This is the third temporary time period visit for this facility. The facility’s Temporary License was issued October 6, 2025, and is valid through April 6, 2026. Restrictions on the permit include a first shift capacity of 38 to include all children, ages 0-12 years of age, and adults. Children under 2 ½ years old must be in a room with a direct exit. Upon arrival, I was greeted by Terrence Tippit, Lead Teacher. Ms. Tamara Tippit, administrator was also present during today's visit. Tameka Johnson, Child Care Consultant, accompanied me during today’s visit. We were accompanied by Ms. Tippit as we completed a general walk-through of the indoor and outdoor environment. A total of nineteen (19) children were present. Children were observed during free play, outdoor play, teacher directed activities, completing routine tasks and having lunch. The facility no longer provides lunch and enrolled children must bring their lunch from home. Nutrition Opt-out forms were observed during today's visit. There has been one (1) new staff person hired since the most recent monitoring visit. In addition to the staff file, we monitored a sample of children’s records and all applicable program records. The last documented fire drill was conducted on February 4, 2026, the last documented shelter-in-place drill was conducted on February 4, 2026, and the last documented outdoor inspection was completed on March 3, 2026. The fire inspection was conducted on November 13, 2025 and the last sanitation inspection was completed on January 20, 2026. The facility received four (4) demerits for a Superior classification. According to the North Carolina Secretary of State website, your corporation Tippitoes Early Education Developmental Center, L.L.C. is current and active. Transportation requirements were also monitored during today’s visit. The facility uses a 2020 GMC Sierra Pickup Truck (ZP5296 exp: 12/2026) and a 2021 GMC Yukon Denali(LKT-2123 exp: 8/2026) to transport. Current and valid registration and insurance were observed and in compliance. The vehicles are insured by National General and expires October 14, 2026. The following violations were observed and documented: Violation Number Comment Rule 1903 Documentation was not on file for medication given in error and/or the documentation did not include the required information. The specific dosage and expiration date was not completed on the authorization form for the administration of diaper creams. .0803(14)(i-vii) 9995 A violation was found for which there is no item number. Employee purses and other personal effects shall be kept out of reach of children.15A NCAC 18A .2800(f) A purse was accessible to children in the classroom designated for preschool children. 9996 A violation was found for which there is no item number. In child care centers, food shall be stored in approved, clean, tightly covered, storage containers once the original package is opened. A bag of rice that was previously opened was not stored in a tightly covered container. All violations must be corrected immediately. Please send me a compliance letter (written statement) describing how and when the above violations were corrected. The compliance letter must be received no later than March 19, 2026. The compliance letter should be sent to me via email, from the email address on file with the Division as the center’s official email address. Contact me if you have questions regarding correction of the violations documented during today’s visit. If you are unable to correct all violations within the specified timeframe, send a compliance letter regarding violations that were corrected and a specific plan to correct the remaining violations. An extension will need to be requested from management. RATED LICENSE: During today's visit, we reviewed QRIS modernization to determine which pathway the center will utilize for your rated license. An application for a rated license assessment was reviewed and signed during today’s visit. Following our discussion, you stated that you wanted to use the Classroom and Instructional Quality Pathway and this worksheet was reviewed with the administrator today. The staff education worksheet, Family and Community Engagement Standards and Facility Continuous Quality Improvement (CQI) plan were also reviewed and completed today. Individual CQI’s are due within one year and must be maintained in each staff file. The family and community engagement practices selected were reviewed, and sample practices were provided. I reminded you to keep documentation of meeting these practices on file for review during your annual compliance visit. You will be submitting the CQI plan for the facility and for each staff member as they were not available today. You stated today that you are using the Experience Early Learning curriculum and I observed these materials today. We also reviewed the coaching and training options for the Administrator and the Lead Teachers. You stated they have not yet completed the requirements, however a plan to complete them is in place. I reminded you to keep documentation of the mentoring or training on file for review during the annual compliance visit. Based on the information provided, this facility will earn a Three Star License. With this star rating, you must meet either enhanced staff/child ratios or enhanced space. You stated that you will meet the enhanced staff/child ratio requirement. The staff/child ratio postings each classroom should be updated to reflect this change. This license will be issued at the end of the temporary time period. Should any of the above projected point levels change upon full evaluation and review of your application, you will be notified. