You, Angela Warren, Administrator, assisted with today’s visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five (5) star license issued September 5, 2017. The permit restrictions were in compliance including First Shift (daytime care), meets enhanced ratios, and meets enhanced space. The Secretary of State website was checked on October 14, 2025, and your business, Preschool Connection, Inc., was active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines, lunch, and nap during the visit. The last sanitation inspection was conducted on September 9, 2025. A “superior” classification was issued with four (4) demerits noted on the grade card. The last fire inspection was conducted on May 23, 2025. The last fire drill was conducted on October 3, 2025. The last emergency drill documented was conducted on May 9, 2025. The last playground inspection was conducted on October 2, 2025. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on April 16, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, an inspection was conducted of your facility on November 22, 2024, showing no lead-based paint or asbestos hazards identified. Two (2) children’s files were reviewed during the visit. Five (5) new staff files and one (1) existing staff file were reviewed during the visit. We reviewed your ABCMS Provider Portal. Two (2) staff members were entered; however, all current staff members were not listed. You stated you are having difficulty adding and removing staff members from the provider portal. I provided you with a copy of the ABCMS Provider Portal Access Guide via email during the visit. Please contact the Criminal Background Check Unit at (919) 814-8401 with any additional questions regarding the ABCMS Provider Portal. The following violations were observed/documented during today’s visit: Violation Number Comment Rule 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One (1) staff member, employed on October 14, 2025, did not have a TB test or TB screening on file. .0701(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The most recent shelter-in-place/lockdown drill was conducted on May 9, 2025. .0604(u);.0302(d)(8) The violations documented must be corrected immediately. On or before October 28, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Please send your letter to christine.rosinski@dhhs.nc.gov. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 97%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: 1. All staff, including the director and individuals who volunteer more than once per week must have the results indicating the individual is free of active tuberculosis on file on or before the first day of work. This must be obtained within the 12 months prior to the date of employment and must be on file on or before the employee’s first day of work. New staff members should not be permitted to begin work until they have the results of a TB test showing negative results or screening form on file. I suggested you review staff files regularly to ensure no documentation has been discarded or misplaced. I also suggested you use the staff file checklist found on the DCDEE website under the “Provider” tab to ensure all required documentation is on file. I provided you with a copy of the staff file checklist during the visit. 2. Shelter-in-Place or Lockdown drills must be completed at least once every three (3) months. I suggested you create calendar reminders at least one year in advance to ensure drills are conducted at least once every three (3) months. I suggested you document the drill immediately after the drill is conducted to ensure required documentation is on file and available for review. Continue to document drills on the drill log form found on the DCDEE website. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. Information on the three (3) pathway options, as well as links to upcoming webinars may be found on the DCDEE QRIS Modernization website: https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/QRIS-Modernization. We discussed changes to the environmental rating scales and the implementation of the “3’s”. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines, lunch, free play, nap and outdoor play during the visit. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. One (1) new staff members were hired since the last visit. The following violation was observed/documented during the visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. An approved fire inspection was conducted on December 15, 2022, and not again until April 29, 2024. 10A NCAC 09 .0304(a) 705 Equipment and furnishings were not sturdy, stable and free of hazards. On the ramp to the playground, one (1) yellow, plastic dump truck, was cracked causing sharp, plastic edges. .0601(c) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by June 26, 2024. The operator must notify the local fire inspector when it is time for the center's annual fire inspection. The operator must submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. I suggested you contact the Fire Inspector at least one (1) month prior to the due date of the next fire inspection to ensure an inspection is conducted in a timely manner. 2. The staff member conducting playground inspections should thoroughly check the playground to ensure equipment and furnishings are sturdy, stable, and free of hazards that may injure children including sharp edges, lead based or peeling paint, rust, loose nails, splinters, protrusions, and pinch and crush points. Teachers should check the playgrounds daily to ensure all equipment is safe and in good repair. Any items that are considered hazardous should be repaired or removed immediately. You removed the broken dump truck and discarded it during the visit. Consultation: You requested additional information on CPR and First Aid training. We reviewed child care rule 10A NCAC 09 .1102 Health and Safety Training Requirements: All staff who provide direct care or accompany children when they are off premises shall successfully complete certification in First Aid appropriate to the ages of children in care. The training shall be completed by June 30, 2018, or for new staff hired on or after September 1, 2017, training must be completed within 90 days of employment. Distance learning shall not be permitted for First Aid training. At all times when children are in care at least one staff member present must have successfully completed First Aid training, as evidenced by a certificate or card from an approved training organization. First Aid training shall be renewed on or before expiration of the certification. "Successfully completed" is defined as demonstrating competency, as evaluated by the instructor. Verification of each required staff member's completion of this course from an approved training organization shall be maintained in the staff member's file in the center. The Division shall post a list of approved training organizations on its website at http://ncchildcare.nc.gov/providers/pv_sn2_ov_pd.asp. The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year will not begin until July 1, 2024, the preparation year for cohort one begins July 1, 2023. Your facility is in cohort one. Beginning in July and throughout the next few months I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024 and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe • Have all staff update WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS) • Determine whether you wish to have Environment Rating Scale (ERS) Assessments [ITERS-R, ECERS-R, SACERS-U, FCCERS-R] conducted • Request technical assistance with your child care consultant and local partners • Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) • Reach out to your local Community College to discuss educational opportunities • Review NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets. A worksheet is available for each subscale of each rating scale. • Family child care home providers consider the FCCERS-R self-study process and timeline for completion since it must occur over a 3-month time period prior to your rated license reassessment • Consider requesting an ERS assessment (free of charge) during the preparation year. The most recent fire drill was conducted on June 1, 2024, the most recent emergency drill was conducted on May 30, 2024, and the most recent playground inspection was conducted on May 1, 2024. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. Children throughout the facility were participating in routines, lunch, free play, nap and outdoor play during the visit. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. One (1) new staff members were hired since the last visit. The following violation was observed/documented during the visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. An approved fire inspection was conducted on December 15, 2022, and not again until April 29, 2024. 10A NCAC 09 .0304(a) 705 Equipment and furnishings were not sturdy, stable and free of hazards. On the ramp to the playground, one (1) yellow, plastic dump truck, was cracked causing sharp, plastic edges. .0601(c) The violations documented above must be corrected immediately. The staff member conducting playground inspections should thoroughly check the playground to ensure equipment and furnishings are sturdy, stable, and free of hazards that may injure children including sharp edges, lead based or peeling paint, rust, loose nails, splinters, protrusions, and pinch and crush points. Teachers should check the playgrounds daily to ensure all equipment is safe and in good repair. Any items that are considered hazardous should be repaired or removed immediately. We reviewed child care rule 10A NCAC 09 .1102 Health and Safety Training Requirements: All staff who provide direct care or accompany children when they are off premises shall successfully complete certification in First Aid appropriate to the ages of children in care. The training shall be completed by June 30, 2018, or for new staff hired on or after September 1, 2017, training must be completed within 90 days of employment. Distance learning shall not be permitted for First Aid training. At all times when children are in care at least one staff member present must have successfully completed First Aid training, as evidenced by a certificate or card from an approved training organization. First Aid training shall be renewed on or before expiration of the certification. "Successfully completed" is defined as demonstrating competency, as evaluated by the instructor. Verification of each required staff member's completion of this course from an approved training organization shall be maintained in the staff member's file in the center. The Division shall post a list of approved training organizations on its website at http://ncchildcare.nc.gov/providers/pv_sn2_ov_pd.asp.
