Upon arrival I met with Jennifer Thomas, center director, and I explained the purpose of my visit. Allegation: There are allegations of violations of childcare requirements. Ms. Thomas and I discussed the center’s policies for children including the sick policy. Parents review and sign at enrollment that they understand the policies and will adhere to them. I asked about how changes to feeding schedules are handled. Ms. Thomas stated that when infants are ready to transition to table foods, the parents update the feeding schedules. They are sent a copy of the center weekly menu on Brightwheel. The menu is also posted in the center. Parents can review and decline any foods on the menu they do not wish for their child to be served. We discussed an incident involving a child in one of the feeding chairs. I spoke with the two staff members present in the room at the time of the incident. I inquired about the video from the day of the incident. Ms. Thomas tried to recover the footage, however, too many days had lapsed and it was no longer available. Based upon photo documentation received for the complaint, the guidelines for serving hot dogs and other foods were followed to reduce any choking hazards. Conclusion: Based upon the information received today and interviews, violations of childcare requirements were substantiated. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 303 Children were not adequately supervised at all times. During a meal service on 2/20/26, two staff members in the room for infants were caring for five children. Some children were seated waiting for the food to arrive. Once nine month old child was trying to get out of his feeding chair. Staff were aware and tried to keep him seated. Staff stated they turned their attention toward a crying infant. The nine month old tried to stand, flipping the feeding chair over and bumping his head on the floor. .1801(a)(1-5) 807 A safe indoor and outdoor environment was not provided for the children. The safety strap in a feeding chair for a nine month old child was not tightened. This allowed him to attempt to stand in the chair causing the chair to flip over. 10A NCAC 09 .0601(a) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Thomas. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 4/7/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. Technical Assistance: We discussed age-appropriate waiting times for infants/toddlers and meals. Ideally, children of this age should be seated when the food is ready to serve. You may offer puffs or something similar for the child to have while waiting also. When children are seated in feeding devices, a staff member should remain within arm’s reach to assist a child. All safety restraints must be used and tightened to accommodate the child in the chair. A checklist was used today to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 12/31/24. Currently this program operates with a four (4) star rated license issued 6/27/19. The facility earned four (4) points in staff education, six (6) points in program standards, and one (1) quality point for education options met. The center director, Jennifer Thomas, assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 7/28/25 with a ‘Superior’ classification and 15 demerits. The last fire inspection was conducted on 9/15/25. Children were observed eating lunch, resting, and routine care activities. The facility does not provide transportation. A full assessment of applicable childcare rules was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. For the month of December, the safe sleep chart was not completed for five days for one infant and one day for a second infant. .0606(g) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The TB test for M. Griggs was older that twelve months on date of hire. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Orientation was not on file for K. McNair. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Record of the training was not on file for K. Pierson. The training was due by 8/27/25. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Record of the training was not on file for K. Pierson. The training was due by 8/27/25. .1102(d) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Staff K. A copy of the completed ongoing training log must accompany the corrective action letter. .1103(a) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. The file for one child monitored today did not contain the required medical statement. CONSULTATION Today we discussed the Pathways to Stars and completed the pathway decision form. You chose Pathway one. I will schedule a technical assistance visit in February 2026, and we will review the forms and plan to request your rated license assessment. The NCRLAP website, https://ncrlap.org, has trainings geared towards the new ECERS3 assessment scale. These will introduce you to the updated assessment scale and provide tips for completing the new scale. A copy of the health and safety training log was left with you today. Please have all staff complete this form as they complete the required trainings to ensure all trainings required are completed. Sleep charts must be kept current and sleep times recorded during each infant’s rest times. The mulch on the upper playground is very compacted. This need to be tilled up to loosen the mulch. A copy of the Continuous Quality Improvement/Professional Development form was left with you today. Staff should use this form moving forward for CQI and Professional Development plans. These are required to be completed annually. A copy of a preschool activity plan was left with you today. The developmental domains are listed at the bottom. Staff can use this as a guide to ensure they are meeting all the domains each week. A TB test and medical statement must not be older than twelve months on date of hire. If it is, it must be completed again prior to employment. If you decide to resume transportation, you must contact me prior to resumption. The facility