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Prior to today's visit, the eighteen month compliance history was 74%. Leigh McKenzie-Lee, Child Care Consultant, accompanied me during today’s visit. Limited child care requirements were monitored during today’s visit. The license was posted. The children were observed engaged in free play and playing outdoors. The infants and one year old children in care were observed playing on the floor and being held by the caregiver. Upon our arrival, we were greeted by the owner/operator, T. Tippit. The visit was concluded before the exit interview was completed with the administrator. A handwritten copy of the visit summary with the violations was reviewed and technical assistance provided as the monitoring was being conducted. The following violations were documented: Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Fifteen (15) children, from one to four years of age, received care from two staff members in Space #1. The maximum group size for this group based on the youngest child was twelve (12) children. In addition, children between the ages of 12-24 months of age were grouped with children 3 years of age or older. Two (2) children, ages seventeen (17) months and twenty-one (21) months, were grouped with children three (3) years of age and older. This is a violation of Child Care Rule .0713(a)(1)(6) GS 110-91(7);.0713(a-d) The violation(s) documented must be corrected immediately. You corrected the violation by moving two children one year of age to Space #3. We also discussed that when multiple ages are grouped together, the ratios and group size for the youngest child must be maintained. Name of Operation: Tippitoes Early Education Developmental Center 2 Facility ID: 51000994 Consultant: BECKY JACKSON Operation Type: Center Case Number: 0126-108L Visit Date: 1/15/2026 Number Present: 17 Completed Date: 1/15/2026 Age: From 0 To 4 Total Minutes: 90 Time In: 09:35 AM Time Out: 11:05 AM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to investigate allegations of non-compliance with child care requirements received by DCDEE, and to follow-up on violations documented during the complaint visit conducted on January 6, 2026. Leigh McKenzie-Lee, Child Care Consultant, assisted me today. The allegations were as follows: There is a concern that children are not properly supervised. There is a concern that there was an unreported incident. There is a concern that safe sleep requirements are not being followed. There is a concern that staff/child ratio requirements are not being followed. There is a concern that the sick child exclusion requirements are not being followed. Upon arrival at the facility I was greeted by the administrator. She was notified of the purpose of the visit. The children were observed engaged in free play and playing outdoors. The infants and one year old children in care were observed in bouncy seats, sitting in the bucket-seat type table, and being fed. The allegations were shared and you were given an opportunity to provide information. The following was determined based on observations made and information provided: Allegation regarding supervision: All groups were properly supervised during today’s visit. You described an incident reported by a parent where it was alleged that a child stepped on another child’s head or face. You stated staff did not observe an incident where this occurred. You stated the children were playing, jumping, and being rambunctious, however appropriate supervision was provided. We discussed the facility has cameras, however they only monitor the hallway and outdoor areas. Based on information provided and observations today, this report is unconfirmed. Allegation regarding an unreported incident: During today’s visit you stated you were aware that a parent had reported their child had been stepped on by another child while in care. You stated you conduct health checks when children arrive for care. You stated the child in question did not have any marks or injuries upon arrival. During the day the children were playing, jumping, and being rambunctious in the classroom, however no injury occurred. You stated the child did have a red mark, however it was not a bruise or a bump, just redness. You stated the child has fair skin, therefore a red mark was not concerning. As no incident occurred, you did not complete an incident report. You stated the parent asked for one, however you did not feel that an incident had occurred. We discussed that best practice is to complete an incident report anytime an anomaly is observed, for documentation and liability purposes. We discussed that in terms of incident reporting, more is better. The child did not return for care, as you stated you terminated care for the child for other reasons. Based on the information provided, this report is unconfirmed. Allegation regarding safe sleep requirements: Upon arrival to the space where infants and children one year of age are in care, an infant was observed sleeping in a bouncer seat. Information was obtained from the teacher, and conversations occurred about the children present. When asked, the teacher in the classroom was unaware that children could not sleep in bouncer seats. It was noted that the staff member had only been employed one day, and orientation was still in process. However, the staff member had completed ITS-SIDS training in September 2025. When I returned to the classroom for monitoring, the child remained in the bouncer, however the child was awake. We discussed that when children fall asleep in bouncy seats, car seats, and swings, they must be moved to the crib immediately. We reviewed the written safe sleep