Your program currently operates with a 5-Star license issued October 11, 2018. The last annual compliance was conducted on November 12, 2024. The sanitation inspection was completed February 10, 2025, with a “Superior” classification. The last fire inspection was conducted on December 3, 2024. Your facility was approved for 1st shift care, the facility meets enhanced ratios, enhanced space, and enhanced ratios reduced by 1 in each age group. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-four (94) percent as of October 12, 2025. . The NC Secretary of State website was reviewed on November 5, 2025, and Carteret County Public School Foundation was listed as current-active. Twenty-eight (28) children, between the ages of three (3) and five (5) years old, are enrolled and twenty-two (22) were present today. There are two (2) approved indoor spaces and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. Preschool children transitioned into the indoor space from physical education. A staff member read the children a book during a group activity and the children transitioned to morning snack. After morning snack, the children transitioned to routine care, then the outdoor space. After playing outside the children had lunch which consisted of a turkey, lettuce, and cheese sandwich, spiced apples, and milk. After lunch the children had rest time. Routine toileting, handwashing, food service, and cot spacing requirements were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment including large legos, large waffle blocks, a sand box with dump trucks, sand toys, and a sand box covering, sidewalk chalk, stepping stones, riding cars, push toys, wagons, balls, a dramatic play sink, picnic tables, large interlocking manipulative pieces, an art easel, two (2) dramatic playhouses, and a large stationary climber with two (2) slides. Health and safety trainings were monitored, and requirements were met. Your program currently operates with a 5-Star license effective July 7, 2020. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97%) percent as of May 7, 2025. The license was posted, and the restrictions were in compliance. The most recent sanitation inspection was conducted on February 10, 2025, with a superior rating. The most recent fire inspection was conducted on October 31, 2024. There is one (1) approved indoor space and one (1) approved outdoor space. Eighteen (18) children between the ages of four (4) and five (5) are enrolled and seventeen (17) children were present today. A walk-through of the facility was completed today. All approved indoor and outdoor spaces being used by the children were observed. Indoor spaces are arranged into activity areas that include but are not limited to language, blocks, art, math, science, sensory, dramatic play, and manipulatives. A variety of age-appropriate learning materials were observed in each classroom. The attendance for the classroom was accurately completed. Daily schedules and activity plans were available for each group of children. Staff/Child ratios were in compliance and all children were adequately supervised. Good interactions were observed between caregivers and children. Upon entering your facility, you and the children were in the gym area for physical education activities. As the children transitioned back to the indoor space, the children washed their hands and prepared for morning snack. Routine handwashing, toileting, and food service procedures were monitored, and requirements were met. After the morning snack, the children transitioned to the outdoor space where they were observed riding tricycles on the bike trail, running, and transferring flowers into painted flower pots for Mother’s Day. A staff member discussed with the children that flowers need sun and oxygen to breathe. The other staff member played hide and seek with the children. The outdoor space had a variety of gross motor equipment which included push trucks, a large climber with two (2) slides, riding cars, tricycles, and wagons. Other outdoor activities included a large connect four game, large legos, large traffic signs, and two (2) playhouses. Health and Safety trainings and the Shaken Baby and Abusive Head Trauma Policy were monitored today, and requirements were met. Before children enter the outdoor area each day, you must take time to observe the outdoor area and ensure there are no safety hazards or debris in the area. If there are any hazards, they must be addressed before children can enter the area. As large areas of chipped paint were observed on the playground equipment, I suggest that you sand and repaint these areas on a regular basis. Your program currently operates with a 5-Star license effective July 7, 2020. