A checklist was used to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 7/23/25. Currently this program operates with a five (5) Star License, issued 11/1/21. The facility earned seven (7) points in staff education, six (6) points in program standards, and one (1) quality point for programmatic options met. The center director, Angela Bayle, assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 4/14/26 with a ‘Superior’ classification and 8 demerits. The last fire inspection was conducted on 6/27/25. The inspector was at the center today to complete the inspection, but due to violations, it was not completed. He is planning to return in the afternoon finish the inspection. Children were observed playing in activity areas, eating lunch, resting, and routine care activities. There are currently two mini-buses on site, JDF 4910 and LFS 7444. You stated you are not currently providing transportation and are not planning to resume transportation for school-age children in the fall. A partial assessment of applicable childcare rules was conducted today. Staff records were not monitored today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last completed inspection was 6/27/25. 10A NCAC 09 .0304(a) 542 The written feeding plan was not modified as the child's needs changed. The plans for children in room 2 were not updated based upon children's needs. 10 NCAC 09 .0902(a) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Bayle. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 7/22/25. CONSULTATION Rated License: You have chosen pathway two and I received your application for assessment today. We reviewed the family and community engagement standards today. For five-stars, you are required to do the foundational practices with four additional options. You will keep a QRIS notebook and document how you are meeting the five foundational practices and the four options you have chosen. I left this form with you today to complete. Each pathway requires the completion of the facility CQI plan and individual CQI plan. This will be your professional development plan moving forward. I will be looking to see items 1-4 completed. You will document progress towards goals. If you complete your goals, you will begin a new form. You and your staff will always be working on goals. I left copies of these forms with you today. You currently use Nest Schools curriculum approved by DCDEE. This curriculum does not have an approved formative assessment. We discussed the free assessment tool DRDP today. Lead teachers will be required to complete two assessments on every enrolled preschool child twice annually and offer parent conferences to review. You will document when these are offered. School-age children are not required to have assessments and conferences. Administrative staff and lead teachers will be required to complete five coaching or training hours annually. These will be in addition to your ongoing training hours. The staff/child ratios for five-star under pathway two requires you to operate with reduced/enhanced ratios. Email the center CQI form and the family and community engagement forms back to me when completed. Once all your staff are in WORKS and you have their letters, send these to me. At the top of each letter, indicate the position and years of experience. I will evaluate your education stars. After I have evaluated education, we can discuss your star level. After forms are received and education is complete, we will complete your rated license. Technical Assistance: I will conduct a follow-up visit to complete a review of the staff files. We discussed the new staff training worksheets today. Once these are completed, email them to me. We discussed the storage of emergency medications today. If a child has an action plan, check to see if an over-the-counter medication is listed. This medication will need to be stored in a small locked box and stored with the unlocked emergency medications. The location of the emergency medications should be clearly labeled in the child’s classroom. It is best practice to attach the child’s photograph to the medical action plan. A parent may obtain a letter from the doctor stating that an over-the-counter medication if for emergency use. If this letter is on file in the child’s classroom, the over-the-counter medication does not have to be locked. As a reminder, a child cannot be in attendance if all medications listed on an action plan are not on site. A copy of the Emergency Preparedness Plan must be printed and place in the Ready to Go File for the center. Upon arrival I met with Angela Bayle, center director, and I explained the purpose of my visit. Allegation: There are allegations of violations of childcare requirements. I left a message with the reporter on 9/29/25 regarding the allegations in the report. No response was received. I spoke with Ms. Bayle about the app the center uses to report items to parents. I viewed the child’s information in the app from the classroom mentioned in the report for 9/10, 9/11, and 9/15/25. I reviewed the postings for each day for meals and rest times. I spoke with the staff person present in the morning. She stated the mom brought in pureed food from home for meals. She stated the child ate that morning. The time in the app was 9:36am. She stated he ate all the food and did not act hungry. According to the app, the child ate food from home at 3:44pm. I spoke with the staff person present when the mother arrived. She stated she began offering a bottle approximately five minutes before the mother arrived. The sign out time was 5:30pm. I reviewed the feeding schedule for the child. Bottles were to be offered every 2-3 hours. Conclusion: Based upon the information received during the interviews and observations, a violation of childcare requirements was substantiated. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence) The feeding schedule for one child in the Ducklings 2 classroom was not followed. The child was offered solid food in the morning and afternoon, however, a bottle was not offered until the end of the day. The feeding schedule stated bottles/food offered every 2 to 3 hours. 10A NCAC 09 .0902(a) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Bayle. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 10/14/25. Technical Assistance: Staff need to review feeding schedules frequently to ensure they are offering food/bottles at the times/frequency the parents has stated. This must be done per parents desire, not based on child’s actions. If the child does not eat, then that is what is documented in the app for the parents. Staff can communicate with parents and discuss changes to the feeding schedules and the child’s needs change. Infants must be offered milk/formula throughout the day. A suggestion is to offer milk throughout the feeding of solid foods to balance out the meal. CONSULTATION Once QRIS is implemented, you will receive information from DCDEE regarding the new pathways for licensure and your timeframe for completing your license renewal. The NCRLAP website, https://ncrlap.org, has trainings geared towards the new ECERS3 and ITERS3 assessment scales. These will introduce you to the updated assessment scale and provide tips for completing the new scale. We discussed the annual review of the center’s Emergency Preparedness Plan. The plan is registered under the director’s personal NCID. Ms. Bayle is the administrator and she does not have the NCID to access the center’s plan. You will need to contact NC Risk Management at 800-367-2229. The website is https://rmp.nc.gov. Once you call for assistance, explain that you are the director and you are trying to access the center’s EPR plan. You will explain that you do not have the NCID the plan was created under and will need to change it your NCID. If you do not see your plan once the NCID has been switched, it may be archived and the Risk Management team can move it back to active. I will conduct a follow-up visit within ten days to complete a review of the staff files. We discussed the storage of emergency medications today. Upon my arrival I was greeted by the Regional Director, Corine Brylski, she assisted me during today’s visit. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed participating in free play indoors and outdoors. The school age children were observed having a talent show in their classroom. The children were observed eating lunch which consisted of cheese ravioli, green beans, peaches and milk. The last annual compliance visit was conducted August 11, 2023. The center is operating five star license that was issued November 1, 2021. The license was earned with 6 points in program standards, 7 points in education and 1 quality point for having a staff benefits package. All permit restrictions were in compliance The last sanitation inspection was conducted June 6, 2024 the center received a Approved rating. Fire drills and playground inspections were monitored. The Department of the Secretary’s website was used to check the status of the corporation. The facility’s owner is current/active. All the required documents were posted. Transportation requirements were in compliance. Your compliance history is 96%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violation was observed during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Two topical medications did have authorizations on file and two emergency medications had expired authorizations on file. 10A NCAC 09 .0803(4)(6-9) The above violation must be corrected immediately, and a letter must be sent to me no later than August 10, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. Technical Assistance There were two topical medications and two emergency medications that did not have authorization form or the form was expired. The Regional Director, removed those medications from the classrooms, she also stated medication will be reviewed with administrative staff before going to the classrooms. All parents were contacted and informed about the medications. A follow-up visit will be conducted in the future to review the children and staff files. Have the staff and training worksheets completed. This visit summary was reviewed with you and provided to you. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed.