currently operates with a Four Star license that was issued on June 27, 2019. The last annual compliance visit was documented December 31, 2024. The Director , was present and assisted me during the visit. Children were observed participating in free play outside and in there classrooms. The last sanitation inspection was completed April 2, 2025 and earned a “Superior” classification. The last fire inspection was documented September 20, 2024. One new staff file was monitored. Fire drills and playground inspections were monitored during today’s visit. During today’s visit, a partial assessment of applicable child care requirements was conducted. Program records, general licensing requirements, staff/child ratios, supervision general safety, medications and storage of hazardous items were monitored. I observed one violation. Violation Number Comment Rule 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. With the warmer weather approaching, review the following information. 1.Safety Check & Maintenance Inspect equipment: Check for loose bolts, rust, splinters, or cracks on play structures, swings, and slides. Surface check: Ensure mulch, rubber mats, or sand are evenly spread and thick enough to cushion falls. Fence & gate: Inspect for damage or sharp edges and make sure gates latch securely. Pest control: Check for bees/wasp nests, ant hills, or signs of other pests. 2. Sun Protection Shade structures: Install or check canopies, sails, or umbrellas over play areas and seating. Natural shade: Trim trees for safety but keep them healthy to provide extra sun coverage. Sunscreen policy: Have a plan for applying sunscreen (with parent permission) and ensure staff follows it. 3. Hydration Station Water access: Ensure water is available with disposable cups. Encourage water breaks: Schedule regular hydration times during play. 4. Outdoor Activities & Toys Summer-appropriate toys: Add water tables, sand toys, sports equipment, or ride-ons. Rotate toys: Keep the environment fresh and engaging by rotating outdoor toys. Nature-based play: Include items like gardening tools, pots, or sensory bins with natural materials. 5. Cleanliness Sanitize outdoor toys regularly—especially anything children put in their mouths. Tidy up landscaping: Remove any debris, broken branches, or trash. 6. Safety from Heat Playtime schedule: Shift outdoor play to cooler times (morning or late afternoon). Cooling activities: Plan for cooling breaks with misting bottles, or popsicle breaks. Watch for signs of heat exhaustion in children and staff (lethargy, excessive sweating, etc.). 7. Communication Notify parents about changes in outdoor routines, sunscreen application, and clothing suggestions (hats, water shoes, etc.). The facility currently operates with a Four Star License that was issued on June 27, 2019 . The last annual compliance visit was conducted on January 8, 2024. The Director and Owner were present and assisted me during the visit. Children were observed participating in free play inside their classrooms and outside on their designated playgrounds. One group of children were observed eating breakfast which consisted of cheese toast, pineapples and milk. The last sanitation inspection was completed May 29, 2024 with a “Superior” classification. The last fire inspection was conducted September 5, 2023. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing.The mulch under the stationary play equipment for school age children is less than 6 inches in depth. .0605(j) 807 A safe indoor and outdoor environment was not provided for the children. There was exposed concrete at the bottom of some of the fence post. 10A NCAC 09 .0601(a) 847 Parent's medication authorization did not include required information. Parental authorization forms were not filled out completely. 10A NCAC 09 .0803(4)(6-9) The violation(s) must be corrected immediately, and a letter must be sent to me no later than July 9, 2024 stating how each item was corrected and how you plan to maintain compliance with the requirement. Please e-mail the letter to me at mona.ndiaye@dhhs.nc.gov . You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Technical Assistance Parental authorization forms for topical medications must have the brand name of the medication and all medication must have specific instruction on when the medication should be used. For prescription medications, the prescription label must be followed and the medication must have a time frame for use. Old toys should be removed from the playgrounds. The playground overall needs to be cleaned. The mulch under the stationary play equipment for school age children is less than 6 inches in depth. Be mindful that the large slide on the playground for school age children can become hot from direct sunlight. You may want to add a sunshade to prevent it from possibly getting too heat. There was concrete exposed around the fence post, these need to be covered to prevent injury. I observe several sand bees flying around the sandbox on the playground, although they are said to not be harmful, they could be scary to the children. Try calling a pest control company to inquire what can be done about the sand bees, try keeping the sand box covered until the children want to use it. Consultation There is a first aid poster located in the hall area near the bathrooms. It is best practice to have a first aid poster in each classroom. The teachers in the infant room cannot easily reference to the poster due to the classroom’s location. It is best practice to label all plants. Tomato plants should not be accessible to young children. I observed three tomato plants on the playground for preschool age children. The Director placed the tomato plants on the other side of the gate where children can still experience the plants growing. Rated License Please be reminded your facility is in cohort one. Your prep year starts July 1, 2023. Your assessment year will begin July 1, 2024 and end June 30, 2025. If you have any questions please contact me.
