A visit was attempted on 7/2/26. Upon arrival I met with Tami Thomas, director, and I explained the purpose of my visit. Allegation: There are allegations of violations of childcare requirements. Upon arrival, I asked Ms. Thomas is there were any concerns in the room for infants. She stated a family had just disenrolled a child from the room for one-year old children. I observed video from the room for infants on 6/15/26. During the time of the recording, one staff member was present with one infant and six one-year old children. A second staff arrived as I watched the video. I received copies of sign in/out sheets for 6/15/26. I also received copies of requested incident reports. Conclusion: Based upon the information received during the interviews and observations, a violation of childcare requirements was unsubstantiated. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. While observing the video in the classroom for infants, one staff member was present with one infant and six one-year-old children. GS 110-91(7);.0713(a-d) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Hinojos. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 7/20/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. Technical Assistance: You are a licensed GS 110 religious sponsored childcare. We discussed working with staff in the classrooms with younger children to provide intentional activities for the children in keeping with progress towards developmental milestones. This will help children be more engaged and active. Staff were observed sitting on the floor interacting with the children during the video observation. We reviewed staff/child ratios of 1:5 for infants and 1:6 if all children are one-year old. We discussed options for when children are moved to a different classroom for staffing purposes. On the day of the incident, five one-year old children were moved into the room for infants. Children can become easily bored as this room is not in keeping with new developmental needs. An infant can be moved up as long as no child in the next room is two years of age or older. Both rooms have a changing table and this would more easily accommodate keeping the older children engaged. If the church does decide to incorporate the childcare, you must notify me prior so we can complete the required forms. If you decide to provide transportation for field trips, you must contact me prior to approve transportation. We discussed emergency medications today. If a parent signs the medication permission and action plan, the forms are valid for six months. If the physician signs them, they are valid for twelve months. Action plans that list an over-the-counter medication such as Benadryl, must be kept in a locked box and stored with the emergency medication that is unlocked. The parent may obtain a note from the physician stating the over-the-counter medication is for emergency use. This note must be kept with the medication and would not be required to be locked. Each medication on an action plan must have a permission to administer form and be included on an action plan. CPR/First Aid is due for some staff in July. Upon my arrival I was greeted by the Director, Tami Thomas, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment and attendance were documented, and observations were made. The infants were observed participating in free play their classrooms, napping and one infant was being held by a teacher. The children were observed eating lunch which consisted of cheese pizza, bananas, carrots and milk. The last annual compliance visit was conducted June 28, 2024. The center is operating under a Notice of Compliance that was issued September 6, 2023. The last sanitation inspection was conducted May 6, 2025 the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted on August January 6, 2025. New staff files were monitored along with ten children’s files were monitored. Your compliance history is 80%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violations during today’s visit. Violation Number Comment Rule 542 The written feeding plan was not modified as the child's needs changed. The feeding plans were not updated as needed for the children in space 2. 10 NCAC 09 .0902(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was an unlocked cabinet in space 6, that had disinfectant wipes and a bottle of body spray. .2820(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In the classroom for infants, there was a prescription medication, Clotrimazole, that was not in locked storage. 15A NCAC 18A .2820(d) 847 Parent's medication authorization did not include required information. There were several diaper creams that did not have a completed parental authorization on file. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space one for infants, there were medications that were left over from children that were no longer enrolled in the classroom. .0803(12) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. Pest control: Check for bees/wasp nests, ant hills, or signs of other pests. 2. Sun Protection Shade structures: Install or check canopies, sails, or umbrellas over play areas and seating. Natural shade: Trim trees for safety but keep them healthy to provide extra sun coverage. Sunscreen policy: Have a plan for applying sunscreen (with parent permission) and ensure staff follows it. 3. Hydration Station Water access: Ensure water is available with disposable cups. Encourage water breaks: Schedule regular hydration times during play. 4. Outdoor Activities & Toys Summer-appropriate toys: Add water tables, sand toys, sports equipment, or ride-ons. Rotate toys: Keep the environment fresh and engaging by rotating outdoor toys. Nature-based play: Include items like gardening tools, pots, or sensory bins with natural materials. 5. Cleanliness Sanitize outdoor toys regularly—especially anything children put in their mouths. Tidy up landscaping: Remove any debris, broken branches, or trash. 6. Safety from Heat Playtime schedule: Shift outdoor play to cooler times (morning or late afternoon). Cooling activities: Plan for cooling breaks with misting bottles, or popsicle breaks. Watch for signs of heat exhaustion in children and staff (lethargy, excessive sweating, etc.). 7. Upon my arrival I was greeted by the Director, Tami Thomas, she assisted me during today’s visit. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed participating in free play indoors and outdoors. The children were observed eating lunch which consisted of cheese pizza, pineapples, carrots and milk. The last annual compliance visit was conducted July 7, 2023. The center is operating under a Notice of Compliance that was issued September 6, 2023. The last sanitation inspection was conducted May 30, 2024 the center received a Superior rating. The last fire inspection was documented January 4, 2024. Fire drills and playground inspections were monitored. Staff and ten children’s files were monitored. The Department of the Secretary’s website was used to check the status of the corporation. The facility’s owner is current/active. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In one of the classrooms for toddlers, there was an unlocked cabinet that had Zep disinfectant spray and Febreze air freshener. .2820(b) 847 Parent's medication authorization did not include required information. Encourage parents to only bring one diaper cream for their child, this will decrease the amount of paperwork and medications. I observed logs on the playground, to decrease the possible places where snakes can hide, we discussed securing the logs to the ground. Check the slide before allowing children to play on them, if direct sunlight is on the slides, they can become very hot. If this becomes an issue consider adding a shade cover over the slides. During today’s visit, the administrative assistant stated the van is not currently being used for transportation. Ensure there is a first aid kit in the van if you decide to use it in the future. Send me your completed and signed staff and training worksheets by July 2, 2024. Today is the last day to conduct a fire drill and complete a playground inspection for June 2024. The toddlers were observed participating in free play in their classrooms and eating lunch. The last sanitation inspection was completed August 29, 2023 with a “Superior” classification.
