Upon my arrival I was greeted by the Teachers The facility currently operates under a Five star license that was issued on March 8, 2019. The last annual compliance visit was conducted on February 27, 2026. The last sanitation inspection was completed May 28, 2026 with a “Approved” classification. The last fire inspection was conducted on October 17, 2025 I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed participating in free play in their classrooms. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted which included, General Licensing Requirements, Program Requirements, Staff Child Ratio, Supervision, Medications and General Safety. There are no new staff. Today I observed one violation(s) regarding the Child Care Requirements Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The Lead Teacher in Classroom A11, had a background check that expired in 3/2026. G.S. 110-90.2(b) & .2703(n)&(o) The above violations must be corrected immediately, and a letter must be sent to me no later than June 12, 2026 stating how each item was corrected and a plan to maintain compliance with the requirement. If I do not receive your letter June 12, 2026 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. The program will be completing the remaining components of the QRIS next school year (2026-2027) Resources The Health and Safety Trainings are offered on the DCDEE Moodle format. Upon my arrival I was greeted by the Teachers they assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment and attendance was documented, and observations were made. The children were observed participating in free play outside and participating in free play in their classrooms. The last annual compliance visit was conducted May 17, 2024. The center is operating five star license that was issued March 8, 2029. The license was earned with 7 points in program standards, 7 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted November 14, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. There are no new staff and four children’s files were monitored. NC Pre-K requirements were monitored. Your compliance history is 88%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 833 The EMC plan did not give the procedures to be followed . Two classrooms did not have a current Emergency Medical Care Plan posted. 10A NCAC 09 .0802(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space A11, there was disinfectant wipes and spray stored on top of the cubbies. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space A11, there was asthma medication in a classroom backpack sitting on the floor. 15A NCAC 18A .2820(d) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. A playground inspection was not completed for April 2025. .0605(q) The above violations must be corrected immediately, and a letter must be sent to me no later than May 16, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous ECERS-R assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Classroom A13 did not documentation of arrival and departures times for the children. 10A NCAC 09 .0302(d)(4) 802 Safe pick-up and delivery procedures were not communicated to parents and/or were not posted where they can be seen by the parents. Safe arrival and delivery procedures were not posted in any of the classrooms. .1003(b) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The fire drill report was not completed or posted. .0604(t); .0302(d)(5) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There an outlets in rooms A12 and A13 that were not covered. Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children. There were alaminator in classroom A11 and A13 that was accessible to the children. .0604(e) The violation(s) must be corrected immediately, and a letter must be sent to me no later than Febuary 15, 2023 stating how each item was corrected and how you plan to maintain compliance with the requirement.
