A checklist was used to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 1/30/25. Currently this program operates with a five (5) Star License, issued 6/3/19. The facility earned five (5) points in staff education, seven (7) points in program standards, and one (1) quality point for programmatic options met. The center director, Dawn Greene, arrived during the visit and assisted me today. I conducted an unassisted walk-through of the center. The last sanitation inspection was completed 10/22/25 with a ‘Superior’ classification and 14 demerits. The last fire inspection was conducted on 1/31/25. A re-inspection was completed 2/25/25. Children were observed playing in activity areas, playing outside, eating lunch, resting, and routine care activities. The center uses two large passenger vans (Ford CAM 5089 and Chevy KEC 9514) to transport children. Both were on site and monitored today. A full assessment of applicable childcare rules was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was completed 1/31/25. The previous inspection was completed 12/6/23. 10A NCAC 09 .0304(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Staff member M. Cruz was short ten hours and K. Daniel was short three hours. CONSULTATION Today we discussed the Pathways to Stars and completed the pathway decision form. You indicted you are leaning towards Pathway 2. A staff meeting is planned for 1/22/26 to discuss with staff and to make a decision. The NCRLAP website, https://ncrlap.org, has trainings geared towards the new ECERS3 assessment scale. These will introduce you to the updated assessment scale and provide tips for completing the new scale. You may reach out to NCRLAP to conduct a practice assessment. There may be a period of time you must wait to complete the actual assessment. You will need to inquire about this. We discussed the annual review of the center’s Emergency Preparedness Plan. The website is https://rmp.nc.gov. You may contact Amia Eaton at 919-814-6365 if you have difficulties accessing your plan. After you make any updates, save and republish the plan. Print page one for your records. We discussed the requirements for screen time today. Children may watch TV when planned for a developmental goal. The time allowed is 30 minutes per day maximum and each child’s daily time must be recorded on a screen time log. A fire inspection must be completed within 364 days of the previous inspection. It is best practice to contact your fire inspector at least thirty days prior to the expiration date to ensure you are scheduled. An inspection is scheduled for 1/20/26. A copy must be emailed to me within seven days. All medications listed on a medical action plan must be located in the child’s classroom. Over-the-counter medications must be stored in a locked box and stored with the emergency medication which is unlocked. The over-the-counter medication can be unlocked when accompanied by a letter from the doctor stating the medication if for emergency purposes. The location of the medication must be clearly labeled in the classroom. Each medication must have a separate medication administration log and permission form. A copy of the action plan and a medication administration permission form for each medication must accompany the medication. It is best practice to attach a photo of the child to the action plan. Copies of the action plan must also be included in the transportation notebook. Action plans signed by a parent are valid or six months and valid for twelve months when signed by a doctor. We discussed the use of tires on the playground. Holes must be drilled in the sides of the tires to allow rainwater to drain. Staff must be sure to check the inside of the tires daily for spiders, snakes, and wasps. Ms. Greene and I completed a walk-through of the outdoor environment. We discussed supervising children on the lower playground. Ms. Greene stated children only play in the wooded area when two teachers are present to provide supervision in that area. We discussed monitoring daily for debris and snakes or other vermin. There are rubber mats under the swings and in the fall zone area. I will inquire if these are allowed for use the fall zone area and will provide follow-up. The Recognizing and Responding to Suspicions of Child Maltreatment training for H. Thompson is due by 2/17/26. Due to connectivity issues, I could not view a copy of the childcare rules pertaining to immunizations for school-age children. Upon my arrival I was greeted by the Administrator, Dawn Greene, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment was documented, and observations were made. The children were observed participating in free play outside and eating lunch. Lunch which consisted of grilled cheese, lima beans, peaches, chicken noodle soup and milk. The last annual compliance visit was conducted February 13, 2024. The center is operating five star license that was issued June 3, 2019. The license was earned with 7 points in program standards, 5 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted November 25, 2024 the center received a Approved rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted December 6, 2023. One new staff file and five children’s files were monitored. Transportation was monitored during today’s visit. Your compliance history is 83%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. Staff and the Director did not have current health questionnaires on file. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Staff and the Director did not have current emergency information on file. .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. One new teacher did not have a criminal background check. G.S. 110-90.2(b) The above violations must be corrected immediately, and a letter must be sent to me no later than February 14, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Vargas does not have a criminal record background check, she cannot work until she has received a qualification letter. Staff did not have updated emergency information or health questionnaires, it may be a good practice to put a day during the year and have all staff update this items every year on the same day. Your last fire inspection was December 2023, during today’s visit you called to request an inspection, in the future, calling to request an inspection at least 60 days before it expires may help. Making a note on an electric calendar may help as well. