    Name of Operation: Young Scholars Academy of Cary Facility ID: 92004022 Consultant: CORRINE FULP Operation Type: Center Case Number: Visit Date: 11/4/2025 Number Present: 44 Completed Date: 11/4/2025 Age: From 0 To 5 Total Minutes: 180 Time In: 10:00 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit. Upon arrival, Amy Pruitt, administrator assisted me with the visit. CURRENT LICENSE STATUS: The program operated with a one-star License, issued May 19, 2025. ANNUAL INSPECTIONS: The last sanitation inspection was completed on 3/7/25. The last fire inspection was completed on 10/23/25. The last outdoor play area inspection was completed on 10/13/25. The last fire drill was conducted on 10/31/25. The last lock down/shelter-in-place drill was completed on 8/27/25. The EPR plan was updated on 3/28/25. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old are enrolled. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements. I viewed two (2) staff files and five (5) children files. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of December 2024 through November 2025 was viewed during today’s visit. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 100%. The NC Secretary of State's website was reviewed on 11/3/25 and Children's Hearts and Minds LLC. Was listed as current/active. You visited each indoor and outdoor space with me. I observed children engaged in free play, small group activities, and outdoor play. The following violations of the Child Care Requirements were documented during today’s visit. Violation Number Comment Rule 532 All children were not held or placed in feeding chairs or other appropriate apparatus to be fed. Two infants were being fed at the same time while the staff member was sitting on the floor. 10A NCAC 09 .0902(b) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Infant hands were not washed before feeding their bottle. 15A NCAC 18A .2803(c) 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. A bottle of hand lotion was not inaccessible to children. .0604(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A key was stored in the utility closet and bleach was not stored in a locked cabinet. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. An Epi pen for RB expired on 10/2025. .0803(12) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. If I do not receive your letter by 11/18/25, and/or if you do not address all violations, I will be making a return visit to your program to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. TECHNICAL ASSISTANCE I observed two classrooms with regular pillows for the cozy areas; to avoid the spread of germs and having to wash them each day you could have someone stitch a plastic case to enclose the pillows. In one of the staff bathrooms a bottle of lotion was stored within the reach of a child. The hand lotion does not have to be locked in storage, but it must be stored 5 feet from the floor. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. The provider access to ABCMS will allow administrators to: To get access, you must complete the ABCMS Child Care Provider Portal Training in Moodle. HEALTH AND SAFETY Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Staff should use their individual NCID username and password to access this training. All staff members shall complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. This training counts toward requirements for Health and Safety. Recognizing and Responding to Suspicions of Child Maltreatment training is available at www.preventchildabusenc.org.
