Upon my arrival I was greeted by the Administrator, Heather Skeen, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment and attendance was documented, and observations were made. The children were observed participating in free play in their classrooms. The last annual compliance visit was conducted April 19, 2024. The center is operating three star license that was issued June 25, 2019. The license was earned with 3 points in program standards, 3 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted September 17, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted August 20, 2024. One new staff file and three children’s files were monitored. Your compliance history is 89%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The Director did not have current ITS SIDS training. .1102(f) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. One child did not have a current immunization record on file. See children's records form. 10A NCAC 09 .0302(d)(2) The above violations must be corrected immediately, and a letter must be sent to me no later than March 23, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. The facility currently operates with a Three Star License that was issued on June 25, 2019. The last annual compliance visit was conducted on April 19, 2024. The Director, Heather Skeens, was present and assisted me during the visit. Children were observed participating in free play outside on their designated playground, the teachers were observed moving about the playground interacting and supervising the children. The toddlers and infants were observed participating in free play in their classroom, the teachers were observed interacting in a positive manner with the children. The last sanitation inspection was completed September 24, 2024 with a “Superior” classification. Four new staff records were monitored during today’s visit. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. A parental authorization form for Tylenol and Ibuprofen did not have specific information on when to give the medication. 10A NCAC 09 .0803(4)(6-9) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. On the playground for toddlers, there is a broken swing and trash. On the playground for preschool age children there is a broken toy shopping cart and trash. 10A NCAC 09 .0604(p) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Two new staff did not have medical reports on file. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. This could potentially become a liability if medications are not used correctly. Talk with the parents and have them pick one diaper cream, explain that they can change it out whenever they need to. The same is true for over the cover medications as well, instructions need to be specific when using over the counter medications. Return medications to parents within 72 hours when the medications are no longer needed. We also discussed the “when to apply” portion of the topical medication form. Talk with parents about the specifics of “after every diaper change” verses “ other, when rash is present” verses “after every bowel movement. Remember medication directions from parents must be followed unless it goes against the intended use or dose of the medication. The tree on the outdoor play area for children 3 years of age and up has a pecan tree, the pecans can be a choking hazard. I observed a child with a pecan she brought in to the classroom and had in her mouth, one of the teachers took the pecan and through it in the trash. Reminders The Director and Lead Teacher or the children in the classroom for infants, ITS SIDS training expires in December 2024. Staff, K. East, must complete Recognizing and Responding to Suspicions to Child Maltreatment training. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. The center is operating under a Three Star License that was issued June 25, 2019. The last sanitation inspection was conducted October 21, 2023 the center received a Superior rating. The last fire inspection was documented May 10, 2023. Fire drills and playground inspections were monitored. New staff and ten children’s files were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 401 Diapers were not changed whenever they were soiled or wet and/or were changed on a shift basis. A toddler was observed walking around the classroom with wet clothing due to needing a diaper change. 10A NCAC 09 .0806(a) 491 Caregiver did not respond at the earliest opportunity to an infant or toddler’s physical and emotional needs. Several children in the center had visibly active and dried runny noses. .0511(b)(1) 604 Lavatories were not kept clean, in good repair and kept free of storage. The bathroom in the front of the building that is used by the children had a strong smell of urine. 15A NCAC 18A .2818(a) 847 Parent's medication authorization did not include required information. Diaper creams in the classroom for toddlers did not have complete parental authorizations on file. 10A NCAC 09 .0803(4)(6-9) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed participating in free play indoors and outdoors on their designated playgrounds. The last annual compliance visit was conducted May 26, 2023. The center is operating under a Three Star License that was issued June 25, 2019. The last sanitation inspection was conducted October 21, 2023 the center received a Superior rating. The last fire inspection was documented May 10, 2023. Fire drills and playground inspections were monitored. New staff and ten children’s files were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. The facility currently operates with a Three Star Child Care License that was issued on June 25, 2019. The last Annual Compliance Visit was conducted on May 26, 2023. The Director, Heather Skeen, were present and assisted me during the visit. Upon my arrival I was greeted by the Director, I informed her the reason for my visit. I visited each classroom to get enrollment. The children were observed eating lunch. Lunch consisted of ham, pineapples, crackers, corn and milk and preparing for rest time. The teacher of the infants was observed interacting with the infants. Allegation 1 : There is concern regarding sanitation and health. During today’s visit I monitored the menu that wads posted and compared it to what the children were observed eating. The current menu was followed. I looked in the refrigerators and cabinets in the kitchen and I observed a variety of food items. The director had an exterminator come out on February 9, 2024 and I observed a signed monthly contract with the company. I did not observe an non-compliance regarding health and sanitation. Findings: The allegations concerning health and sanitation is unsubstantiated based on the information obtained during today’s visit. Allegation 2 : There is concern regarding a safe and developmentally appropriate environment. Hazardous items and chemicals were in locked storage. All the classrooms were in ratio and a had developmentally appropriate materials and activities. I observed a mouse trap in a cabinet in the bathroom used by the children however the trap was not set. Findings: The allegations regarding a safe and developmentally appropriate environment is unsubstantiated based on the information obtained during today’s visit. The following violations were documented during today’s visit. Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference.