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97%) percent as of May 7, 2025. The license was posted, and the restrictions were in compliance. The most recent sanitation inspection was conducted on February 10, 2025, with a superior rating. The most recent fire inspection was conducted on October 31, 2024. There is one (1) approved indoor space and one (1) approved outdoor space. Eighteen (18) children between the ages of four (4) and five (5) are enrolled and seventeen (17) children were present today. A walk-through of the facility was completed today. All approved indoor and outdoor spaces being used by the children were observed. Indoor spaces are arranged into activity areas that include but are not limited to language, blocks, art, math, science, sensory, dramatic play, and manipulatives. A variety of age-appropriate learning materials were observed in each classroom. The attendance for the classroom was accurately completed. Daily schedules and activity plans were available for each group of children. Staff/Child ratios were in compliance and all children were adequately supervised. Good interactions were observed between caregivers and children. Upon entering your facility, you and the children were in the gym area for physical education activities. As the children transitioned back to the indoor space, the children washed their hands and prepared for morning snack. Routine handwashing, toileting, and food service procedures were monitored, and requirements were met. After the morning snack, the children transitioned to the outdoor space where they were observed riding tricycles on the bike trail, running, and transferring flowers into painted flower pots for Mother’s Day. A staff member discussed with the children that flowers need sun and oxygen to breathe. The other staff member played hide and seek with the children. The outdoor space had a variety of gross motor equipment which included push trucks, a large climber with two (2) slides, riding cars, tricycles, and wagons. Other outdoor activities included a large connect four game, large legos, large traffic signs, and two (2) playhouses. Health and Safety trainings and the Shaken Baby and Abusive Head Trauma Policy were monitored today, and requirements were met. In the outdoor space, the green metal bench and the green metal poles for the large shade structure had large areas of chipped paint. The rubber border boundaries for the large sand box were not visible due to grass growth around the sandbox and sand covering the borders. Outdoor Space Hazards: Child Care Rule 10A NCAC 09 .0604 (p) SAFETY REQUIREMENTS FOR CHILD CARE CENTERS states that once a day, prior to initial use, the indoor and outdoor premises shall be checked for debris, vandalism, and broken equipment. Debris shall be removed and disposed of. The sandbox has debris growing around it and the covering is broken. Before children enter the outdoor area each day, you must take time to observe the outdoor area and ensure there are no safety hazards or debris in the area. If there are any hazards, they must be addressed before children can enter the area. As large areas of chipped paint were observed on the playground equipment, I suggest that you sand and repaint these areas on a regular basis. Chipped paint is considered a safety hazard as children may ingest it. The gravel surfacing under the large climber needs to be tilled at the slide landing area. Your program currently operates with a 5-Star license effective July 7, 2020, earning seven (7) points in the education component, seven (7) points in the program standards component, and one (1) quality point for a programmatic option. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97) percent as of April 23, 2024. The NC Secretary of State website was reviewed on April 23, 2024, and Carteret County Public School Foundation, was listed as current-active. The license was posted, and the restrictions were in compliance. Administrator, Charity Clemmons, was available for consultation during the visit. There is one (1) approved indoor space and one (1) approved outdoor space. Sixteen (16) children between the ages of four (4) and five (5) are enrolled and sixteen (16) children were present today. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in your classrooms. Age-appropriate materials and activities were provided for all children. A walk-through of the facility was completed today. All indoor and outdoor areas were monitored. An activity plan was observed posted in the classroom. Upon entering the outdoor space, children were observed running, climbing, sliding, and riding tricycles and cars. Water was available for the children in the outdoor space. The children transitioned to the indoor space for lunch. Lunch consisted of cheeseburgers, broccoli, diced peaches, and milk. Toileting, handwashing, and food service procedures were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment which included large legos, large waffle blocks, a sand box with dump trucks, sand toys, and a sand box covering, sidewalk chalk, stepping stones, riding cars, push toys, wagons, balls, a dramatic play sink, picnic tables, large interlocking manipulative pieces, an art easel, two (2) dramatic playhouses, and a large stationary climber with two (2) slides. Health and Safety trainings and the Shaken Baby and Abusive Head Trauma Policy were monitored today, and requirements were met. