Upon arrival Diane Mosorjak, administrator assisted me with the visit. CURRENT LICENSE STATUS: The program operated with a five-star License, issued November 17, 2023. ANNUAL INSPECTIONS: The last sanitation inspection was completed on 10/17/24. The last fire inspection was completed on 9/12/25. The last outdoor play area inspection was completed on 11/13/25. The last fire drill was conducted on 11/11/25. The last lock down/shelter-in-place drill was completed on 11/7/25. The EPR plan was updated on 5/15/25. The last annual compliance visit was conducted on 1/8/25. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old are enrolled. The director stated that they are no longer caring for school ager. The buses were not monitored for transportation; according to the owner they are just parked on the premises at this time. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements. I viewed seventeen (17) staff files and fifteen (15) children files. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of January 2024 through December 2025 was viewed and in compliance during today’s visit. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 85%. The NC Secretary of State's website was reviewed on 12/8/25 and Green Acorn LLC. was listed as admin. Dissolved. The owner shared documents with me today that the process is being viewed and the annual report will be emailed to me when she receives it. You visited each indoor and outdoor space with me. I observed children engaged in free play, small group activities, and lunch preparation. The following violations of the Child Care Requirements were documented during today’s visit. Violation Number Comment Rule 604 Lavatories were not kept clean, in good repair and kept free of storage. In one child's bathroom classroom materials were stored on the floor. 15A NCAC 18A .2818(a) 847 Parent's medication authorization did not include required information. A child's authorization form did not include the approval date. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A container of Aquaphor expired on 8/8/25 and was not returned to the parent .0803(12) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. Thirteen employees did not have verification for their annual EMC plan review. 10A NCAC 09 .0802(a) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. Thirteen employee did not have verification on file for their annual EPR review. .0607(f) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. An employee hired on 7/21/25 did not have a signed acknowledgement on file. .0608(d)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A staff member hired on 7/21/25 did not complete the training within 90 days. .1102(g) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. If I do not receive your letter by 12/23/25, and/or if you do not address all violations, I will be making a return visit to your program to monitor for compliance of violations and to obtain your compliance letter. Only essential items like toiletries, diapering supplies (in childcare), or cleaning supplies (in locked cabinets) are permitted. CONSULTATION AND RESOURCES: Today we discussed the pathway you decided to use. Congratulations on your COGNIA accreditation. Please send me a copy of the certificate and the application. You can go the website or email me for the forms. North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. The provider will have access to see background check status for all employees, add/delete applicants on their roster, and print rosters. To get access, you must complete the ABCMS Child Care Provider Portal Training in Moodle. HEALTH AND SAFETY Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Staff should use their individual NCID username and password to access this training. All staff members shall complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. This training counts toward requirements for Health and Safety. Upon arrival, I was greeted by Diane Mosorjak, and she accompanied me throughout the visit. CURRENT LICENSE STATUS: The program operates with a 5 star-rated License issued on 2/21/19. They received 6 points in program standards, 7 points in staff education and 1 quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. ANNUAL INSPECTIONS: The last sanitation inspection was completed on: 10/17/24. The last fire inspection was completed on 8/13/24. The last outdoor play area inspection was completed on 12/12/24. The last fire drill was conducted on 12/13/24. The last lock down/shelter-in-place drill was completed on 12/17/24. The EPR plan was updated on 10/3/24. The last annual compliance visit was conducted on 2/15/24. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 90%. Prior to the visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Green Acorn, LLC. was active/not current. You visited all approved indoor/outdoor spaces with me during today’s visit. Children were engaged in classroom activities and routine care. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements and applicable enhanced requirements including but not limited to approved space, child interactions, required in-service trainings, indoor and outdoor materials and equipment, storage of hazardous items, storage of medication and medication forms, children records, Staff Criminal Record Checks, attendance records, CPR and First Aid certificates, activity plans and supervision. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of February 2024 through January 2025 was not reviewed during today’s visit. An additional visit will be made to review the training worksheets. Four new staff files were viewed, and no violations were noted. