Name of Operation: ANGELS' GARDEN HOME DAYCARE Facility ID: 92003202 Consultant: CORRINE FULP Operation Type: Family CC Home Case Number: Visit Date: 1/28/2026 Number Present: 4 Completed Date: 1/28/2026 Age: From 1 To 2 Total Minutes: 180 Time In: 12:00 PM Time Out: 03:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit. Upon arrival, I was greeted by Jessica Ormaza, and we conducted a walk-through of the indoor learning environment. I walked the outdoor play area; the provider was engaged in group time activities in the classroom. I observed four children engaged in free play. The children were preparing for nap transition when I arrived. LICENSE STATUS: The Family Child Care Home currently operates with a three (3) star license that was issued 7/24/2017. The facility is licensed for 8 children on 1st shift and does not provide transportation. INSPECTION/SAFTEY DRILL STATUS: The last outdoor play area inspection was completed on 1/5/25. The last fire drill was conducted on 12/17/25. The last documented lockdown/shelter-in-place drill was conducted on 10/2/25. The EPR plan was last updated on 3/3/25. The last annual compliance visit was conducted on 3/5/25. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements. One child’s file was viewed today. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 1404 Medication remaining after the course of treatment is completed, after authorization is withdrawn. or after authorization has expired, was not returned to the parent/guardian. Any medication not retrieved by parent/guardian within 72 hours of completion of treatment, or withdrawal of authorization, was not discarded. A container of diaper cream with an expiration date of 9/2024 was not returned to the parents. 10A NCAC 09 .1720(c)(5) 2024 The health and safety training did not include all of the required topic areas. The most recent medication administration training was completed on May 10, 2018. .1703(c )(1-11) Please submit a corrective action letter for the above violations. While the documented; allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. If I do not receive your letter by 2/11/26, and/or if you do not address all violations, I will be making a return visit to your program to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. TECHNICAL ASSISTANCE During today’s visit, it was noted that one of the Health and Safety trainings was completed on 5/10/18. Training in all required Health and Safety topics must be completed at least every five years REMINDER: The emergency drill [shelter-in-place] or [lockdown] is due by 1/30/26. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 96%. CONSULTATION Today, we discussed the available pathways, and you were given the opportunity to ask questions and voice any concerns. You stated that you will be using Pathway #2 (Classroom and Instructional Quality Pathway) for your rated license process. HEALTH AND SAFETY TRAINING Training in all the required health and safety topics must be completed at least every 5 years.
