Limited child care requirements pertinent to this visit type were monitored. Your facility operates with a Four-Star License issued February 29, 2024, earning 7 points in Education, 2 points in Program, and 1 quality point for choosing an Education Option. Restrictions on the license include daytime care only, capacity of 8 children aged 0-12, maximum of 5 preschool children at any time, fireplace/woodstove not used during operating hours, and children in care on ground level only. The license was posted, and the restrictions were in compliance. The last annual compliance was conducted at this facility on August 11, 2025. As of March 17, 2026, your facility has an 18-month compliance history of 96%; this information was reviewed with you during today’s visit. The NC Secretary of State website was reviewed today and the owner of this facility, E-Ma’s Family Child Care Home LLC, was listed as current and active. Owner/Operator, Kathi Davis, was present and available for consultation. There were three preschool children present today; children were observing nap/quiet time. The children were observed laying on covered cots with blankets napping or playing quietly. After nap, child completed handwashing routines and had snack. Snack consisted of bananas, crackers, and milk. All areas used by the children were monitored. Age-appropriate materials and activities were observed in the child care space. A daily schedule and current activity plan were observed posted. The information on the Verification of Required Information for Operator and Additional Caregivers Form was verified and/or updated as needed. During today's visit child care requirements monitored for compliance include supervision, staff/child ratio, CPR and First Aid coverage, ITS-SIDS, Criminal Record Checks, storage of hazardous substances and medications, approved space, licensing requirements, staff, and program records, posting of the license, and other applicable requirements. There was one violation observed today. Violation must be corrected immediately. Continued compliance with applicable child care requirements is always required. Violation Number Comment Rule 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. ITS-SIDS training for operator expired on February 27, 2026. .1703(a)(4) For your facility to remain in compliance with NC Child Care Requirements, violations must be corrected immediately. On or before March 31, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected as well as how compliance will be maintained in the future. Technical Assistance regarding violation cited: Family child care home operators licensed to care for infants 12 months and younger must: Complete the ITS-SIDS training again no more than 3 years from the completion of previous ITS-SIDS training. ITS-SIDS training for Ms. Davis expired 02/27/2026; it should have been renewed on or before the date of expiration. During today’s visit, Ms. Davis contacted Region 4 to register for an ITS-SIDS training scheduled for tonight 630pm-830pm. Choosing a Pathway to the Stars: Your facility operates with a Four-Star License issued February 29, 2024. A QRIS Conversation Template form for your facility was completed today. Ms. Davis reported that she choose Pathway 2 - Classroom and Instructional Quality, and plan to apply for a rated license by August 2026. Pathway 2 – Classroom and Instructional Quality: You are eligible for this pathway, which allows your program to demonstrate quality through smaller groups of children, child-focused planning, approved curriculum implementation, child observations, training and coaching for staff, CQI plans, and family engagement. Family and Community Engagement: The facility must complete the Family and Community Engagement Standards Worksheet selecting the applicable items Educational Opportunity, Engagement & Leadership, and Communication. Items selected must be verifiable. Continuous Quality Improvement (CQI): A CQI plan must be completed for the facility and each staff member. The CQI Plan must include the following: Identification of goal(s), Necessary resources, supports and actions needed to accomplish the goal(s). Document the reason for the goal(s), Annual review of efforts toward completion of the goal(s). During today's visit, we reviewed your education information in WORKS. You have a bachelor’s degree in early childhood education. Ensure all your information and education has been submitted to DCDEE WORKS. This is of vital importance as the information in WORKS for your staff will determine the number of points that your facility earns in the education component of QRIS. We understand this is a big change and are committed to ensuring all providers have a good understanding of the new opportunities to make informed decisions on which pathway best suits the needs of the facility. Other Information Health and Safety Training - These training topics outlined in Child Care Rule 10A NCAC 09 .1102 must be completed within the first year of employment for new staff, except for Recognizing and Responding to Suspicions of Child Maltreatment – it must be completed within 90 days of hire. You completed these trainings in September 2022 and must be renewed by September 2027. These required trainings must be renewed every five years. Use the Health and Safety Training Record which is found on the DCDEE website under Provider Documents and Forms to track the completion of these trainings. Certificates will need to be attached to the training record for verification purposes. Ensure this information is maintained on file. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. Restrictions on the license include daytime care only, capacity of 8 children ages 0-12, maximum of 5 preschool children at any time, fireplace/woodstove not used during operating hours, and children in care on ground level only. As of August 19, 2024, your facility has an 18-month compliance history of 95%; this information was reviewed with you during today’s visit. Owner/Operator, Kathi Davis, was present and caring for the children. There were four (4) children ranging in ages of two (2) through ten (10) years of age enrolled and all were present today. Ms. Davis’s school age child, age 8, was also present today. All areas used by the children were monitored. Age-appropriate materials and activities were provided for all children. A daily schedule and activity plan was observed posted. Children were observed participating in free choice activities. Free choice activities included playing with toys and the older children were playing games with the alphabet, guessing game, and “hot potato.” Children also completed handwashing routines and had lunch. Lunch consisted of lasagna, bake beans, watermelon, garlic bread, and milk. The children were adequately supervised. Caregiver and children interacted in a positive and appropriate manner. During today’s visit, a copy of the FCCH Operator’s Statements of Responsibility and the Verification of Required Information for Operator and Additional Caregivers Form was completed. There were three violations observed today. All violations must be corrected immediately. Continued compliance with applicable child care requirements is required. Restrictions on the license include daytime care only, capacity of 8 children ages 0-12, maximum of 5 preschool children at any time, fireplace/woodstove not used during operating hours, and children in care on ground level only. The license was posted, and the restrictions were in compliance. This facility was license on August 28, 2023. As of August 19, 2024, your facility has an 18-month compliance history of 95%; this information was reviewed with you during today’s visit. The NC Secretary of State website was also reviewed on August 19, 2024, the owner of this facility, E-Ma’s Family Child Care Home LLC was listed as current and active. Owner/Operator, Kathi Davis, was present and caring for the children. There were four (4) children ranging in ages of two (2) through ten (10) years of age enrolled and all were present today. Ms. Davis’s school age child, age 8, was also present today. All areas used by the children were monitored. Age-appropriate materials and activities were provided for all children. A daily schedule and activity plan was observed posted. Children were observed participating in free choice activities. Free choice activities included playing with toys and the older children were playing games with the alphabet, guessing game, and “hot potato.” Children also completed handwashing routines and had lunch. Lunch consisted of lasagna, bake beans, watermelon, garlic bread, and milk. The children were adequately supervised. Caregiver and children interacted in a positive and appropriate manner. During today’s visit, a copy of the FCCH Operator’s Statements of Responsibility and the Verification of Required Information for Operator and Additional Caregivers Form was completed. There were three violations observed today. All violations must be corrected immediately. Continued compliance with applicable child care requirements is required. Clean Classrooms for Kids Program – All child care programs must have their water tested for lead and your building tested for lead and asbestos hazards. May 1, 2024, was the initial deadline to enroll your facility in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. Facilities that have not yet enrolled should complete the process by November 1, 2024. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years(centers) and to have inspections for lead paint and asbestos if applicable.
