Upon arrival, Laura Funderburk, administrator assisted me with the visit. CURRENT LICENSE STATUS: The program currently operates with a five-star accreditation License, issued August 2, 2024. ANNUAL INSPECTIONS: The last sanitation inspection was completed on 8/21/25. The last fire inspection was completed on 2/2/25 The last outdoor play area inspection was completed on 1/5/26. The last fire drill was conducted on 12/19/25. The last lock down/shelter-in-place drill was completed on 12/18/26. The EPR plan was updated on 1/6/26. The last annual compliance visit was conducted 2/5/25. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements. I viewed twenty (20) staff files and eighteen (18) children’s files. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of February 2025 through January 2026 was viewed and in compliance during today’s visit. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 100%. The NC Secretary of State's website was reviewed on 1/11/26 and was listed as current-active. You visited each indoor space with me. Children were resting during nap time when I arrived. Children under twelve months old receive care according to their individual needs including nurturing and individual one-on-one care. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. A child's bottle in space #2 was not dated with the current date. 15A NCAC 18A .2804(d) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A container of Destine cream expired 10/2025 and was not returned to the parent. .0803(12) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). An employee [Amber Williams] did not have a current letter on file. G.S. 110-90.2(b) & .2703(n)&(o) Please submit a corrective action letter for the above violations. While the documented; allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. If I do not receive your letter by 1/28/26, and/or if you do not address all violations, I will be making a return visit to your program to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. TECHNICAL ASSISTANCE During today’s visit an employee, Amber Williams (hired 11/14/22) did not have a qualified letter on file. DCDEE will allow the child care provider or uncompensated provider fourteen (14) days to get a new criminal background check completed and a copy of the qualification letter on file. Upon receiving your letter a copy must be mail to the consultant at 1781 Indian Camp Road Clayton, NC 27520. CONSULTATION AND RESOURCES: Discussion of the pathway was not necessary, as your license was updated on July 26, 2024, and remains valid through July 26, 2027. ABCMS PROVIDER PORTAL North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. The provider will have access to see background check status for all employees, add/delete applicants on their roster, and print rosters. To get access, you must complete the ABCMS Child Care Provider Portal Training in Moodle. HEALTH AND SAFETY Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Staff should use their individual NCID username and password to access this training. All staff members shall complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. This training counts toward requirements for Health and Safety. Recognizing and Responding to Suspicions of Child Maltreatment training is available at www.preventchildabusenc.org. A staff member failed to provide appropriate discipline when the staff member hit a five-year-old child on the head with a book, pinched a child and squeezed a child's shoulders. .1803(a)(1) 907 Discipline was related to food, rest or toileting.
