This visit was conducted by Turkessha Casterlow, Childcare Consultant with Shanika Alston, Site Administrator. Your program currently operates with a Five Star license. Restrictions include Daytime care only: No children in diapers; Meets enhanced ratios; Meets enhanced space. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on November 17, 2025, and Randolph Community Services was listed as current and active. The program’s compliance history was 88% prior to today’s visit. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during group time, outside play, and free play. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on December 13, 2024. A sanitation inspection was completed on October 14, 2025, with a Superior classification. The last fire inspection was conducted on January 28, 2025. Program records and required postings were monitored. A fire drill was conducted on October 20, 2025. A lockdown drill was documented on November 12, 2025. An outdoor inspection was documented on November 13, 2025. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In Spaces ! and 6 activity plans were not posted for the current week. GS 110-91(12); .0508(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Upon my arrival I was greeted by the Administrator, Susan Moore, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, attendance was documented, and observations were made. The children were observed participating in free play in their classrooms and outside on their designated playgrounds. The last annual compliance visit was conducted December 19, 2023. The center is operating five star license that was issued October 10, 2024. The license was earned with 6 points in program standards, 7 points in education and 1 quality point permit restrictions were in compliance The last sanitation inspection was conducted November 21, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted February 27, 2024. Four new staff files were monitored, and 10 children’s files were monitored. Your compliance history is 95%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In space 3, the posted activity plan had November dates. GS 110-91(12); .0508(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. One staff required to have 20 hrs of training annually,only completed 11 hrs. .1103(a) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. One monitored child's file did not have a medical report on file. (see childrens record form) GS 110-91(1);.0302(d)(2); .0304(g) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Several children's files that were monitored did not have this notification on file. .0604(j) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One staff did not have a signed acknowledgement on file. .0608(d)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff hired 8/27/2024 did not completed the training. .1102(g) The above violations must be corrected immediately, and a letter must be sent to me no later than December 27, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Technical Assistance The staff and training worksheets were submitted, however some of the dates were incorrect, please correct the dates and add the training hours for each staff. Training hours are counted using the staff hire dates. Use the training log on the DCDEE website to keep track of the trainings. I reviewed the staff and training worksheets and information for some of the requirements of the form with the Director. Send the updated staff and training worksheets to me by 12/25/2024. I discussed the requirements for a schedule and activity plan for wrap care. I explained that all the requirements are the same and afternoon monitoring visit can take place during wrap care. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed.
