The center director, Kathryn Davis, assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 3/12/26 with a ‘Superior’ classification and 3 demerits. The last fire inspection was conducted on 3/20/26. Children were observed playing outside, resting, and routine care activities. The facility does not provide transportation. The file for one new staff member was reviewed. A partial assessment of applicable childcare rules was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. We reviewed the procedures for recording an incident. The DCDEE incident report must be completed for any type of accident or injury. This includes insect bites, human bites, scratches, bumps, falls, etc. Only injuries requiring additional medical care must be sent to me within seven days. If as a GS110 you plan to accept subsidized children, you and your staff will be required to have current health and safety trainings and maintain those for as long as you receive subsidy. Upon my arrival I was greeted by the Administrator, Kathryn Smith, she assisted me during today’s visit. I completed a walk-through of the classroom space, attendance was documented, and observations were made. The infants were observed interacting with the teachers. The older children were observed participating in free play on the outdoor learning environment and in their classrooms. The children in the classroom for 3 to 5 years olds were observed painting. . The last annual compliance visit was conducted December 18, 2023. The center is operating under a Notice of Compliance that was issued February 22, 2022. Permit restrictions were in compliance The last sanitation inspection was conducted February 27, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted February 14, 2024. New staff files and 5 children’s files were monitored. The center does not provide transportation. Your compliance history is 83%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violation was observed during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Parental authorizations for sunscreen and bug repellent had expired permissions. 10A NCAC 09 .0803(4)(6-9) The above violations must be corrected immediately, and a letter must be sent to me no later than November 18, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. Technical Assistance The Administrator and I discussed procedures for conducting fire drills, the actual fire alarm should be used when conducting a drill. We also discussed the safe procedures for drop off and pickup. The center uses Procare for track sign in and out and parents sign in at a kiosk near the entrance. We talked briefly about the church’s plan to build a new child care center. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed. The facility currently operates with a Notice of Compliance that was issued on February 22, 2022. The last annual compliance visit was conducted on December 18, 2023. The Director was present and assisted me during the visit. Children were observed participating in free play in their classrooms. Some of the children were observed eating breakfast which consisted of biscuits, sausage, blueberries and milk. The last sanitation inspection was completed February 27, 2024 with a “Superior” classification. The last fire inspection was conducted February 14, 2024. There are no new staff. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. All children under 15 months of age did not have a feeding plan on file and the plans were not posted. 10A NCAC 09 .0902(a) 807 A safe indoor and outdoor environment was not provided for the children. On the playground for toddlers, there is a concrete area that has exposed raised edges that were not covered to prevent injuries. There is a gap at the bottom of the fence that is wide enough in one area for a child to climb under the fence. 10A NCAC 09 .0601(a) 847 Parent's medication authorization did not include required information. Children under 15 months of age are required to have a feeding plan that is updated as the feeding needs change, the plan must be posted in the classroom. On medication forms for topical medications such as diaper creams, parents need to be specific about when the medication should be used. As needed should not be used and what the parent specifies on the form must be followed. On the playground, monthly checks are required however during the warmer months a daily check should be done as best practice to check for spiders, bees, snakes and other small animals. On the playground for toddlers, the carpet covering the concrete needs to be moved over to cover the edges of the concrete. There is a gap at the bottom of the fence that is wide enough in one area for a child to climb under the fence. This needs to be covered up to close the gap. Name of Operation: A CHILD'S JOURNEY Facility ID: 76000505 Consultant: MONA NDIAYE Operation Type: Center Case Number: Visit Date: 6/13/2024 Number Present: 40 Completed Date: 6/13/2024 Age: From 0 To 5 Total Minutes: 145 Time In: 08:35 AM Time Out: 11:00 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for applicable child care requirements. The facility currently operates with a Notice of Compliance that was issued on February 22, 2022. The last annual compliance visit was conducted on December 18, 2023. The Director was present and assisted me during the visit. Children were observed participating in free play in their classrooms. Some of the children were observed eating breakfast which consisted of biscuits, sausage, blueberries and milk. The last sanitation inspection was completed February 27, 2024 with a “Superior” classification. The last fire inspection was conducted February 14, 2024. There are no new staff. During today’s visit, a partial assessment of applicable child care requirements was conducted. I observed the following violations. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. All children under 15 months of age did not have a feeding plan on file and the plans were not posted. 10A NCAC 09 .0902(a) 807 A safe indoor and outdoor environment was not provided for the children. On the playground for toddlers, there is a concrete area that has exposed raised edges that were not covered to prevent injuries. There is a gap at the bottom of the fence that is wide enough in one area for a child to climb under the fence. 10A NCAC 09 .0601(a) 847 Parent's medication authorization did not include required information. Several topical medications such as diaper creams did not have completed parental authorizations on file. 10A NCAC 09 .0803(4)(6-9) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Three staff did not have current First Aid training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Three staff did not have current CPR training. .1102(d) The violation(s) must be corrected immediately, and a letter must be sent to me no later than June 25, 2024 stating how each item was corrected and how you plan to maintain compliance with the requirement. Please e-mail the letter to me at mona.ndiaye@dhhs.nc.gov . You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Technical Assistance A fire drill should be conducted right before nap time or directly after nap time at least once time per year. Children under 15 months of age are required to have a feeding plan that is updated as the feeding needs change, the plan must be posted in the classroom. On medication forms for topical medications such as diaper creams, parents need to be specific about when the medication should be used. As needed should not be used and what the parent specifies on the form must be followed. On the playground, monthly checks are required however during the warmer months a daily check should be done as best practice to check for spiders, bees, snakes and other small animals. On the playground for toddlers, the carpet covering the concrete needs to be moved over to cover the edges of the concrete. There is a gap at the bottom of the fence that is wide enough in one area for a child to climb under the fence. This needs to be covered up to close the gap. Jeanette Rollins, Lead Teacher, was present and assisted me during today’s visit. I completed a walk-through of the facility, attendance was documented, and observations were made. The children were observed participating in music and movement. Infant were observed participating in free play and interacting with the teacher. The last annual compliance visit was conducted January 13, 2023. The center is operating under a Notice of Compliance that was issued February 22, 2022. The last sanitation inspection was conducted November 7, 2023 the center received a Superior rating. The last fire inspection was conducted February 21, 2023. Fire drills and playground inspections were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. Two violations were observed during today’s visit. Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. In the classroom for two year olds, there were Three (3) one year olds enrolled and present and Two (2) three year olds enrolled and present during today's visit. 10A NCAC 09 .0713(a)(6) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. The teacher was using an app to document the times the infants slept however it did not contain the other required information. .0606(g) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The Director and Teacher working in the classroom for infants did not have current ITS-SIDS training. .1102(f) The above violation must be corrected immediately, and a letter must be sent to me no later than December 28, 2023 stating how each item was corrected and a plan to maintain compliance with the requirement. If I do not receive your letter by December 28, 2023 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter.