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. A bottle in space #1 was not dated with today's date. 15A NCAC 18A .2804(d) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #10 a aerosol container was stored on a shelf in the children's bathroom. .2820(b) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. Toxic substances, or any product which is under pressure in an aerosol dispenser, shall be kept in the original container and stored in a locked storage room or cabinet when not in use. CONSULTATION AND RESOURCES: ANNUAL LICENSE FEE Per North Carolina General Statute § 110-90(1a), licensed child care facilities are assessed an annual license fee. The amount of your annual license fee is based on your licensed capacity. Invoices were emailed to the facility and online payments are due. The center director, Diane Mosorjak, assisted me with today’s visit. We conducted a walk-through of the indoor and outdoor learning environments. I observed groups of children engaged in free play in their classrooms and on the playgrounds. In addition, I observed diapering and rest time in an infant classroom. The program moved from their previous location in November 2023. This is the first visit monitoring visit at their new location. RATED LICENSE STATUS The program operates with a 5 star-rated License issued on 11/17/23. The program moved to a new location and they were issued this new license. Because the center was in a “held harmless state,” the license was issued based on the points received on their previous rated license. The last rated license assessment was conducted 2/21/19. They received 6 points in program standards, 7 points in staff education and 1 quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. Full rated license assessments will occur based on the program’s Cohort assignment; your program is in Cohort 2, therefore your assessment period would begin on or around July 1, 2025. You should begin preparing and planning for your assessment on or after July 1, 2024, if you wish to complete the Environment Rating Scales. We discussed various resources available to assist you in preparing for your rated license assessment. You should also ensure that all staff have registered and have their education assessed in WORKS. It is recommended that you complete this process as part of your on-boarding process with new hires and as existing staff members complete additional coursework. INSPECTIONS The last sanitation inspection was conducted 11/9/23. The last fire inspection was conducted 8/31/23. The last documented fire drill was conducted 2/1/24 The last documented shelter-in-place or lockdown drill was conducted 1/31/24. The last documented playground inspection was conducted 1/23/24. The status of the owners Green Acorn, LLC., is listed as current and active on the NC Secretary of State website. All applicable child care requirements were assessed during today’s visit. I confirmed today that all required center postings were completed, the EPR plan was updated and staff had been trained on the EPR plans for their new visit. I also assessed their 2 mini-buses used to transport school-age children. The vehicle with NC license plate PS APEX, was inspected in August 2023; the vehicle with NC license plant PS APEX1 was last inspected in February 2023, they plan to have it inspected this week. The vehicle insurance for both vehicles is valid until July 2024. Staff on-going training was assessed from July 1, 2023 – February 29, 2024. The program is completing their annual compliance visit outside of their previous annual compliance date, so the required training hours for this visit was prorated. The following violation observed and recorded during today’s visit. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. One child's emergency inhaler had expired January 2024 10A NCAC 09 .0803(1)(d) This violation was corrected during today’s visit, so a compliance letter is not required. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 93%. TECHNICAL ASSISTANCE One child’s inhaler expired in January 2024 and the medication permission form for this medication expired in December 2023. One of the center’s administrators routinely checks the medication and forms and the director stated that she must have missed that medication when it was last reviewed. This medication was removed from the classroom, so a compliance letter is not required. CONSULTATION There are new Sanitation Requirements effective 7/1/23. You should review their website or contact your Environmental Health Specialist for more information. Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Medication Administration is also a required health and safety training topic. This training is available through Moodle as a separate training. Staff should use their individual NCID user name and password to access this training. Staff should use their individual NCID user name and password to access this training. If you are unable to access Moodle, you can also complete this training on Pro Solutions or through a local vendor. Recognizing and Responding to Suspicions of Child Maltreatment must also be completed at least every 5 years. There is a link to this training on Moodle. The expiration date on staff qualification letters has been extended by two years. New letters reflecting this change will not be sent. However, I will note this change as I make visits to the center.
