J. Owens, office assistant, assisted me with today’s visit. L. Lewis, administrator arrived during the visit. Your program currently operates with a five-star license, issued 9/22/2021 earning 6 points in the education component, 6 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having a staff benefits package and an infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent as of 6/18/26. The NC Secretary of State website was reviewed on 6/18/26 and Antioch Church Ministries, Inc. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environment. In classroom space #1 children were observed transitioning from the outdoor space. Diapering and handwashing routines were observed. In classroom space #6 children were observed during morning group. The children were singing a good morning song, counting in English and Spanish, and singing a song about colors of foods as the teacher displayed pictures of colored fruits. Lunch was observed and included spaghetti, peas, mandarin oranges, and milk. The following violation was observed. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. In classroom space #1 one teacher cared for ten children two to three years of age. 10A NCAC 09 .2818 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 2, 2026 I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. You arrived during the visit, and I shared with you that there were ten children in the classroom space. You stated that the staff were supposed to move children, but you were not here and it did not happen. You also stated that you have children attending today who only attend on designated days. In any multi age group situation, the staff/child ratio for the youngest child in the group shall be maintained for the entire group. Review staff/child ratios with staff and set procedures for when you and your assistant director are offsite. Additional Comments: Transitions: Many transitions occur throughout the day between indoor and outdoor learning environments, activities, and routine transitions. Consider activities during this time, have a transition container of activities that can include books, puzzles, or manipulatives that can be easily sanitized if used directly after outdoor play. Having these items readily available with assist with wait time, and the children’s engagement during the transitions. In addition to activity container, songs and finger plays are also beneficial in transitions. The children engaged in variety of gross motor activities including playing on stationary climbers, playing in the sand table, throwing and kicking balls and riding on tricycles. Lunch consisted of sweet peas, pears, turkey corndogs, and milk. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Stored above five feet in an unlocked cabinet in classroom space #3 there was a container of ZEP wipes and on a shelf below five feet in a child's cubby there was a tube of Desitin. Store above five feet in an unlocked cabinet in classroom space #1 was a bottle soft Family Guard Disinfectant. .2820(b) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In classroom space #3 an authorization form for a tube of Desitin was not on file. 10A NCAC 09 .0803(1)(a & b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Have parents turn medications into the office to ensure authorization forms are completed as required. Staff Records: The health and safety of children requires that information regarding each staff working with the children be kept and available when needed. Staff records consist of various documentation such as employment application, medical report, TB test results, Annual Health Questionnaire, emergency contact information, CRC qualification letter, and documentation of orientation, in-service hours, CPR and First Aid, playground safety, and BSAC. One staff hired on 1/5/26 did not have a valid medical report on file or a completed emergency information form on file. Use the staff file checklist when hiring new staff to ensure all paperwork is received within the required timeframe. Additional Comments: During the visit an overview of section .3200 of the child care rules was provided. Each of the following pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. You informed me that your facility will pursue Pathway #1: Program Assessment Pathway. We reviewed the requirements for the three-month self-study process. Visit ncrlap.org under the resources tab for additional information on the self-study for centers. Education requirements were shared with you, and individual education staff were reviewed. Rated License Timeline: We discussed your three-month self-study June-August 2026 and submit your Rated License Application by September 2026. Clean Classrooms for Carolina Kids: Child care centers should be free of lead poisoning hazards. Every three years all water outlets used for drinking or food preparation shall be tested. Today we reviewed the facilities that lead in water, lead in paint and asbestos testing. Your last water testing was completed in 2021, you will to complete lead in water testing. Reminders: Be knowledgeable of the child care laws and rules and environmental health rules; review them with your staff and assist them with understanding and implementing them to maintain compliance and keep children safe and healthy. We discussed using the health and safety training record to track five year renewal dates. Children’s Files: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Organization and completeness of records is important to ensure easy access to information in emergency situations. Within in 30 days of enrollment a child must have a health assessment on file and each child must also have an immunization record on file. Review children’s files often and use the Center children’s checklist to ensure you have all required documents within the specified timeframe. Safe Sleep: Documenting the supervision that is provided for sleeping infants is one way to reduce the risk of Sudden Infant Death Syndrome (SIDS). The use of sleep charts is identified in your center’s ITS-SIDS policy. Sleep charts are part of the center’s program records and must be kept on file and available for review. The last annual compliance visit was conducted 1/24/24. The sanitation inspection was completed 9/11/24 with a “Superior” classification. During outdoor play the children engaged in gross motor activities included climbing on a stationary structure. L. Lewis, assistant director assisted me with today’s visit. Your program currently operates with a five-star license, issued 9/22/2021 earning 6 points in the education component, 6 points in the program standards component (meeting enhanced space and ratio) and 1 quality point for having a infrastructure of family involvement and staff benefits package. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety - three percent as of 6/20/24. The NC Secretary of State website was reviewed on 6/20/2024 and Antioch Church Ministries, Inc. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Contact me if you need to make any changes to your contact information, e.g., phone numbers, mailing address. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children enrolled in classroom spaces #1 and 3 were preparing for lunch. The children sat at child sized tables with the teacher. Children enrolled in classroom space e#6 were engaged in free play in designated learning areas. Children enrolled in classroom space #8 were observed in the indoor and outdoor environments. While inside the children engaged in free play with age-appropriate materials and completing bathroom routines. The teacher moved throughout the classroom space assisting the children as needed. During outdoor play the children engaged in gross motor activities included climbing on a stationary structure. The violations are as follows: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In classroom space #8 the floor panel was lifting creating a tripping hazard. The concrete base of the pole for the swings in outdoor space #1 was exposed. There is a gap in the bottom of the fence that separates outdoor space #1 and 2, creating a space for entrapment. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In classroom space #1 the cabinet was unlocked and stored inside was two containers of Zep Wipes. In classroom space #8 the closet in the bathroom was unlocked and stored inside was a container of ZEP wipes and a container of Great Value Disinfectant wipes. On two of the vans used to transport children hand sanitizer was stored in unlocked containers. .2820(b) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. One of eleven children being transported on van #2 did not have emergency and identifying information with a photograph. 10A NCAC 09 .1003(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 4, 2024 I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. You immediately contacted your maintenance personnel during the visit as items were brought to your attention. During the visit you informed me that maintenance fixed the classroom floor in space #8 by gluing the floor panel. Submit photos of the fence and swing post with your correction letter. Have maintenance do regular safety inspections to fix any hazards as they occur. Additional Comments: Medications-Medication administration errors and accidental exposure to medications can result in serious illness and even death in young children. It is important to follow all medication requirements to ensure safe storage and administration of medication. Today we reviewed the requirements for medications, including a review of information needed on the prescription label. Transitions- Children spend a lot of time transitioning throughout the day. Transitions can be stressful times for children and provoke challenging behaviors. Whole group transitions, that often include periods of waiting with nothing to do, can increase the likelihood of challenging behaviors. During these periods of transitions encourage staff to read books, sing songs, ask open-ended questions to promote dialog and social interactions, do finger plays, provide age-appropriate transitions manipulatives, and play age appropriate games. Remember waiting periods should last no longer than 3 minutes. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. Prior to being transported, permission must be granted by each child’s parent stating when and where the child will be transported to and from. Today three children did not have a signed permission to transport form on file. Each child transported must be have on file emergency contact information along with a photograph of each child. This is helpful in identifying children in the event of an emergency/accident. Three children being transported did not have emergency information on file with a photograph attached. Reminders: NCDHHS Children’s Environmental Health adopted new Child Care Sanitation Rules, effective July 1, 2023.
