R. Matthews assisted with the visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, first aid chart, menus, daily schedules, activity plans, summary of NC Child Care Law and staff/child ratios were posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. A sample of children's records were reviewed during the visit and the worksheets were completed. Files for all new staff members and a sample of existing files were to be reviewed during the visit and the information on the Staff and Training Worksheets was verified. An outdoor inspection was last completed on 6-27-25. A fire drill was documented as conducted on 6-27-25 at 8:53 am. A shelter-in-place drill was documented as completed on 3-31-25 at 9:30 am. An approved fire inspection was completed last on 2-4-25. A sanitation inspection was last completed on 5-7-25 with zero demerits and a superior classification. The children were observed during free play and eating lunch which consisted of spaghetti with meat sauce, corn, applesauce, and milk. The corporation status of Two by Two Day Care Development Center, Inc. was verified as current and active during the visit using the Secretary of State's website. A five-star child care center with five points in education, seven points in program standards and one quality point was issued to you effective 11-7-17. Violation Number Comment Rule 107 The center did not comply with the permit restrictions. Permit restrictions were not being met. GS 110-91; GS 110-106 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu by the front door was not current. 10A NCAC 09 .0901(b) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last emergency drill was completed on 3-31-25. .0604(u);.0302(d)(8) A violation regarding permit restrictions was documented today because the program was not complying with enhanced ratios minus one. You stated today that you wish to change the number of points in program standards so you no longer have to meet enhanced minus one. I spoke with my supervisor during the visit for clarification on the steps that need to be taken to remove the minus one from the permit. Once I receive clarification I will contact you and discuss the next steps that need to be taken. **Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules.**** Please send me a written statement describing how and when the above violation was corrected. The written statement must be received no later than 7-23-25.
