Name of Operation: CHILDREN'S LIGHTHOUSE OF APEX Facility ID: 92003731 Consultant: CORRINE FULP Operation Type: Center Case Number: Visit Date: 11/6/2024 Number Present: 77 Completed Date: 11/6/2024 Age: From 0 To 5 Total Minutes: 360 Time In: 10:00 AM Time Out: 04:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit. Upon arrival, I was greeted by Asha Berry, and she accompanied me throughout the visit. I observed groups of children playing indoors, participating in small/large group free play activities. I also observed diapering and hand washing practices. CURRENT LICENSE STATUS: The program operates with a 5-star-rated license issued on 1/5/22. They received 6 points in program standards, 7 points in staff education and one quality point through the programmatic option of having an infrastructure for parent involvement and a staff benefit package. ANNUAL INSPECTIONS: The last sanitation inspection was completed on: 2/21/24. The last fire inspection was completed on 1/5/24. The last outdoor play area inspection was completed on 10/7/24. The last fire drill was conducted on 10/16/24. The last lockdown/shelter-in-place drill was completed on 9/24/24. The EPR plan was updated on 12/5/23. The last annual compliance visit was conducted on 12/5/23. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 93%. Prior to the visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, REYKHA CARE AND EDUCATION LLC. was active/not current. Your business currently is delinquent in filing its annual report, to avoid any further action please contact the NC Secretary of State customer service department at (919) 814-5350. A copy of the notice was given to you today. I observed the owner calling the customer service department during today’s visit and the notification has been updated to current status. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements and applicable enhanced requirements including but not limited to approved space, child interactions, required in-service trainings, indoor and outdoor materials and equipment, storage of hazardous items, storage of medication and medication forms, children records, Staff Criminal Record Checks, attendance records, CPR and First Aid certificates, activity plans and supervision. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of December 2023 through November 2024 was reviewed during today’s visit. All staff who was employed for more than 1 year have completed their required training hours for this year. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. A purse in space #3 was stored less than five feet from the floor. .0604(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags were stored accessible to children younger than three years of age. .0604(q) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. One staff file did not have documentation on file for EMC review. 10A NCAC 09 .0802(a) 1313 Emergency information did not include name, address, and telephone number of parent or other emergency contact person. One child's file did not have a doctor listed for emergency purposes. .0802(c)(1) 1314 Emergency information did not name childs health care professional. Two children's files did not have a hospital preference listed. .0802(c)(2) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. One staff file did not have documentation on file for EPR review. .0607(f) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. If I do not receive your letter by 11/20/24, and/or if you do not address all violations, I will be making a return visit to your program to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. TECHNICAL ASSISTANCE One staff member’s purse was stored less than five feet from the floor. Early care and education programs should store any potentially toxic substances behind doors/cabinets with child-resistant locks/latches. A personal item such as an employee’s purse could potentially have medication or other toxic chemicals. A locked cabinet or room that children cannot open or enter is best, but it must be locked all the time. Two classrooms where children were younger than two years of age; plastic bags were stored within their reach. Plastic bags, whether intended for storage, trash, diaper disposal, or any other purpose, should be stored out of reach of children. Plastic bags have been recognized for many years as a cause of suffocation. In space #5 there were only 3 books accessible for children to read on the language center. The indoor learning environment should be rich in first-hand experiences that offer opportunities for language development. They should also have an abundance of books of fantasy, fiction, and nonfiction, and provide opportunity for the children to relate to stories. The owner placed additional books in the library center that was stored in a closed classroom. CONSULTATION AND RESOURCES: HEALTH AND SAFETY Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Staff should use their individual NCID username and password to access this training. All staff members shall complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. This training counts toward requirements for Health and Safety. Recognizing and Responding to Suspicions of Child Maltreatment training is available at www.preventchildabusenc.org. Child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE criminal background check system. The ABCMS portal allows administrators to: •See the real-time background check status of staff members. •Run a printable report of the staff roster to assist with compliance visits. •See new background check applicants and add to staff roster. To get started, just complete the ABCMS Child Care Provider Portal Training in Moodle.