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed. Upon my arrival I was greeted by the Administrator, Dawn Greene, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment was documented, and observations were made. The children were observed participating in free play outside and eating lunch. Lunch which consisted of grilled cheese, lima beans, peaches, chicken noodle soup and milk. The last annual compliance visit was conducted February 13, 2024. The center is operating five star license that was issued June 3, 2019. The license was earned with 7 points in program standards, 5 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted November 25, 2024 the center received a Approved rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted December 6, 2023. One new staff file and five children’s files were monitored. Transportation was monitored during today’s visit. Your compliance history is 83%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was documented December 2023. 10A NCAC 09 .0304(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Staff and the Director did not have current health questionnaires on file. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. An unannounced follow up visit was conducted on June 25, 2024 and violations were cited regarding supervision and safe environment. A corrective action letter was received on July 11, 2024. Upon my arrival I was greeted by the teachers, they assisted me during today’s visit. The Director was not present. I observed the school age children participating in free play outdoors on their designated playground. I observed a group of two and three year olds participating in free play outdoors on their designated playground. I observed a group of children ages three to five participating in free play in their classroom and having a group time story. During today’s visit the following were monitored, staff child ratio, supervision, and safe environment. Staff child ratio and supervision were found in compliance. The following violation was documented during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There was a large long tree vine hanging down that children to swing, if a child were to fall they could be injured. There was an area of the ground that dropped down creating a large ditch where children could get stuck in and be unable to climb out. This is a repeat violation from 6/25/2024. 10A NCAC 09 .0601(a) The violation(s) must be corrected immediately, and a letter must be sent to me no later than August 2, 2024 stating how each item was corrected and how you plan to maintain compliance with the requirement. Please e-mail the letter to me at mona.ndiaye@dhhs.nc.gov . I did a walk through of the wooded area on the playground for school age children. No children were observed in this area during today’s visit, the teacher stated the children were not playing in the wooded area because there was only one teacher with them today. I observed a deep ditch that was discussed with the Director during the last visit, there were no changes made to the ditch since the last visit. In the corrective action letter received on July 11, 2024, the Director stated she would inquire about having someone put logs in the ditch to allow the children to be able to climb out of the ditch. There was also a vine hanging in the wooded area for children to swing on. This vine has no surfacing under it to prevent injury if a child should fall while swinging. In the corrective action letter received on July 11, 2024, the Director stated she was still deciding what to do about the hanging swinging vine. She stated she did not want to cut it because the children enjoyed swinging on it. Technical Assistance Based on my observation of the wooded area, more clearing needs to be done to allow adequate supervision. I would recommend focusing on the outer areas of the wooded area. Greenery can grow fast during the summer, so maintenance of the wooded area on a regular basis is required if the area will be used by the children. Until the Director decides what to do about the hanging vine used for swinging, children should not be allowed to swing on it. In addition the ditch needs to be filled in or altered to allow children to be able to climb out of it, especially the preschool age children. The ditch should be off limits to the children until changes have been made to it. Once you have made these items safe for the children contact me to schedule a visit to monitor for safety. A complaint visit was conducted on June 14, 2024 and violations were cited regarding supervision and safe environment. Upon my arrival I was greeted by the teachers, they assisted me during today’s visit. The Director was not present at the time of my arrival, she later arrived towards the end of the visit. I observed the school age children participating in free play in the classroom. I observed a group of two and three year olds participating in free play in their classroom. I observed a group of children ages three to five outside, playing freely on the playground for school age children. There is a wooden area with trails located at the back of the playground, the children were playing in this area. A corrective action letter was received on June 21, 2024, however it was an old corrective action letter from an annual compliance visit that was conducted on February 13, 2024. The letter was due by June 21, 2024. Once the Director arrived she emailed the correct letter and printed a copy for me to review. During today’s visit the following were monitored, staff child ratio, supervision, and safe environment. The following violations were documented during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. Several children were observed running through the wooded area on the playground and although the teacher was centrally located in the wooded area, all the children could not be seen. There was a back corner area where a child was standing, his red shorts could be seen however the teacher was unable to see what the child was doing. This is a repeat violation from 6/14/24. .1801(a)(1-5) 807 A safe indoor and outdoor environment was not provided for the children. There are several wooden vines spreading across the ground that are potential tripping hazards. There was a large long tree vine hanging down that children were observed swinging on, this could be a potential hazard if the children fall while swinging..There was an area of the ground that dropped down creating a