Your program currently operates with a four-star license effective 11/27/23 earning 7 points in the education component, 2 points in the program standards component (meeting enhanced ratios) and 1 quality point for meeting an education option. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on 9/18/25. The sanitation inspection was completed on 5/5/26 with a “Superior” classification. The last fire inspection was conducted 9/16/25 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The outdoor space includes stationary climber, ride on toys, tunnel, balls, and portable materials. Prescribed medications should be in the original container (with a label that includes the child’s name, date filled, prescribing clinician’s name, pharmacy name and phone number, dosage/instructions, and relevant warnings). A staff member stated the parent sent the medication in the Ziploc bag. In space#2 was an Epi-pen that expired on 12/25. Staff stated they had not realized that it had expired. Assign one staff member to receive medication from parents to ensure that all documents accompany the medication to the classroom. In space #1, the children were engaged in free play building a road for cars, looking at books, and completing puzzles. In Space #2, the children were engaged in large group time. The teacher read the story “Mommy is that you?”. In space #3, the children were engaged in large group time. The teacher reviewed numbers 1-10 on paper eggs and placed them on the floor for each child to see. The children each picked 2 plastic eggs, opened them to find a number, and when the teacher asked who had each number the children put the number in sequence. The outdoor space includes stationary climber, ride on toys, balls, stationary equipment and portable materials. Lunch was being prepared and consisted of sloppy joes on bun, French fries, pears, and milk. The following violation(s) were documented. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A medical action plan is required for children that need emergency medication, the form needs to be completed by the parent or a health care professional. In space #1, a child enrolled did not have a medical action plan on file for asthma. We discussed the importance of understanding the symptoms the child shows and the instructions to administer the inhaler that is on site for the child. I gave you a copy of a medication flyer to review when receiving medication to know how long the written authorization will be valid. You stated that you did not know the child needed a different medical action form for Asthma. Have one staff member receive medicine and check to see if everything is filled out correct. Emergency Drills - Practicing emergency drills help providers and children respond to emergency situations in a calm and safe manner. A shelter-in-a-place or lockdown drill must be completed every 3 months. A shelter-in-place drill was completed on 9/26/24, however the next documented drill was conducted on 1/23/25. You stated that you did not realize lockdown/ shelter in place drill needed to be completed every three months. Add dates to a calendar of when things need to be completed by as a visual reminder. Additional Comments: We discussed Clean Classrooms for Carolina Kids and you have completed the Lead in Water testing, lead-based paint and asbestos testing. No further action is needed. Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that you do not need to schedule a rated license assessment unless you voluntarily request one. The new QRIS has been approved by the Child Care Commission. The Division is working on implementation and trainings for the new system. The children throughout the center were engaged in morning free play, art activities, block play, large group time, and interacting with peers. Children in space #1 were dancing on the carpet. In space #2, children were playing a fine motor game at the table. Children in space #3 were coloring pictures at the table. The outdoor space included a stationary climber, portable materials, riding toys, and balls for the children to engage in gross motor activities. Lunch was observed and consisted of chicken tenders, whole grain bun, french fries, mixed fruit, and milk. The following violation(s) were documented. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A CBC must be completed prior to employee hire and every five years thereafter. Submissions can also be made six months before the expiration date listed on the current Qualification Letter. Background screenings are needed to provide a safe program environment, support children’s health and safety and are important for preventing child maltreatment in early care and education programs. Background screenings can help by identifying applicants with unsafe backgrounds and risk factors, such as a history of violent behaviors or drug use. We discussed using the Staff File Checklist to document dates to ensure that they are received on time. Staff Records: The health and safety of children requires that information regarding each staff working with the children be kept and available when needed. Staff records consist of various documentation such as employment application, medical report, TB test results, Annual Health Questionnaire, emergency contact information, CRC qualification letter, and documentation of orientation, in-service hours, CPR and First Aid, playground safety, and BSAC. Three (3) staff members hired on 8/12/24 had a medical statement and TB test on file for review; one staff member’s medical statement and TB test was dated 8/20/24; one staff member’s medical statement and TB test on file was dated 8/20/24; one staff member’s medical statement was dated for 8/16/24 and TB test was dated 8/19/24. You stated that it has been a while since you have hired any new staff and thought they had 30 days to complete the medical statement and TB test. Use the Staff File Checklist for Centers found on the DCDEE website to monitor ensure deadlines are met. Additional Comments: Required Water, Lead Paint, and Asbestos Testing - New child care rules require child care facilities to test for lead in drinking water, lead paint, and asbestos. Caregivers/teachers position themselves so that they can observe all the children: watching, counting, and listening at all times. Supervision is directly tied to safety and the prevention of injury and maintaining quality child care for infants, toddlers, preschoolers, and school-aged children. The caregiver assigned to space #3 left the classroom space and walked into space #2 to get a copy from the printer leaving eight (8) children 4-5 years of age unattended. We discussed that rather than leaving the classroom, communicate with the staff member in the other classroom space to assist them in what is needed. We also discussed that in the time that the caregiver was out of the classroom space, a child could have been injured and no one would have witnessed the incident. You also stated that when you post the ratio sheet, you will have a discussion with the caregiver about how they are unable to leave the classroom space as they are the staff meeting the ratio. Have a staff meeting with each of the staff members to discuss the importance of supervision and how to communicate when assistance is needed. Additional Comments: Rule Changes Effective January 2024 Training Modules Available in DCDEE Moodle The Child Care Commission adopted child care rule changes in January 2024. Changes relate to definitions; lead and asbestos for centers and family child care homes; building requirements for family child care homes; multi-unit child care centers; and criminal background checks. Consultants will assist as you begin to review and implement the changes, but please note, some of the rule changes may or may not impact your facility. An example is the rules in section .2600 for multi-unit child care center. These rules are specific to child care centers with multiple licensed centers within one building. Natural Learning Initiative: Time outdoors is a critical tool for supporting mental and physical health in young children. Adoption of best practices for outdoor play and learning spaces is critical to increase daily time spent outside. The four components of this project include: 1. Relaunch a supportive network for outdoor play and learning environments in North Carolina; 2. Research nature-based programs and spaces and develop a plan to embed these in licensed childcare in North Carolina; 3. Motivate and train early educators and specialists to promote best practices and increase time in outdoor play and learning in NC childcare facilities; 4. Increase time outdoors playing and learning in NC family child care homes through professional development and technical assistance. Today, we discussed ways to create a natural learning environment in the outdoor spaces. Some of the ideas included a music wall made from kitchen accessories, a garden that the children can help plant, a science center, and stepping stone made from logs.
