A checklist was used to note childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 6/24/26. This family childcare home currently operates with a two (2) star license issued 11/12/19. The owner/operator Lisa Grooms assisted me during today’s visit. Four preschool children are enrolled. Care is provided for first shift. Three preschool children were present today. The file for one new child were reviewed. The children use two downstairs rooms and bathroom as designated childcare spaces. A full assessment of applicable childcare requirements was conducted today. The following violations were observed today: Violation Number Comment Rule 929 Developmentally appropriate activities for the ages of children in care, were not provided as planned on the written schedule and activity plan. An activity plan was not posted for the children in care. Technical assistance was provided. G.S.110-91(12) & .1718(a)(6) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Grooms. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 7/8/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to lori.rierson@dhhs.nc.gov. CONSULTATION: We reviewed the family and community engagement standards today. For two-stars, you are required to do the foundational practices with no additional options. We discussed how to document these practices by adding a family policy to your parent handbook on how you are incorporating the practices. Review your current handbook to see how you may already be implementing these. You will send me an email with each bullet item and how you are implementing this into your program. Each pathway requires the completion of the facility CQI plan. This will be your professional development plan moving forward. You will complete steps 1-4 and email back to me. Once you complete the goal selected, you will begin a new form with a new goal. Your current WORKS evaluation is for lead teacher. You must have your education evaluated as an FCCH provider. You will need to contact WORKS to see if you need to resubmit your transcripts. We attempted to reset your password today. WORKS does not identify your NCID as Individual User. I gave you the contact number for NCID. You will need to contact them and ask them to verify that you have an Individual NCID. You will need this for criminal background check as well. On 6/3/26, Ms. Grooms and I spoke, and she indicated she wanted to choose pathway 2. The CQI and Family and Community Engagement forms were emailed that day. We discussed the forms again today. Once I receive these, I can begin to process your new rated license. I received the Application for Assessment today. You stated you are planning to use the Gee Wiz curriculum. A copy of the current activity plan reflecting incorporation of this curriculum must accompany the corrective action plan. The criminal background letter for you and Mr. Grooms will expire 8/18/26. It is recommended to begin the renewal process at least 45 days prior to expiration to avoid any delays in obtaining the new letter. You stated you have an infant enrolling soon. You must have a feeding schedule posted and document sleep checks on the DCDEE sleep check form. All bottles must have the child’s name and date on them. The home was placed in inactive status from 2/10/25 – 9/2/25. Five preschool children are enrolled. Care is provided for first shift. Four preschool children were present today. The provider's training expired 4/2025. She was on inactive status until 9/2/25. The training was completed 9/18/25. .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. The training was completed 9/18/25. .1703(a)(3) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. An aerosol can of disinfectant and an air freshener were stored in an unlocked cabinet in the kitchen area. .1719 (a)(7) 811 Baby food, snack items, and meal items sent from home, for each individual child, were not dated and labeled with the child's name. A bottle and sippy cups in the refrigerator brought from home by the children were not labeled with each child's name and the date. .1706(o) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. The file for one child did not have the signed copy of the policy on file. .1726(b)&(c) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Grooms. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 2/4/26. CONSULTATION: All family childcare homes with a two – five star rated license are required to choose a Pathway for obtaining a new rated license. We discussed these options today and completed the Pathways decision form. I will schedule a technical assistance visit to review each pathway in detail and your options. We discussed the Emergency Preparedness Plan today. You completed the training on 10/29/25. The online plan must be completed by 2/28/26. The website is https://rmp.nc.gov. Once all bullet items are green, you will save, publish and print your plan. We reviewed requirements for daily schedules and activity plans today. You may combine the two into one document. All the requirements for each must be met. It must reflect blocks of time for the required developmentally appropriate activities. They must reflect the five developmental domains in the NC Foundations for Early Learning book. You may refer to NC Childcare rule 10A NCAC 09 .1718(6) and (c) for the rule requirements. We discussed the use of screen time today. Children under three years of age are not allowed to use screen time. Children over three may have screen time up to thirty minutes per day. It must be for a developmental purpose and the child’s time recorded on a screen time log. Family childcare home providers must visit the Clean Classrooms for Carolina Kids to register for the asbestos training and testing. You will login to the same account used for your lead water testing. The website is https://www.cleanwaterforuskids.org/en/carolina/. The lead water test completed reflected trace amounts of lead. Remediation was not required but was recommended. You stated you use water filtration in your home. The emergency medical care plan should be updated. Line two can state call 911 or child’s doctor. The posted ITS/SIDS policy was not current. A current policy must be posted and provided to parents if a child under twelve months enrolls. Any bottles or cups from home must have the child’s name and date. A separate cup must be used for each meal unless they are washed in a dishwasher or three compartment sink in between uses. We discussed the CACFP requirements for beverages served at meal times. Children cannot be served juice during lunch. They are required to be provided milk or a milk substitute if documentation is on file for the reason of the substitution. We discussed the requirements if you decided to administer medication. You would need to develop a medication administration policy. The parents of all enrolled children must receive the policy and a signed/dated copy placed in the child’s file. The policy must be added to the parent handbook. Aerosol cans or those with warnings in addition to ‘Keep Out Of Reach Of Children’ must be in locked storage. Locked storage rooms and cabinets shall include those which are unlocked with a combination lock, electronic or magnetic device, keypad, key, or equivalent locking device. Keys and electronic or magnetic unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. The program currently operates with a two star license that was issued on November 12, 2019. The last annual compliance visit was conducted February 8, 2024. Upon arrival at the home, I was greeted by the provider, Lisa Grooms, who was present with three preschool children. The children were observed participating in free play in the designated child care space. During the visit I monitored indoor and outdoor space. The provider uses the finished basement of the home as the playroom space. The space included a variety of age appropriate toys and materials. The provider’s license and required information was posted. The provider’s file, children’s files, outdoor inspection and fire drill report were reviewed. There was one new children enrolled since the last visit. The provider only provides transportation for emergencies. The provider stated there are no other residents in the home besides her daughter and husband, listed on the verification sheet. Your compliance history is 94% before this visit. The following violations were observed Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. One of the provider's dogs did not have a current rabies vaccination on file. .1719(b)(1) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The provider has not completed her health and safety trainings as required every 5 years. .1703(d)(2) 2058 The family child care home operator did not collect and submit samples of water from each outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. The provider has not started the process. 10A NCAC 09 .1725(a)(2) The above violations must be corrected immediately and a letter must be sent to me no later than February 16, 2025 stating how each item was corrected.
