G. Nelson, owner, assisted me with the visit. This facility was issued a temporary license on February 11, 2026, caring for no more than 66 children, 3 -12 years of age on first shift. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and determined no changes were required. Contact me anytime you need to make any changes to your contact information, e.g., phone numbers, mailing address. This facility is owned and operated by Tiny Hearts Child Care Center, LLC. The NC Secretary of State website was reviewed on 5/11/26 and the LLC was listed as current- active. The children entered the facility and completed hand washing activities before entering the classroom. The group leader and the children engaged in an activity where they used sign language to spell their names. Snack included vanilla wafers, strawberries, craisins, yogurt and milk. During snack the group leader and children talked about events that occurred during their school day. The fenced outdoor spaces located at the rear of the facility include portable play equipment as well as a shaded play space. Staff, children, and program records were monitored. Compliance History - Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The compliance history for this facility prior to today’s visit is 98%. The violation(s) documented may impact the compliance history score and must be corrected immediately. The following violations were documented and corrected during the visit. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. The annual health questionnaire for one employee hired on 6/12/23 expired on 4/23/25. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The annual emergency information form for one employee hired on 6/12/23 expired on 4/23/25. .0701(a) * Technical Assistance with Documented Violations: Emergency Information and Staff Health Questionnaire – The Emergency Information and Health Questionnaire forms must be completed for all staff by day one of hire and updated as changes occur or at least annually. Both forms for one employee hired on 6/12/23 expired on 4/23/25. Both forms for one employee hired on 4/25/25 expired on 4/9/26. Both employees updated the forms on 5/11/26. We discussed using your Staff and Training Worksheet to track all staff file expiration dates. We also discussed having everyone at your facility complete annual updates at the same time each year to simplify tracking the review dates. You indicated you will post the staff & training worksheet in your office to assist you with creating a digital tracking tool. Rated License Discussion: Discussions occurred today regarding the Pathways to the Stars. You indicated Pathway 2 -Classroom and Instructional Quality is your choice. We reviewed the Application for Assessment for a Rated License for Centers. Each tab within Pathway 2 was opened and reviewed. Please submit the application and supporting documents by June 30, 2026. Facilities must be issued at least a Three Star License to be eligible to continue receiving subsidy after the six-month temporary time. Ensure you document new staff on the staff and training worksheets for future monitoring. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
