L. Lewis, administrator assisted during the visit. Your program currently operates with a five star license, issued 9/22/2021 earning 5 points in the education component, 7 points in the program standards component(meeting enhanced space and reduced ratios) and 1 quality point for the child care administrator has at least 10 years of documented child care administration work experienced in a licensed program. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted 1/14/25. The sanitation inspection was completed 11/5/25 with a “Superior” classification. A fire inspection was conducted 1/30/24 and your facility was approved for day and night care. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-nine percent as of 1/2/26. The NC Secretary of State website was reviewed on 1/2/2026 and Antioch Church Ministries, Inc. was listed as current- active. Children enrolled in classroom space #1 were observed during free play in designated learning areas. Children engaged in fine motor play with a variety of manipulatives, practiced writing their names and played learning games on tablets. Children in classroom space #2 were observed transitioning from free play to breakfast. Breakfast was observed and consisted of cheerios, mandarin oranges and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance The current NC Pre-K staff are lead teachers J. Owens and T. Vaughan and teachers, P. Speight and N. Daniels. A selection of files were monitored for completed health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The first day of school was 9-3-25. The classroom operates from 7:30 am to 2:15 pm. Parent conferences are held two times per year. You stated that you use the Class DoJo app to communicate with parents. In addition, to a parent newsletter. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children engaged in gross motor activities including playing basketball, riding tricycles, and playing badminton. The following violations were observed. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not have a valid First Aid training on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not have a valid CPR training on file. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 26, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Being trained in CPR and first aid can be invaluable when someone is in serious medical distress. Therefore, it is so important that all staff working directly with children have current CPR and First Aid Certification. Staff T. Vaughn had a training certificate on file that indicated the staff completed the CPR/FA training online and was eligible for skills session within 90 days. I inquired if staff had completed the skills portion of the trainings. You informed me that the staff did not complete the skills testing. We discussed that the staff member would need to complete CPR and first aid training. Submit a copy of the staff training certificate or card as part of your compliance letter. Additional Comments: We discussed off-premises activities during today’s visit, we reviewed that activities planned should be posted in the classroom or shared with parents and must include the location, purpose, time and date, person in charge, and telephone number or method for contacting the person in charge. We reviewed the following sanitation rule: Lunches, snacks, and other meals that a child brings from home to the child care center shall be labeled with the date on which the food is brought to the child care center and the name of the child to whom the food belongs at the child's home and shall be returned to the child's home or discarded at the end of each day. Lunches, snacks, and other meals containing potentially hazardous foods shall be refrigerated at 45 degrees Fahrenheit or below and stored in the child care center kitchen or approved food preparation area. Hot foods that a child brings from home to the child care center in double-walled, insulated thermos containers may be stored outside of refrigeration at the child care center with the written permission of the child's parent or guardian. The last annual compliance visit was conducted 2/7/2024. The sanitation inspection was completed 9/11/24 with a “Superior” classification. The last fire inspection was conducted 1/30/24 and your facility was approved for day time care only. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eight percent as of 1/3/25. The NC Secretary of State website was reviewed on 1/3/25 and Antioch Church Ministries, INC. was listed as current-active. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The lead teacher in classroom space #2 was observed sitting at a child-sized table conducting an assessment with a child. You visited indoor and outdoor spaces with me. Children were observed during free play with a variety of age-appropriate materials including dramatic play and manipulatives. Teachers moved throughout the classroom spaces engaging in conversation and paly with the children. Today’s menu reflected peanut butter and jelly sandwich, carrots, mandarin oranges and milk. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted in classroom space #3 was dated 1/6/25-1/10/25. GS 110-91(12); .0508(a) 807 A safe indoor and outdoor environment was not provided for the children. There was hole in the ground at the fence line creating a space for entrapment. 10A NCAC 09 .0601(a) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The administrator did not complete the ABCMS Provider Portal training or complete the staff roster. G.S. 110-90.2 & .2703(r) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Staff N. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Recognizing and Responding to Suspicions of Maltreatment: Child care staff are expected to be well informed about child abuse and neglect issues and adequately trained to prevent and recognize potential maltreatment. Use the Health & Safety training record to record trainings on the five-year timeframe. Activity plans: Activity plans help teachers to plan for and implement daily activities for children.