documentation in the classroom. We observed the last recorded check for this child was completed on January 9, 2026. You stated that sleep checks were completed in the BrightWheel software program, however it was not observed during the visit. Based on observations made, this report is confirmed, and a violation regarding safe sleep requirements was documented. Allegation regarding staff/child ratio requirements: Upon arrival both groups of children were meeting applicable staff/child ratio requirements. However, when reviewing information with the Administrator in the office, one staff member was observed on the camera monitoring screens in the office leaving space #1 and entering the hallway. When we stepped into the hallway to make visual observations of what was occurring, we observed that one staff member was in the hallway opening the door to Space #4. No children accompanied this staff member. You stated the staff member was assisting the children in exiting the building for outdoor play. Children were subsequently areobserved exiting the classroom and joining the staff member in the hallway. However, when the staff member was in the hallway near the file cabinets and opening the door to Space #4, the twelve children present, aged two through four years of age, were left in the classroom with one staff member. As the cameras do not record the classrooms, we were unable to observe if classrooms were meeting staff/child requirements on other days and times. After this observation, you unplugged the monitor showing the camera feeds in the office and requested that one of the staff members present remove the cameras from the facility. Based on observations made, this report is confirmed, and a violation regarding staff/child ratio requirements was documented. Allegation regarding sick child exclusion requirements: You stated you were aware of an allegation made by a parent that she was not contacted when her child became sick while in care. You stated that both flu and RSV are going around, and that children have been absent. You stated the child in question did not experience any symptoms while in care. You stated the child did not appear sick, was not warm to the touch, and was not demonstrating malaise. You stated the parent took the child’s temperature at home rather than while at the center. You stated that you are aware of the child care rules regarding exclusion of sick children, and that you have policies listed in your parent handbook. Appropriate responses were provided regarding when children should be excluded from care. Based on information provided, this report is unconfirmed. The violation documented during the January 6, 2026 complaint visit was also monitored today. We observed that twelve children from two to four years of age received care from one staff member when the second staff member in the room stepped into the hallway and left the classroom. Therefore, a repeated violation was documented today. The following violations were documented today: Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Twelve children, from two to four years of age, received care from one staff member in Space #1. GS 110-91(7);.0713(a-d) 871 Center staff did not comply with the safe sleep policy. An infant was observed sleeping in a bouncer seat. 10A NCAC 09 .0606(a) All violations must be corrected immediately. Please send me a compliance letter (written statement) describing how and when the above violations were corrected. The compliance letter must be received no later than January 29, 2026. The compliance letter may be sent to me via email. Contact me if you have questions regarding correction of the violations documented during today’s visit, pertaining to your compliance letter, or if you will not be able to meet the two week timeframe for submission of your compliance letter. If you are unable to correct all violations within the specified timeframe please submit a request for an extension as soon as you determine you will not meet the imposed timeframe. This request should include a specific plan to correct the remaining violations and any time frames needed. This request will need to be approved by DCDEE management. Additionally, DCDEE may also issue an administrative action following a complaint visit during which allegations are confirmed, or when serious violations are documented on concurrent visits. If it is determined that an administrative action is warranted, you will be notified by mail. Until a determination is made, visits will continue to monitor compliance. During today’s visit we discussed the compliance verification letter submitted for the temporary time period visit conducted on January 6, 2026. Many of the items were stated to be in process of compliance. I reminded you that the letter must reflect the items have been completed, noting how they were completed, and when. I asked that you resubmit the letter noting completion of all items. You stated you may need an extension for some items. I reviewed how to request an extension, noting the request should include the item number, why the extension is needed, and the projected completion date. The request will need to be submitted for approval. Technical Assistance During today’s visit we reviewed the requirements regarding opting out of the center’s meals and snacks. We discussed that if parents have not completed an opt-out form, and food is brought from home that does not meet the meal patterns for children requirements, it must be supplemented to meet these standards. If the parent has completed the opt-out form, the parent is responsible for supplying all meals, beverages, and snacks, and you will feed the child whatever is