The director, Mrs. Shannon Page, assisted me on today's visit. Upon arrival, you greeted me and welcomed me into your program. Eight (8) classrooms were opened today. I conducted a general walk-though of the indoor and outdoor learning environment. I observed the children having free play, reading books, exploring their outdoor learning environment, and eating lunch in their indoor environment. The infants were having one on one with their teacher, eating and resting their bodies. RATED LICENSE STATUS The program holds a 5-star-rated license issued on 9/30/19. The program received 6 points in Program Standards, 7 points in Staff Education, and 1 quality point through the programmatic options of having a staff benefits package and an infrastructure of parent involvement. INSPECTIONS The last sanitation inspection was conducted on 8/7/23. The last fire inspection was conducted on 8/7/23. The last fire drill was conducted on 5/16/24. The last emergency drill was conducted on 5/2/24. The last playground inspection was conducted on 6/4/24. During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. I did not assess transportation requirements because the program does not transport children. The following violation was observed, recorded, and corrected during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The seat on the tricycle was cracked on the older playground. 10A NCAC 09 .0601(a) COMPLIANCE HISTORY Prior to today's visit the program's compliance history was 96%. It will be update after today's visit and you will be informed of the new score. TECHNICAL ASSISTANCE The following items were discussed to help you prevent a reoccurrence of the violations observed during today's visit. I would suggest informing your teachers to make sure they double check for broken toys on a daily basis. CONSULTATION Some providers are experiencing delays in scheduling their fingerprinting appointments. Criminal record checks can be completed up to six months prior to the expiration date. We are encouraging all staff to begin their renewal process early to avoid lapses in their qualification. HEALTH AND SAFETY TRAININGS Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings dare provided free of charge. You should use your NCID username and password to access the Moodle portal. As per Child Care Rule .1103(b) the Health and Safety trainings must be renewed at least every 5 years. Some of your staff completed the trainings in 2017, they should begin to retake the training. Name of Operation: EVERBROOK ACADEMY Facility ID: 92003614 Consultant: AMBER TELFAIR Operation Type: Center Case Number: 0324-316A Visit Date: 3/25/2024 Number Present: 89 Completed Date: 3/25/2024 Age: From 0 To 5 Total Minutes: 65 Time In: 10:00 AM Time Out: 11:05 AM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of this unannounced visit was to investigate allegations of violations of child care requirements at this child care facility. Shannon Page, Administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Page and an additional staff member. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 491 Caregiver did not respond at the earliest opportunity to an infant or toddler’s physical and emotional needs. On March 21, 2024, two staff members repeatedly failed to respond to a three-month-old child as the child cried for extended periods of time. .0511(b)(1) 904 Child was handled roughly. On March 21, 2024, a staff member failed to attend to a three-month-old child in a nurturing and appropriate manner when the child was bottle-fed in forceful manner by squeezing liquid in the child’s mouth while the child waved their arms, kicked, arched their back, and cried. The staff member held down the child’s arms and legs and continued to reposition the bottle in the child’s mouth without attempting to calm the child from crying. The staff member held a cloth/bib around the bottle and over the child’s mouth while feeding. .1803(a)(1) 1810 There was a substantiation of child maltreatment. On August 20, 2024, the Division substantiated child maltreatment regarding an incident in which a staff member failed to meet the physical needs of a child.
