A checklist was used to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 7/29/25. Currently this program operates with a five (5) Star License, issued 9/5/17. The facility earned five (5) points in staff education, seven (7) points in program standards, and one (1) quality point for programmatic options met. Holly Price and Latoya Glover assisted me today. I conducted an assisted walk-through of the classrooms. The last sanitation inspection was completed 3/10/26 with a ‘Superior’ classification and 10 demerits. The last fire inspection was conducted on 9/24/25. Children were observed playing in activity areas, group activities, lunch, playing outside, and routine care activities. Transportation is provided for school-age children. The center uses two vans, NX 1851 and RBM 3897, for transportation. Both were on site and monitored today. Today a partial assessment of applicable childcare requirements was conducted. Staff files were not monitored today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Three children transported did not have photographs attached to their emergency medication. 10A NCAC 09 .1003(d) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Price. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 4/16/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. CONSULTATION: I will conduct a follow-up visit within two weeks to review the staff files. Once you have completed the staff worksheets, email them to me. Rated License Assessment: We reviewed the requirements for Pathway one and two. You are completing an outreach assessment with NCRLAP. The window for assessment is 4/20/26-5/15/26. Once you receive your scores, please let me know and we will review them. At that time, you will decide which pathway you will choose. Family and Community Engagement: We reviewed this form today and discussed the options. At the four-star level, you will choose one option from each category. Continuous Quality Improvement: All staff should complete items 1-4 on the form and have it available for me to review during the scheduled assessment review meeting once the scores are received. Ms. Price will also complete items 1-4 on the CQI for the center. We reviewed the process for the self-study. If you choose pathway one, we can schedule a TA visit with the staff to review the process. Education: All staff are in WORKS. I will obtain copies of the letter for each staff member at the time we complete the rated license. Based on current lead teachers, it looks like you are at four stars for education. If you decide to choose pathway 2, you will need to purchase a curriculum for the school-age children. We reviewed these options today. You will also need to use a formative assessment for all preschool children enrolled. We reviewed the choices today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current inspection dated 10/28/24 was not received within 364 days of the previous inspection completed 10/25/23. 10A NCAC 09 .0304(a) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. CONSULTATION Once QRIS is implemented, you will receive information from DCDEE regarding the new pathways for licensure and your timeframe for completing your license renewal. The NCRLAP website, https://ncrlap.org, has trainings geared towards the new ECERS3 and ITERS3 assessment scales. These will introduce you to the updated assessment scale and provide tips for completing the new scale. We discussed the annual review of the center’s Emergency Preparedness Plan. The plan is registered under the director’s personal NCID. Ms. Price is the administrator and she does not have the NCID to access the center’s plan. You will need to contact NC Risk Management at 800-367-2229. The website is https://rmp.nc.gov. Once you call for assistance, explain that you are the director and you are trying to access the center’s EPR plan. You will explain that you do not have the NCID the plan was created under and will need to change it your NCID. If you do not see your plan once the NCID has been switched, it may be archived and the Risk Management team can move it back to active. We discussed the storage of emergency medications today. If a child has an action plan, check to see if an over-the-counter medication is listed. This medication will need to be stored in a small locked box and stored with the unlocked emergency medications. The location of the emergency medications should be clearly labeled in the child’s classroom. It is best practice to attach the child’s photograph to the medical action plan. We discussed the staffing requirements for a staff member under the age of eighteen. "Teacher's aide" or "Aide" means a person who assists the lead teacher or the teacher in planning and implementing the daily program. A teacher's aide shall be at least 16 years old and less than 18 years old, shall be literate, and may count in staff/child ratio as long as there is also a credentialed staff person who is at least 21 years of age present in the room and available to respond to the needs of the teacher's aide and children in care. Upon my arrival I was greeted by the Director, Holly Price, she assisted me during today’s visit. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed participating in free play outdoors and indoors. Both groups of children observed outdoors were playing with bubbles and interacting with the teachers. One group on children were observed having a group time discussion. The school age children were on a field trip to a park in a neighboring city. They were observed arriving back to the center during today’s visit. One diaper change was observed, and the correct procedures were followed. The children were observed eating lunch which consisted of beans, bananas, bread, bologna and milk. The last annual compliance visit was conducted September 29, 2023. The center is operating five star license that was issued September 5, 2017. The license was earned with 7 points in program standards, 5 points in education and 1 quality point for having enhanced policies and an infrastructure of parental involvement. All permit restrictions were in compliance The last sanitation inspection was conducted February 6, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted October 25, 2023. The Department of the Secretary’s website was used to check the status of the corporation. The facility’s owner is current/active. All the required documents were posted. Your compliance history is 98%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. The plan shall include the location, purpose, time and date, person in charge, and telephone number or method for contacting the person in charge. 10A NCAC 09 .0801 APPLICATION FOR ENROLLMENT All children with chronic health conditions must have a medical actin plan on file. (b) For any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be attached to the application. The medical action plan shall be completed by the child's parent or a health care professional and may include the following: (1) a list of the child's diagnosis or diagnoses including dietary, environmental, and activity considerations that are applicable; (2) contact information for the child's health care professional(s); (3) medications to be administered on a scheduled basis; and (4) medications to be administered on an emergency basis with symptoms, and instructions. The medical action plan shall be updated on an annual basis and when changes to the plan are made by the child's parent or health care professional. The facility currently operates with a five star license that was issued on September 5, 2017. The last annual compliance visit was conducted on September 29, 2023. The Director was present and assisted me during the visit. Children were observed participating in free play in there classrooms and outside. The infants were observed in their classrooms participating in free play and interacting with the teacher. The last sanitation inspection was completed February 6, 2024 with a “Superior” classification. The last fire inspection was conducted October 25, 2023. There was two new staff and their files were monitored. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violation. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. A parental authorization for an asthma inhaler had expired in 2/2024. 10A NCAC 09 .0803(4)(6-9) The violation(s) must be corrected immediately, and a letter must be sent to me no later than March 29, 2024 stating how each item was corrected and how you plan to maintain compliance with the requirement. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Consultation Be sure to use the correct medication authorization forms. Medication forms can be found on the DCDEE website. We discussed information regarding the prep year Environment Rating Scale assessment and staff education. If you are interested in completing a prep year ERS assessment, please contact me for the necessary paperwork. The prep year assessment would need to be requested before June 30, 2024. Resources The Division of Child Development has announced their plan to resume the Star Rated License process. Facilities have been divided into 3 Cohorts. Each cohort will have a preparation year and an assessment year. Your facility is in Cohort 1. Your preparation year began July 1, 2023 and will end on June 30, 2024. Your assessment year begins July 1, 2024. The preparation year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. It could be helpful to consider areas where refresher information might be helpful. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous ECERS-R and ITERS-R assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Seek out webinars and/or in-person trainings related to topics of interest. 4. Start a self-study process to become more familiar with the ERS or priority areas relevant to your program 5. Schedule staff meetings to discuss topics related to the Environment Rating Scales 6. Support and encourage staff during a self-study process. Connect them with resources at ncrlap.org 7.
