    Name of Operation: STELLA LOWERY SMALL DAY CARE HOME Facility ID: 92000022 Consultant: CORRINE FULP Operation Type: Family CC Home Case Number: Visit Date: 3/18/2025 Number Present: 3 Completed Date: 3/18/2025 Age: From 1 To 4 Total Minutes: 90 Time In: 09:45 AM Time Out: 11:15 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to conduct a routine unannounced monitoring visit. Upon arrival, I was greeted by Stella Lowery. We conducted a walk-through of the indoor learning environments. I walked the outdoor play area while the provider was caring for the children. I observed three children engaged in free play activities. LICENSE STATUS: The program currently operates with a 1 Star License, issued on September 27, 2013. INSPECTION/SAFTEY DRILL STATUS: The last outdoor play area inspection was completed on 2/25/25. The last fire drill was conducted on 2/24/25. The last documented emergency drill [shelter-in-place] was conducted on 1/14/25. The EPR plan was last updated on 3/18/25. The last annual compliance visit was conducted on 9/5/24. A partial monitoring of applicable child care requirements was conducted, monitoring Supervision, Staff Child Ratio, CPR/First Aid, Special Training, Storage of Hazardous Substances, Storage of Medication, Adequate Approved/Space, Staff Records, Program Records, License Posted, and Permit Restrictions. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. Cisco [dog] vaccination expired on 2/7/25. .1719(b)(1) 1404 Medication remaining after the course of treatment is completed, after authorization is withdrawn. or after authorization has expired, was not returned to the parent/guardian. Any medication not retrieved by parent/guardian within 72 hours of completion of treatment, or withdrawal of authorization, was not discarded. A child that not longer attends medicine was stored on a shelf. 10A NCAC 09 .1720(c)(5) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you will correct the violations, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. If your letter is not received by 4/1/25, and you have not addressed all violations, a return visit will be made to monitor noncompliance violations and to obtain your compliance letter. Additional violations may be documented if corrections have not been addressed. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 97%. TECHNICAL ASSISTANCE MEDICATION A child that no longer attends the family child care home medication was stored on a shelf. Unused medications should be returned to the parent/guardian for disposal. In the event medication cannot be returned to the parent or guardian, it should be disposed. VACCINATION Animals should receive regular care from a licensed veterinarian. Dogs should receive routine rabies vaccinations. Please include proof of rabies vaccination for the dogs, according to local or state requirements. Send a copy to me with your correction letter. It will be time to update your EPR Plan in September. You can go to the Risk Management Portal at NC.gov https://rmp.nc.gov › portal. CONSULTATION DCDEE UPDATES AND NEWS CRC Qualification Letters are now valid for 5 years from the date of issuance. Re-Qualification can be started up to 6 months before expiration; start early as some areas are still experiencing delays in fingerprinting appointments. HEALTH AND SAFETY TRAINING Training in all the required health and safety topics must be completed at least every 5 years. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Medication Administration is a safety topic that you would have to sign up for. You should use your NCID username and password to access this training. Recognizing and Responding to Suspicions of Child Maltreatment is a listed topic of Health and Safety training and must also be completed every 5 years. This training is available at www.preventchildabusenc.org.