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. In the outdoor space, the sand in the covered sand box area covered the majority of the sand box barrier. Only a small area of the barrier was visible around the sand causing a tripping hazard for the children. .0601(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. Any violations documented must be corrected immediately. On or before Monday, May 6, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violation was corrected. For the safety of the children, you must ensure weeds and debris are removed and sand does not cover the barriers of the sand box. I also urged you to create a regular schedule for maintenance of the sandbox. To verify compliance, please submit a picture of the sand box showing the tripping hazard has been addressed. Rated License: Your facility’s three-year rated license reassessment was due to be completed in March 2021 but was not completed due to reassessments being voluntary during the COVID-19 pandemic. As we discussed today, reassessments will not become mandatory again until July 1, 2024, and your facility has been placed into “Cohort 1.” Placement in this cohort means you will have from July 1, 2023, to June 30, 2024, to plan and prepare for your reassessment, with your reassessment being due between July 1, 2024, and June 30, 2025. Your program currently operates with a 5-Star license issued July 7, 2020. The last annual compliance was conducted on November 21, 2022. The sanitation inspection was completed April 5, 2023, with a “Superior” classification. The last fire inspection was conducted on October 18, 2023. Your facility was approved for 1st shift care, the facility meets enhanced ratios, enhanced space, and enhanced staff/child ratios reduced by 1 in each group. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight (98) percent. The NC Secretary of State website was reviewed on November 15, 2023, and Carteret County Public School Foundation was listed as current-active. Sixteen (16) children, between the ages of four (4) and five (5) years old are enrolled and sixteen (16) were present today. There is one (1) approved indoor space and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in all classrooms. Age-appropriate materials and activities were provided for all children. Preschool children were engaged in a variety of free play activities. Free play activities included sand play with farm animals, barns, and windmills, kinetic sand and feathers, water color painting, scarf play while listening to music, puzzles, dressing baby dolls, cooking, and painting with kwik stix tempura paint. One staff member seated on a large group time carpet, read a book to a group of children. Both staff members interacted with the children as they played. Toileting and handwashing procedures were monitored, and requirements were met. The outdoor space had a variety of gross motor equipment including large legos, large waffle blocks, a sand box with dump trucks, sand toys, and a sand box covering, sidewalk chalk, stepping stones, riding cars, push toys, wagons, balls, a dramatic play sink, picnic tables, large interlocking manipulative pieces, an art easel, two (2) dramatic playhouses, and a large stationary climber with two (2) slides. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 NC Pre-K Monitoring: The NC Pre-Kindergarten Program (NC Pre-K) Site Monitoring Tool dated October 24, 2023, was reviewed today, and the NC Pre-K Contracting Agency has conducted and completed the monitoring visit. The program follows the Carteret County school calendar. The length of the day, 8:00am until 3:00pm, meets the instructional day requirement of 6.5-10 hours. Child Assessment and Required Screenings: The assessment tool, GOLD, Teaching Strategies 2010-2011, is in place. Portfolios for each child are maintained by TS Gold online application and the teacher shares samples with each parent electronically. The facility uses the developmental screening tool, DIAL-4 for pre-screening and ongoing assessments. Documentation of health assessments, vision, hearing, dental screenings, and DIAL-4 pre-screenings were on file for each child. Curriculum: The Creative Curriculum for Preschool, 6th Edition, Teaching Strategies, 2016 has been implemented and the classroom has the required resources available for use. Materials to support the current activity plan were displayed throughout the classroom. Family Engagement: Parent involvement includes open house, daily DoJo online application postings, an upcoming Thanksgiving skit, and parent teacher conferences are held twice a year. Staff Requirements: All staff met NC Pre-K education requirements. Technical Assistance: Safe Indoor Environment: A safe indoor and outdoor environment must be provided for the children in care. All equipment and furnishings should be in good repair. Equipment and furnishings should be sturdy, stable, and free of hazards that may injure children including sharp edges, lead based or peeling paint, rust, loose nails, splinters, protrusions (excluding nuts and bolts on sides of fences), and pinch and crush points. As we discussed, the rubber caulking that has lifted around the handwashing sink needs to be replaced. Outdoor Climbing Equipment: Dirt was observed on the slides on the climbing equipment. To prevent any possible hazardous residue, build up on your playground equipment, I suggest you have the equipment power washed.
