Your program currently operates with a one-star license, issued May 30, 2012. The last annual compliance visit was conducted July 14, 2025. The family child care home's compliance history was reviewed with the operator. The program’s compliance history was ninety four percent as of June 22, 2026. Corporation Status: Per your statements and according to the NC Secretary of State's website, this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. We also reviewed the FCCH and Centers Located in a Residence Operator’s Statements of Responsibility and FCCH and Centers Located in a Residence Verification of Required Information for Operator and Additional Caregivers forms. Upon arrival, you were present in the kitchen with the children. Children assisted you in measuring and mixing pancake batter. Those not helping with meal prep play with kinetic sand. The group discussed activities they enjoy, how to properly cover your mouth and nose when you cough and sneeze, what they saw on the trail cam, and the Fourth of July. The group transitioned to engaged in indoor free play including engaging with dramatic play materials, kinetic sand, and the balance beam. You moved about the space, assisting children with the activities and engaged in conversation with the children. Proper handwashing practices were observed. The following violation was observed. Violation Number Comment Rule 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. The ITS-SIDS training certification on file expired on January 24, 2026. You stated that you did not realize the training had expired. Additionally, you shared that a child had moved the pages in you’re the notebook used to store your records, as you were attempting to purge old documents, causing disorganization that you have not fully reorganized, and the training expiration was missed unintentionally. Today we discussed your tracking and organizational practices. You may use the FCCH and Center Located in a Residence Verification of Required Information for Operator and Additional Caregiver as a tracking tool. Highlight the expiration dates to help attract your attention to them. Performing diligent background screenings also protects the child care facility against future legal challenges. All owners of licensed family child care home and household members, 16 years of age and older are required to submit a Criminal Background Check (CBC) and receive their Qualifying Letter and complete a five-year requalification (on-line application and fingerprints) so that the new Qualifying Letter is on file before the expiration date listed on their current Qualifying Letter. An individual’s current Qualification Letter will be extended 2 years for the total of 5 years. New Qualification Letters will not be generated. Your program currently operates with a one-star license, issued 5/30/12. The last annual compliance visit was conducted 9/13/22. The family child care home's compliance history was reviewed with the operator. The program’s compliance history was ninety four percent as of 8/18/23. Corporation Status: Per your statements and according to the NC Secretary of State's website, this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding the ownership of this facility. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. The group engaged in indoor free play including a group game of The Floor is Lava, gross motor tunnel, and art with crayons, paper, and tape. You moved about the space, assisting children with the activities and engaged in conversation with the children. Learning material were in bins on low lying shelves for easy access. The outdoor area provided riding toys, balls, and a portable climbing structure.
