Name of Operation: BRIGHT START CHILD CARE LLC Facility ID: 92003424 Consultant: CORRINE FULP Operation Type: Family CC Home Case Number: Visit Date: 5/7/2026 Number Present: 5 Completed Date: 5/7/2026 Age: From 1 To 6 Total Minutes: 180 Time In: 10:30 AM Time Out: 01:30 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit. Upon arrival, I was greeted by Janaki Shetty, and we conducted a walk-through of the indoor learning environment. I walked the outdoor play area; the provider was engaged in group time activities in the classroom. I observed five children, four children were between the ages of 12 months and three years of age. One child was school age. The children were engaged in free play and group time activities. LICENSE STATUS: The Family Child Care Home currently operates with a five (5) star license that was issued 11/14/19. The facility is licensed for 8 children on all three shifts and does not provide transportation. INSPECTION/SAFTEY DRILL STATUS: The last outdoor play area inspection was completed on 4/10/26. The last fire drill was conducted on 4/10/26. The last documented lockdown/shelter-in-place drill was conducted on 2/11/26. The EPR plan was last updated on 6/9/25. The last annual compliance visit was conducted on 6/12/25. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements. Five children’s file was viewed today. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 603 Child(ren) ages 15 months and older went more than four hours without a meal or snack being provided. A child that was 12 months of age did not have a feeding scheduled posted. .1706(h) 716 Electrical outlets not in use were not covered. An outlet in the space where a child had access was not covered. 10A NCAC .1719(a)(27) 908 Health questionnaire was not completed annually. The caregiver did not have an annual health questionnaire on file. .1703(a)(1) 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. One child's file did not have a signed discipline statement. G.S.110-91(10); .1727(a)&(b) Please submit a corrective action letter for the above violations. While the documented; allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. If I do not receive your letter by 5/21/26, and/or if you do not address all violations, I will be making a return visit to your program to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. TECHNICAL ASSISTANCE During my walk through a child that just turned one year of age did not have a feeding schedule posted. North Carolina child care rule stated that an individual written feeding plans for children under 15 months old must be posted for reference by caregivers. These plans, which include milk/formula types, food amounts, and frequencies, are required for child care centers and must be updated as feeding habits change. They should be posted until the child reaches 15 months of age. Your file didn’t have a yearly health questionnaire. All caregivers must complete a health questionnaire to ensure they are mentally and physically fit to care for children or clients. This requirement includes an annual review for all caregivers including substitutes, and volunteers in family child care homes to document health status and any communicable diseases. REMINDER: The emergency lockdown/shelter-in-place drill is due this month. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 96%. RATED LICENSE APPLICATION INFORMATION The provider submitted an application for Pathway #3 [Classroom and Instructional Quality] in the new QRIS system. The following items will need to be monitored and/or completed) Family and community Engagement standards and b) Continuous Quality Improvement/Professional Development Plan. Today, we discussed your pathway in detail, and you were given the opportunity to ask questions and voice any concerns you had. You decided to switch pathways from the one you selected on December 4, 2025. You also allowed me to review your curriculum, The Creative Curriculum – 2nd Edition. Additionally, you decided to use the free formative assessment, Desired Results Developmental Profile (DRDP). According to the information you presented to me today, you stated that your timeline for completing the star-rated license package would potentially be around June 2026. HEALTH AND SAFETY TRAINING Training in all the required health and safety topics must be completed at least every 5 years. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle.