large ditch and a child got in that area and was unable to get out without teacher assistance. There were two teachers with 12 children ages 3 to 5 years of age. One of the teachers was positioned at the front area of the playground and the other teacher was in the wooded area. I observed a large area that dropped down like a ditch and a child got in the ditch and was unable to get out without teacher assistance. Several children were running through the trails wooded area and although the teacher was centrally located in the wooded area, all the children could not be seen. There was a back corner area where a child was standing, his red shorts could be seen however what the child was doing could not be seen because he was standing behind a tree. There was a large long tree vine hanging down that children were observed swinging on, this could be a potential hazard if the children fall while swinging. Technical Assistance I spoke with the Director about the wooded area, we discussed how the area was great for the children to explore nature however it needed to be safe. We discussed cutting down more of the greenery and bushy areas to allow better supervision of the children. Some type of surfacing must be added if the children will be using the tree vine to swing. We discussed filling in the ditch with dirt or adding wooden steps to allow children to climb in and out on their own, while allowing the area to remain a natural part of the environment. I recommended walking the trails again and cutting any large vines running across the ground to prevent tripping hazards. Children can enhance many skills from playing and exploring in a natural learning environment, including environment awareness, sensory development, risk assessment, cognitive and physical development. The facility currently operates with a Five Star License that was issued on June 3, 2019. The last annual compliance visit was conducted on February 13, 2024. Kaye Adkins, Child Care Supervisor, assisted me on today’s visit. The Director, Dawn Green, was present and assisted us during the visit. I informed her the reason for our visit. The children were observed participating in free play indoors and outdoors. The school age children were on a field tip to a local park. Allegation 1 : There is concern regarding supervision Findings: During today’s visit, we monitored the playgrounds for each age group. There is a lot of greenery, plants and trees through the playgrounds. There was a group of preschool age children observed in the wooded area in the back of the school age playground. As we walked through the entrance to the school-age playground, we observed two children two to three years of age standing by the entrance. We observed other children walking up towards the fence as we walked down towards the wooded area. The teacher was coming out of the wooded area and was observed calling for children to come out of the wooded area. The wooded area has several trails, tall trees and tall greenery throughout it. The teacher could not see the children standing at the entrance of the playground nor could she see the two children that were in the wooded area. The allegations concerning supervision is substantiated based on the information obtained during today’s visit. Allegation 2 : There is concern regarding a safe outdoor environment. We walked through the wooded area in the back of the school age outdoor play area. The area had several trails, the area was overgrown with plants, trees and general greenery that prohibited adequate supervision. The were vines on the ground throughout the area and large tree roots that were coming out of the ground that are potential tripping hazards. There were areas of the ground covered with greenery which made it difficult to see harmful insects, small animals, and snakes that could in those areas. The wooded area also had several tires throughout the area that could hold water and vermin. We discussed the different types of plants and trees on the playgrounds with the Director. She stated that she had recently removed poison ivy from the area. There were tomato plants that were accessible to the children. Findings: The allegations regarding safe outdoor environment is substantiated based on the information observed during today’s visit. Allegation 3 : There is concern regarding sanitation and health. We did not observe noncompliance regarding sanitation and health during today’s visit. Findings: The allegations regarding sanitation and health is unsubstantiated based on the information obtained during today’s visit. Ratios were observed in compliance. The following violations were documented during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. The teacher was unable to see and hear the children in her care on the playground due to the overgrown wooded area on the playground. .1801(a)(1-5) 807 A safe indoor and outdoor environment was not provided for the children. There were several tripping hazards and potential injury from outdoor animals and insects due to overgrown greenery areas. There were tomato plans and poison ivy accessible to the children. 10A NCAC 09 .0601(a) The above violations must be corrected immediately, and a letter must be sent to me no later than June 21, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. Limbs and vines that run across the ground should be monitored to prevent tripping hazards. We spoke with the Director about making the tomato plant inaccessible to the children. Dawn Green, Director, was present and assisted me during today’s visit. I completed a walk-through of the facility, attendance was documented, and observations were made. The children were observed participating in free play and interacting with the teachers. The last annual compliance visit was conducted February 20, 2023. The center is operating under a Five Star License that was issued June 3, 2019. The last sanitation inspection was conducted November 28, 2023 the center received an Approved rating. The last fire inspection was conducted December 6, 2023. Fire drills, transportation and playground inspections were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. One child had an Epi Pen in a locked cabinet that did not have a parental authorization on file. 10A NCAC 09 .0803(4)(6-9) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff did not have a completed and signed orientation on file. .1101(a) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file.
