You, Angela Warren, Owner/Administrator, assisted with today’s visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five (5) star license issued January 8, 2018. The permit restrictions were in compliance including First Shift (daytime care), meets enhanced ratios, and meets enhanced space. Your facility uses Mother Goose Time Curriculum and Formative Assessment Tool. The Secretary of State website was checked on April 1, 2026, and your business, Preschool Connection, Inc., was active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today and all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment. Children throughout the facility were participating in routines, group time, outdoor play, and free play during the visit. The last sanitation inspection was conducted on March 6, 2025. A “superior” classification was issued with zero (0) demerits noted on the grade card. The last fire inspection was conducted on October 17, 2024. The last fire drill documented was conducted on November 25, 2024. The last emergency drill documented was conducted on December 18, 2024. The last playground inspection was conducted on April 9, 2025. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on August 16, 2024. Your facility completed a lead risk assessment report on November 22, 2024, and lead-based paint hazards were identified. You completed repairs and the occupant protection plan submitted on May 21, 2025, reported that all identified hazards had been mitigated and access to the areas was no longer restricted. Your facility completed a Asbestos Inspection Report on November 22, 2024, and no asbestos hazards were identified. Six (6) children’s files were reviewed during the visit. No new staff members were hired since the last visit. Two (2) existing staff files were reviewed during the visit. The following violation was observed/documented during today’s visit: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In Space #4, one (1) tube of Eucerin Baby sensitive Mineral Zinc Oxide Protection Sunscreen Lotion expired December 2024, and one (1) tube of Black Girl Kids Sunscreen expired March 2026. .0803(12) The violation documented must be corrected immediately. On or before April 15, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violation was corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Please send your letter to christine.rosinski@dhhs.nc.gov. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 100%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: 1. Any medication remaining after the course of treatment is completed, after authorization is withdrawn or after authorization or medication has expired must be returned to the child's parents. Any medication the parent fails to retrieve within 72 hours of completion of treatment, or withdrawal of authorization, should be discarded. I suggested you assign a staff member to each room to check all medications and Permission to Administer Medication Forms at least once a month to prevent further non-compliance regarding medication. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. You, Elizabeth Baucom, Assistant Administrator, assisted me with today’s visit. Your program currently operates with a five (5) star license issued January 8, 2018. The permit restrictions were in compliance including First Shift (daytime care), meets enhanced ratios, and meets enhanced space. The Secretary of State website was checked on September 23, 2024, and your business, Preschool Connection, Inc., was active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. The last sanitation inspection was conducted on October 24, 2023. A “superior” classification was issued with two (2) demerits noted on the grade card. The last fire inspection was conducted on February 16, 2024. The most recent fire drill was conducted on September 4, 2024, the most recent emergency drill was conducted on July 1, 2024, and the most recent playground inspection was conducted on September 3, 2024. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines and free play during the visit. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. One (1) new staff member was hired since the last visit. The following violation was observed/documented during the visit: Violation Number Comment Rule 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. In Space #4, the medical action plan for one (1) child diagnosed with food allergies was last updated on August 20, 2024. .0801(b) The violation documented must be corrected immediately. On or before October 7, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violation was corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. The medical action plan is to be completed by the child's parent or a health care professional and may include the following: a list of the child's diagnosis or diagnoses including dietary, environmental, and activity considerations that are applicable; contact information for the child's health care professional(s); medications to be administered on a scheduled basis; and medications to be administered on an emergency basis with symptoms, and instructions. The medical action plan is to be updated on an annual basis and when changes to the plan are made by the child's parent or health care professional. Continue to use the forms found on the DCDEE website under the “Provider” tab. I suggested you set calendar reminders to ensure medical action plans are updated at least once per year. I also suggested you keep a copy of the medical action plans with the medications to ensure the correct response and care is given to any emergent situations. Consultation Hold harmless has been extended until the new QRIS system is implemented. Programs still have the option to choose to voluntarily complete the star rated license reassessment if they so desire. The mulch on the playground under the stationary play structure measured two (2) to four (4) inches across the fall zone. The required depth of mulch based on the critical height of the stationary play structure is six (6) inches. .0605(k)(1-4) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Add mulch now and as needed to ensure an appropriate depth. I suggested you rake and fluff the mulch regularly when needed to ensure a proper depth. I also suggested you note the depth of the mulch on the monthly playground inspections to ensure the appropriate depth of mulch is maintained at all times.
