The visit was conducted in conjunction with a complaint investigation. I monitored the indoor and outdoor environments for compliance with applicable child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plan, daily schedule, staff/child ratios and a summary of NC Child Care Law were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. Staff files were reviewed and the the Staff and Training worksheet was completed. A sample of children's records were reviewed and the worksheet was completed. A fire drill was documented as completed on 9-25-24 at 9:10 am with an evacuation time of two minutes and twenty eight seconds. An outdoor inspection was documented as conducted on 10-4-24. An approved fire inspection was completed on 1-16-24. A sanitation inspection was last completed on 1-31-24 with zero demerits and a superior classification. The children were observed during free play and naptime. A meal/snack was not observed. A four star license with three points in education, seven points in program standards and one quality point was issued to your classroom on 5-25-18. Based on observations made and on information provided, you continue to meet the requirements previously met to obtain such a license. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. Verification of receipt was not available in one child's file. GS 110-102 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. Immunization records were not available in one child's file. 10A NCAC 09 .0302(d)(2) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. Verification of receipt was not available in one child's file. .1804(c) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Verification of receipt was not available in one child's file. .0604(j) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Verification of receipt was not available in one child's file. .0608(b)(1-6) ****Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules.**** Please send me a written statement describing how and when the above violations were corrected. The written statement must be received no later than 11-7-24. Please note that if your letter is mailed, a hand written signature is required. As an alternative, compliance letters can now be submitted via email. I recommend emailing your letter to reduce the time needed to ensure my receipt of your information; however, if you do not have access to email, please ensure that your letter is mailed in a timely fashion and that you allow for any possible delays in the mail service. New staff files and a sample of the existing staff files were reviewed and the information on the Staff and Training worksheet was verified. A sample of children's records were reviewed and the worksheet was completed. A fire drill was documented as completed on 10-13-23 at 9:01 am. An outdoor inspection was documented as conducted on 10-11-23. An approved fire inspection dated 1-5-23 was received prior to the visit. A sanitation inspection was last completed on 10-5-23 with two demerits and a superior classification. The children were observed during naptime. A meal/snack was not observed. A four star license with three points in education, seven points in program standards and one quality point was issued to your classroom on 5-25-18. Based on observations made and on information provided, you continue to meet the requirements previously met to obtain such a license. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet in space 506 was not covered or in use. 10A NCAC 09 .0604(c) The violation that was documented was also corrected during the visit.