provided, regardless of its nutritional value or meeting meal patterns. I asked that you let me know if you have any questions about the opt-out requirements. We also discussed that menus must be dated. I observed that you are using a rotational menu, labeled Week 1, Week 2, etc. Various ways to show dates corresponding to the weeks were discussed, including use of a calendar to note which week’s menu is being served, or noting the dates on the menu itself, reflecting which each week’s menu is being served. All activity plans must be dated and current. We also discussed the items remaining for the change of ownership. We discussed that the building inspection and the sanitation inspection must be completed by April 6, 2026. I notified you that you must contact the Environmental Health Specialist to request the sanitation inspection. You stated you had done so. I asked that you follow-up with them to ensure the request was received. I also reminded you that in some jurisdictions in Johnston County the building inspection must be completed by an architect or engineer. I asked that you contact the local inspections department to determine what would be required to obtain an approved inspection. A follow up visit may be conducted in the near future to verify compliance with violations documented during the visit. Leigh McKenzie-Lee, Child Care Consultant, assisted me today. The allegations were as follows: There is a concern that children are not properly supervised. There is a concern that there was an unreported incident. There is a concern that safe sleep requirements are not being followed. There is a concern that staff/child ratio requirements are not being followed. There is a concern that the sick child exclusion requirements are not being followed. Upon arrival at the facility I was greeted by the administrator. She was notified of the purpose of the visit. The children were observed engaged in free play and playing outdoors. The infants and one year old children in care were observed in bouncy seats, sitting in the bucket-seat type table, and being fed. The allegations were shared and you were given an opportunity to provide information. The following was determined based on observations made and information provided: Allegation regarding supervision: All groups were properly supervised during today’s visit. You described an incident reported by a parent where it was alleged that a child stepped on another child’s head or face. You stated staff did not observe an incident where this occurred. You stated the children were playing, jumping, and being rambunctious, however appropriate supervision was provided. We discussed the facility has cameras, however they only monitor the hallway and outdoor areas. Based on information provided and observations today, this report is unconfirmed. Allegation regarding an unreported incident: During today’s visit you stated you were aware that a parent had reported their child had been stepped on by another child while in care. You stated you conduct health checks when children arrive for care. You stated the child in question did not have any marks or injuries upon arrival. During the day the children were playing, jumping, and being rambunctious in the classroom, however no injury occurred. You stated the child did have a red mark, however it was not a bruise or a bump, just redness. You stated the child has fair skin, therefore a red mark was not concerning. As no incident occurred, you did not complete an incident report. You stated the parent asked for one, however you did not feel that an incident had occurred. We discussed that best practice is to complete an incident report anytime an anomaly is observed, for documentation and liability purposes. We discussed that in terms of incident reporting, more is better. The child did not return for care, as you stated you terminated care for the child for other reasons. Based on the information provided, this report is unconfirmed. Allegation regarding safe sleep requirements: Upon arrival to the space where infants and children one year of age are in care, an infant was observed sleeping in a bouncer seat. Information was obtained from the teacher, and conversations occurred about the children present. When asked, the teacher in the classroom was unaware that children could not sleep in bouncer seats. It was noted that the staff member had only been employed one day, and orientation was still in process. However, the staff member had completed ITS-SIDS training in September 2025. When I returned to the classroom for monitoring, the child remained in the bouncer, however the child was awake. We discussed that when children fall asleep in bouncy seats, car seats, and swings, they must be moved to the crib immediately. We reviewed the written safe sleep documentation in the classroom. We observed the last recorded check for this child was completed on January 9, 2026. You stated that sleep checks were completed in the BrightWheel software program, however it was not observed during the visit. Based on observations made, this report is confirmed, and a violation regarding safe sleep requirements was documented. Allegation regarding staff/child ratio requirements: Upon arrival both groups of children were meeting applicable staff/child ratio requirements. However, when reviewing information with the Administrator in the office, one staff member was observed on the camera monitoring screens in the office leaving space #1 and entering the hallway. When we stepped into the hallway to make visual observations of what was occurring, we observed that one staff member was in the hallway opening the door to Space #4. No children accompanied