Your program currently operates with a five-star license, issued 10/23/24 earning 7 points in the education component, 6 points in the program standards component (meeting enhanced space and enhanced ratios) and 1 quality point for staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on 3/10/25. The sanitation inspection was completed on 9/26/25 with a “Approved” classification. The last fire inspection was conducted on 3/4/26. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website. The information is current. Contact me anytime you need to make any changes to your contact information, e.g., phone numbers, mailing address. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent as of 3/2/26. The NC Secretary of State website was reviewed on 3/2/26 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding ownership of this facility. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children engaged in indoor free play, personal care routines, and outdoor free play. This week’s theme was centered around weather. In space #2, children colored umbrellas and glued raindrops on their umbrella, built roads using blocks, used scissors to cut pictures of food out of sales flyers, and engaged in group time. The outdoor learning area included a stationary climbing structure, ride on toys, balls, and tricycles. Proper handwashing was observed. Lunch was observed and consisted of chicken legs, wheat bread, broccoli, peaches, and milk. The following violation was documented: Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. The plastic boarder located on the front right side of playground, is cracked and broken. 10A NCAC 09 .0604(p) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 3/25/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. It is important that the outdoor area is checked daily and proper upkeep is conducted to ensure the environment is safe for use. The plastic boarder located on the front right side of playground, is cracked and broken. A child can have their fingers pinched in the opening. You stated that you will have your husband replace it. Regular outdoor inspections are critical to prevent deterioration of equipment and accumulation of hazardous materials in the play. It also ensures that appropriate repairs are made as soon as possible. The outdoor play area should be inspected daily to ensure the materials are in good repair and ready for use by the children. Observations should be made while the children are playing, too, to spot any maintenance problems and correct them as soon as possible. This week’s theme was centered around camping. A tent has been added to the space and story time included a book entitled Monster and Mouse Go Camping. After story time, the children helped the teacher pack for camping by naming items they wanted to take camping. Conversations between the teachers and the children were inclusive of building a camp fire, making smores, etc. Lunch included chicken, pasta, green peas, pears and milk. The following violations were documented. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Arrival times were documented for five (5) out of the seven (7) children present when I entered the facility. 10A NCAC 09 .0302(d)(4) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Insert Technical Assistance with Documented Violation (s): Attendance Records - Promoting the health and safety of individual children requires keeping records regarding supervision of each child in care. You must know which children are receiving care at any given time and for how long they were in your care each day. During today’s visit, attendance records for all children in care were not complete. S. Jenkins, Lead Teacher, in Space 1 stated she forgot to document the arrival times for two out of the seven children present when I arrived. Your attendance records must always accurately reflect the number of children in care. You added the arrival times for the two children and continued to document arrival times as other children arrived. Electrical Outlets- Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. For this reason, all electrical outlets, including those on power strips, not in use that are in areas accessible to children must be protected with a safety plug or a tamper-resistant electrical outlet. An open electrical outlet was observed on the wall near the computer in Space 2. You indicated someone unplugged something and forgot to replace the safety cover. You replaced the cover during the visit. Adding a reminder on the wall over each socket can serve as a reminder to add the safety cover when not in use or have the wall plate on the socket replaced with a tamper resistant electrical outlet. Plastic Bags – Plastic bags have been recognized for many years as a suffocation hazard. The U.S. Consumer Product Safety Commission (CPSC) has received average annual reports of twenty-five deaths per year to children due to suffocation from plastic bags. In child care facilities, plastic bags must be made inaccessible to children less than three years of age. In Space 2, two children ages two-years- old were being cared for before going into their designated space. Plastic zip lock bags used to store craft sticks, drinking straws, markers, pipe cleaners, and pom poms were observed on the art shelf. When I informed you, you nodded your head and made the items inaccessible. If the teachers are going to use the plastic zip lock bags, then these bags must be stored out of reach of the children less than three years of age, which is defined as five feet or higher from the finished floor. Monthly Outdoor Inspections - Young children spend many of their waking hours in child care centers. Outdoor play for children encourages movement and physical activity stimulates children’s learning. Children learn by exploration, so outdoor spaces must be designed in such a way that children can test their abilities in a challenging yet safe environment. To ensure outdoor spaces are safe for exploration, daily checks must occur with documentation occurring at least one per month using the Playground Inspection Checklist. Monthly documentation was not available for March 2024. You indicated it occurred and you thought you had documented it but must not have in haste. Completing an end of month audit of paperwork will help you to check for errors in instances like this. Reminders: Stay updated with changes and new rule updates by visiting the DCDEE website at https://ncchildcare.ncdhhs.gov/. Their phone number is (252) 758-8885 or check out their website at mppfc.org. Lead Based Paint & Asbestos - The Child Care Commission adopted childcare rule changes in January 2024. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation will allow facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law.
