Your facility operates as an NC PreK program in the following spaces: five, where children participating in the NC Pre-K program are cared for. Kim Barnes, Child Care Consultant, assisted with today's visit. Upon arrival, You, C. Arrington, Administrator, was present and available to assist with today’s visit. Your program currently operates with a five-star license issued on June 27, 2025, earning 7 points in the education component, 6 points in the program standards component and 1 quality point for the Administrator has completed a business training course of at least 30 clock hours. The last annual compliance visit was conducted on June 3, 2025. The last sanitation inspection was completed on December 31, 2025, with a “Superior” classification “0” demerits. The last fire inspection was conducted on April 30, 2026, and your facility was approved for daytime and night care. The license was posted, and the restrictions were in compliance. Thirty-one (31) children were present from infants to five years old. The children were observed in free-play in centers, music and movement, engaging in gross-motor play, in the outdoors, and personal care routines. Staff were observed interacting with the children in a positive and nurturing manner. The children were served the following for lunch: pizza, green beans, mixed fruit and milk. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. The following violations were documented during this visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was completed on April 30, 2026; however, the completed inspection was not sent to the child care consultant within seven (7) days of the inspection being completed. 10A NCAC 09 .0304(a) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. You contacted fire extinguisher service company to have the fire extinguisher serviced. STAR RATED LICENSE: You have decided to complete Pathway One: Program Assessment. Originally you wanted to complete Pathway Two but didn’t realized you would have to reduce your ratios by at least one child to meet the required reduced enhanced ratios. We reviewed the NC Rated License Assessment Project (NCRLAP) and reviewed the self-study forms. You are going to be working on your self-study from May through August and request the environment rating scales in August. REMINDERS: CLEAN WATER FOR US KIDS: We discussed the Clean Water for Carolina Kids program. According to the Clean Water for Us Kids website you last tested the drinking water on March 27, 2024. C. Arrington, Administrator, was present and available to assist with today’s visit. Your program currently operates with a five-star license issued on November 27, 2017, earning 7 points in the education component, 6 points in the program standards component, and 1 quality point for having an administrator who has completed a business training course of at least 30 clock hours. The last annual compliance visit was conducted on June 3, 2025. The sanitation inspection was completed on May 8, 2025, with a “Superior” classification “0” demerits. The fire inspection was completed on May 16, 2025, and the center was approved for daytime and nighttime care. The license was posted, and the restrictions were in compliance. Eighteen (18) children from ages one (1) to five (5) years old were present during the visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were engaging in art activities, group time, free-play, and individualized activities with the teacher. The following items were monitored: Supervision, Staff Child Ratio, First Aid, CPR, ITS SIDS, Special Training, CBC Qualification, Emergency Medical Care Plan, Storage of Hazardous Substances, Administration of Medication, Adequate/Approved Space, Permit Restrictions, and Program Records. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. The following violations of the child care requirements were documented today: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. R. NC PRE-K REQUIREMENTS: This program currently has one NC Pre-K classroom serving a total of nine children. Lesson plans for December 2023 to March 2024 were reviewed today. Children were engaged in activities documented on the lesson plan posted. Staff observed working in the classroom was the same as listed in the NC Pre-K plan. Staff interacted with children in a positive and nurturing manner. You reported the program continues to use the Creative Curriculum and Teaching Strategies Gold. You added the program also uses the Foundations for Early Learning in conjunction with the Creative Curriculum. Ten percent of children’s file were monitored. Developmental screenings using the Ages and Stages were completed and on file in children records reviewed today. The program’s monitoring tool dated October 31, 2023, was reviewed, and no concerns identified. Menus for the months of December 2023 through March 2024 were posted and reviewed. MEDICATION REQUIRMENTS: I explained medication authorization forms should include specific information as listed in child care rule 10A NCAC 09 .0803 (2) (b). The medication authorization form on file did not include the child’s name or the name of the medication on site. It was further explained that the authorization form does not indicate which child the medication should be administered to and what medication to provide. You stated when the form was completed and submitted, the medication was not available. It was recommended you and staff review the form upon receipt to ensure all information is included. If information is missing, contact the parent to update the medication authorization form. LEAD WATER/PAINT AND ASBESTOS: Ensure you review the information provided concerning Lead in Water/Paint and Asbestos. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. CHILDREN’S ORAL HEALTH: We discussed the newsletter and resources sent out via email by the Child Care Health Consultant regarding children’s oral health. The information will help early child care educator’s understand the importance of children’s oral health and could be shared with parents. The Child Care Health Consultant is offering an upcoming training. The training topic is Stay Healthy, Stay Clean (Handwashing, Diapering and Food Handling in Child Care). This 2-hour training will focus on handwashing, diapering, and food handling procedures. The training is scheduled for March 21, 2024, 6 pm – 8 pm, Braswell Memorial Library. Upon arrival, W. Hagans, Lead Teacher assisted with the visit. You (C. Arrington), Administrator arrived shortly after and assisted. This center currently operates with a five-star license, earning seven points in the education component, six points in the program standard component and 1 quality point for the administrator completing a thirty-hour business training course. The last annual compliance visit was completed on July 20, 2022. This program continues to meet enhanced space and ratio requirements. The last sanitation inspection was completed on December 29, 2022, with a “Superior” classification. The last fire inspection was completed on August 16, 2022, and the center was approved for daytime and night care. Fourteen children were observed in care today ranging from ages zero to seven years old. Children were engaged in morning activities, free-choice play with developmentally age-appropriate materials, outdoor play, completing hand washing and toileting routines, eating lunch, and interacting with the caregivers. Lunch consisted of meatballs, crackers, blueberries, strawberries, and milk. The center’s compliance history score was reviewed with the operator. You added the child would be transitioned back to the typical classroom to ensure staff/child ratio requirements are met. Be reminded it is important to monitor ages when grouping children. To ensure parents understand the child care requirements for staff/child ratios, the information could be reviewed during enrollment. This would be a good time to explain your program is currently meeting enhanced staff/child ratio requirements and explain the difference. I shared and discussed Child Care Rule 10A NCAC 09 ;0713 (6) regarding grouping children between the ages of 12- to 24-month-old with other children. UNANNOUNCED FOLLOW-UP VISIT: I informed you an unannounced follow-up visit will be conducted in the future to monitor staff/child ratio. TRANSPORTATION REQUIREMENTS: You stated the program uses three vehicles to transport children; however, during the summer months only one vehicle is used. You added the other vehicles are kept at a different location during the summer months to prevent vandalism. The white Dodge Grand Caravan was monitored today. I informed you the other two vehicles would be monitored for compliance during future visits. The vehicles will need to be at the facility for monitoring during that time. You should monitor the other two vehicle to ensure all safety and transportation requirements are met prior to using for transporting children. Be reminded you are responsible for always maintaining compliance with transportation and safety requirements. EXPANSION PLANS: You stated you were planning to add two additional classrooms to your building. You added the toddler class will also be enlarged if possible. We discussed contacting the required agencies (zoning, fire, sanitation, and building) prior to making changes to ensure approval is granted. You stated an architect has been retained to assist with determining and guiding the structure of the expansion, then zoning, building, sanitation, and fire will be contacted. You stated the process should take approximately two months from the first meeting with the architect. I informed you once expansion has been completed and classroom spaces are set-up, a visit would be conducted to measure spaces and increase capacity. You must notify me of completion to ensure a visit is scheduled. STAR RATED LICENSE: I informed you information regarding the preparation for the star rated license assessment process is located on the Division of Child Development (https://ncchildcare.ncdhhs.gov/) and North Carolina Rated License Project website.
