The facility currently operates with a Five Star license that was issued on February 28, 2018. The last annual compliance visit was documented February 10, 2025 The Lead Teachers were present and assisted me during the visit. Children in two of the four classrooms were observed eating lunch. The children in the other two classrooms were observed participating in free play. The last sanitation inspection was completed November 18, 2024 and earned a “Superior” classification. There are no new staff. Fire drills and playground inspections were monitored during today’s visit. During today’s visit, a partial assessment of applicable child care requirements was conducted. Program records, general licensing requirements, staff/child ratios, supervision general safety, medications and storage of hazardous items were monitored. I observed one violation. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. The arrival times were not documented from today in classroom 9. This is a repeat violation from 2/10/2025. 10A NCAC 09 .0302(d)(4) The above violations must be corrected immediately, and a letter must be sent to me no later than May 27, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. If I do not receive your letter by May 27, 2025 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Technical Assistance I spoke with the teacher in space 7 about how the teacher's personal belongings should be stored. Best practice is to lock these items up, if the items are not inaccessible or locked, personal belongings can be inspected for hazardous item that may be stored in them. This includes personal backpacks as well. 15A NCAC 18A .2820 STORAGE (g) Purses and other personal effects belonging to child care center employees shall be kept inaccessible to children and shall be stored in accordance with this Rule, as applicable. Upon my arrival I was greeted by the Lead Teachers, they assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment and attendance was documented, and observations were made. The children were observed participating in free play in their classrooms. This is a NC PreK program located in Trindale Elementary School. The last annual compliance visit was conducted February 27, 2024. The center is operating five star license that was issued February 28, 2018. The license was earned with 7 points in program standards, 7 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted November 18, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. All staff files and six children’s files were monitored. NC PreK requirements were monitored. Your compliance history is 91%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. A one on one staff did not have a current criminal background check. G.S. 110-90.2(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Melody Marley, Lead Teacher, was present and assisted me during today’s visit. I completed a walk-through of the facility, attendance was documented, and observations were made. The children were observed participating in free play outside and in their classrooms and interacting with the teachers. The last annual compliance visit was conducted March 6, 2023. The center is operating under a Five Star License that was issued February 28 2018. The last sanitation inspection was conducted October 23, 2023 the center received a Superior rating. Fire drills and playground inspections were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. One of the files monitored did not have the Summary on file. A copy of the DCDEE form was emailed to Ms. Marley during today’s visit. 10A NCAC 09 .0302 (d)(4) UPDATES -DCDEE has announced their plan to resume the Star Rated License process. Facilities have been divided into 3 Cohorts. Each cohort will have a preparation year and an assessment year. Your facility is in Cohort 1. Your preparation year began July 1, 2023 and will end on June 30, 2024. Your assessment year begins July 1, 2024. The preparation year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. It could be helpful to consider areas where refresher information might be helpful. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous ECERS-R and ITERS-R assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Seek out webinars and/or in-person trainings related to topics of interest. 4. Start a self-study process to become more familiar with the ERS or priority areas relevant to your program 5. Schedule staff meetings to discuss topics related to the Environment Rating Scales 6. Support and encourage staff during a self-study process. Connect them with resources at ncrlap.org 7. Melody Marley, Lead Teacher, was present and assisted me during today’s visit. I completed a walk-through of the facility, attendance was documented, and observations were made. The children were observed participating in free play outside and in their classrooms and interacting with the teachers. The last annual compliance visit was conducted March 6, 2023. The center is operating under a Five Star License that was issued February 28 2018. The last sanitation inspection was conducted October 23, 2023 the center received a Superior rating. Fire drills and playground inspections were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One of the files monitored did not have the Summary on file. GS 110-102 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. There was no sign in/out documentation for the children in the classrooms. 10A NCAC 09 .0302(d)(4) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. A child was observed having a diaper change and the child walked around the classroom and touched several toys before his hands were washed. 15A NCAC 18A .2803(c) 847 Parent's medication authorization did not include required information. In classroom 9, there was cortisone cream that did not have parental authorization on file in the classroom. In classroom 10, there was an Epi Pen and Seizure mediation that did not have a parental authorization on file in the classroom. 10A NCAC 09 .0803(4)(6-9) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. One of the files monitored did not have the documentation of receipt of the discipline policy. .1804(c) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. One of the files monitored did not have this acknowledgement on file . .0608(b)(1-6) The above violation must be corrected immediately, and a letter must be sent to me no later than March 12, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. Consultation Daily records of arrival and departure times at the center for each child which shall be maintained as children arrive and depart. One of the rationales for this requirement is to keep track of all the children in care in case an evacuation has to take place, the electronic attendance does not list times of arrival and departure. In the instance of an evacuation the electronic attendance may not be accessible.This must be maintained daily in each classroom. The facility currently operates under a Five Star License that was issued on February 28, 2018. The last annual compliance visit was conducted on March 6, 2022. The last sanitation inspection was completed February 16, 2023 with a “Superior” classification. I completed a walk-through of the facility where attendance was documented, and observations made. The children were observed playing freely in their classrooms and in the gym. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted which included, General Licensing Requirements, Program Requirements, Staff Records, Staff Child Ratio, Supervision, Medications and General Safety. Today I observed three violation(s) regarding the Child Care Requirements Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space 9 and space 8, there were uncovered outlets. 10A NCAC 09 .0604(c) 898 All electrical appliances were not used in accordance with the manufacturers instruction. Appliances with heating elements, such as bottle warmers, crock pots, curling irons, irons, coffee pots, and/or their cords were accessible to preschool-age children. In space 9, there was a laminator within the children reach. .0604(e) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. One teacher had a Dr. Pepper drink on her desk. .0901(i) The violations were corrected during the visit and discussed with the teachers, no corrective action letter is required.
