and present a hazard because of the compressed gas used as a propellant. If punctured, the contents may be released so forcefully that injuries can result. An aerosol can of Lysol disinfectant spray was located on an open shelf 5 feet above the floor in the bathroom. You moved the aerosol can during the visit to a locked storage room. All aerosols must be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. Emergency Drills: You must conduct either a shelter-in-place or a lockdown drill every three months to ensure that the operator and children are aware of what actions to take in the event of a life-threatening emergency. An emergency drill was not completed in the first quarter of 2026. The last documented drill was 12/29/25. You stated that you forgot to conduct the emergency drill. Set reminders on your calendar for proposed dates to conduct emergency drills. Emergency Preparedness and Response Plan/ Emergency Drills- It is important to consider how to prepare for and respond to emergency or natural disaster situations and develop written plans accordingly. You have procedures in place to address natural disasters that are relevant to your location (such as tornados, flash floods, and hurricanes) and all hazards/disasters that could occur in any location including acts of violence, bioterrorism/terrorism, exposure to hazardous agents, facility damage, fire, missing child, power outage, and other situations that may require evacuation, lock- down, or shelter-in-place. However, these procedures must be updated and reviewed annually. The last update occurred January 6, 2025. You stated that you had forgotten to update the plan. Add reminders to your calendar to update your EPR Plan annually. The only way to prepare for disasters is to consider various worst case or unique scenarios, and to develop contingency plans. By brainstorming, practicing & documenting emergency drills at least every three months and thinking through a variety of “what if...” situations and developing records, protocols/procedures, and checklists, facilities will be better able to respond to an unusual emergency or disaster situation. Reminder: • Send application for assessment for a rated license by the first week of June 2026. • Send the Family and Community Engagement Standards form and Continuous Quality Improvement plan (CQI) for Facility and Individual by third week of June 2026. • Complete coaching/training options by last week of June 2026. • E.P. is to update WORKS by applying for FCCH and teacher position by May 2026. • J.H. is to complete 1 additional hour of on-going training by 5/5/26. • E.P. is to complete 2.5 additional hours of on-going training by 5/5/26. Additional comments: Sex Offender Registry: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. Children were engaged in group time identifying colors. The children transitioned to free play indoors and then to outdoor free play. Activity plan was posted. The fenced outdoor play area in the back yard consisted of portable play equipment, ride-on toys, balls, stationary equipment, and an area for shade. Lunch was observed and consisted of hot dogs on whole grain bun, mixed fruit, baked beans, and milk. The following violations were documented: Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. The paint on the stationary is starting to peel away and some rusty areas are beginning to be exposed. .1719(a)(1)&(17) 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance: ITS-SIDS Training – Caring for Our Children states, “About 3,500 SIDs occurred in the U.S. in 2014.” Safe sleep practices help reduce the risk of sudden unexpected infant deaths. Staff members’ ITS-SIDS training expired on 7/22/24. You stated you thought you had taken it but could not find the certificate. Family Child Care Home operators licensed to provide care for infants must implement a Safe Sleep Policy and undergo training on the importance of consistent use of safe sleep policies and practices before they are allowed to care for infants and at least every three years thereafter. Outdoor Environment: The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. The paint on the stationary is starting to peel away and some rusty areas are beginning to be exposed. It is important to remove rust on child toys and other metal objects, as children can get a skin infection from or tetanus if they have a small break in the skin and touch rust. You stated that you had not realized the paint had begun to peel away exposing rust. During your daily playground inspections, inspect outdoor equipment for hazards. Observations should be made while the children are playing, too, to spot any maintenance problems and correct them as soon as possible. Additional Information: We discussed Clean Classrooms for Carolina Kids and your Lead in Water testing is in progress, Lead-Based Paint and Asbestos testing is complete. Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that you do not need to schedule a rated license assessment unless you voluntarily request one. The new QRIS has been approved by the Child Care Commission. The Division is working on implementation and training for the new system.
