A checklist was used to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 11/13/25. Currently this family childcare home operates with a four (4) Star License, issued 3/5/20. The home earned seven (7) points in staff education, two (2) points in program standards, and one (1) quality point for education options met. The owner/operator Katrina Fonville assisted me today. I conducted an assisted walk-through of the home. Children were observed playing outside, eating lunch, resting and routine care activities. Transportation is provided for routine activities and field trips. The provider uses a Toyota Sienna. The vehicle was monitored today. The files for two new children were reviewed. A full assessment of applicable childcare rules was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1406 Authorization for up to 6 months to administer prescription or over-the-counter medication, when needed, for chronic medical conditions and for allergic reactions was not in writing and did not contain information required by rule 10 NCAC 09 .1720(c)(6)(A-I). Permission to administer medication was not on file for one Epi-Pen. 10A NCAC 09 .1720(b)(4) 1871 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The plan was not updated annually. This was corrected during the visit today. .1705(b)(8) 2006 Individuals who provide care during unplanned absences of the operator, such as medical emergencies, were not 18 years old, and have not submitted criminal background checks forms as required in Rule .2703. A. Thompson did not have a criminal background letter on file. .1729(d) 2009 Copies of the required information for an additional caregiver was not on file in the home and/or made available for review. The required paperwork for A. Thompson was not on file. .1729(a) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. S. Fonville and K. Fonville did not complete the trainings. These were due by 2/2026. Technical assistance was provided. .1703(d)(2) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Fonville. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 7/13/26. CONSULTATION Rated License information. You have chosen pathway two. I will obtain your application once your choose a curriculum. We reviewed the family and community engagement standards today. For four stars, you are required to do the foundational practices with two additional option. We discussed how to document these practices by adding a family policy to your parent handbook on how you are incorporating the practices. Review your parent handbook to see how you may already be meeting these foundational practices. You will write down how you are meeting the practices for review prior to completing the rated license. Each pathway requires the completion of the facility CQI plan. This will be your professional development plan moving forward. I will be looking to see items 1-4 completed at the time we renew your rated license. You will document progress towards goals. If you complete your goals, you will begin a new form. You will always be working on goals. You must obtain an approved curriculum that serves all ages of children you have enrolled. You have a current list of curricula to choose from. I will follow-up with you about the school-age children and if you need a separate curriculum for them. You will be required to complete one approved formative assessment during the year and offer parent conferences to review the assessment. This does not have to be done for school-age children. We discussed the DRDP free assessment today. You will be required to complete five coaching or training hours annually. These will be in addition to your ongoing training hours. After you choose your curriculum, formative assessment, and complete the family and community engagement and CQI forms, we can process your new license. Technical Assistance: Once you access the Emergency Preparedness plan, you will make any updates required and resave, publish, and print the first page for your records. This must be done annually. You and your husband must complete the medication administration and Recognizing and Responding to Suspicions of Child Maltreatment trainings to finish the health and safety trainings. These can be accessed on the DCDEE website, hyyps://nchcildcare.ncdhhs.gov. Click provider, trainings and professional development, and then DCDEE MOODLE. You will use your NCID to login. Scroll down to the search box and type CCDF. The trainings will come up. The medication administration training in not on the list. You will have to go back to the search box and type medication. Then you will see the training. The training for Recognizing and Responding to Suspicions of Child Maltreatment is found and https://www.preventchildabusenc.org. Your niece must obtain an NCID at https://ncid.nc.gov. She must then go to ncchildcare.ncdhhs.nc.gov, provider tab, criminal background tab, then register for the background check. Once she pays, she must go to the local sheriff or police station to have her fingerprints done. Once the qualification letter is received, you will print for your records. She must be linked to your ABCMS account within five days of receiving the letter. She cannot come back to help in the home to assist until a letter is received. I left a copy of the file checklist for an additional caregiver with you today. She must have a complete file. We discussed having your grandson drop-in during the summer. Your grandson must have a complete file if he is in attendance during your business hours and he counts towards your enrollment. Once the new stationary structure outside is complete, you must have required mulch and fall zones around it before use. A copy of the manufacturer’s instructions must be on file for review. If you plan to take children on field trips, you must have a signed/dated permission form for each trip. The form has the location and times of the trip. When children are taken on field trips, you should document by name to face attendance every thirty minutes. This can be done with a head-count sheet.