this staff member. You stated the staff member was assisting the children in exiting the building for outdoor play. Children were subsequently areobserved exiting the classroom and joining the staff member in the hallway. However, when the staff member was in the hallway near the file cabinets and opening the door to Space #4, the twelve children present, aged two through four years of age, were left in the classroom with one staff member. As the cameras do not record the classrooms, we were unable to observe if classrooms were meeting staff/child requirements on other days and times. After this observation, you unplugged the monitor showing the camera feeds in the office and requested that one of the staff members present remove the cameras from the facility. Based on observations made, this report is confirmed, and a violation regarding staff/child ratio requirements was documented. Allegation regarding sick child exclusion requirements: You stated you were aware of an allegation made by a parent that she was not contacted when her child became sick while in care. You stated that both flu and RSV are going around, and that children have been absent. You stated the child in question did not experience any symptoms while in care. You stated the child did not appear sick, was not warm to the touch, and was not demonstrating malaise. You stated the parent took the child’s temperature at home rather than while at the center. You stated that you are aware of the child care rules regarding exclusion of sick children, and that you have policies listed in your parent handbook. Appropriate responses were provided regarding when children should be excluded from care. Based on information provided, this report is unconfirmed. The violation documented during the January 6, 2026 complaint visit was also monitored today. We observed that twelve children from two to four years of age received care from one staff member when the second staff member in the room stepped into the hallway and left the classroom. Therefore, a repeated violation was documented today. The following violations were documented today: Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Twelve children, from two to four years of age, received care from one staff member in Space #1. GS 110-91(7);.0713(a-d) 871 Center staff did not comply with the safe sleep policy. An infant was observed sleeping in a bouncer seat. 10A NCAC 09 .0606(a) All violations must be corrected immediately. Please send me a compliance letter (written statement) describing how and when the above violations were corrected. The compliance letter must be received no later than January 29, 2026. The compliance letter may be sent to me via email. If you are unable to correct all violations within the specified timeframe please submit a request for an extension as soon as you determine you will not meet the imposed timeframe. This request should include a specific plan to correct the remaining violations and any time frames needed. This request will need to be approved by DCDEE management. Additionally, DCDEE may also issue an administrative action following a complaint visit during which allegations are confirmed, or when serious violations are documented on concurrent visits. If it is determined that an administrative action is warranted, you will be notified by mail. Until a determination is made, visits will continue to monitor compliance. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. During today’s visit we discussed the compliance verification letter submitted for the temporary time period visit conducted on January 6, 2026. Many of the items were stated to be in process of compliance. I reminded you that the letter must reflect the items have been completed, noting how they were completed, and when. I asked that you resubmit the letter noting completion of all items. You stated you may need an extension for some items. I reviewed how to request an extension, noting the request should include the item number, why the extension is needed, and the projected completion date. The request will need to be submitted for approval. Technical Assistance During today’s visit we reviewed the requirements regarding opting out of the center’s meals and snacks. We discussed that if parents have not completed an opt-out form, and food is brought from home that does not meet the meal patterns for children requirements, it must be supplemented to meet these standards. If the parent has completed the opt-out form, the parent is responsible for supplying all meals, beverages, and snacks, and you will feed the child whatever is provided, regardless of its nutritional value or meeting meal patterns. I asked that you let me know if you have any questions about the opt-out requirements. We also discussed that menus must be dated. I observed that you are using a rotational menu, labeled Week 1, Week 2, etc. Various ways to show dates corresponding to the weeks were discussed, including use of a calendar to note which week’s menu is being served, or noting the dates on the menu itself, reflecting which each week’s menu is being served. All activity plans must be dated and current. We also discussed the items remaining for the change of ownership. We discussed that the building inspection and the sanitation inspection must be completed by April 6, 2026. I notified you that you must contact the Environmental Health Specialist to request the sanitation inspection. You stated you had done so. I asked that you follow-up with them to ensure the request was received. I also reminded you that in some jurisdictions in Johnston County the building inspection must be completed by an architect or engineer. I asked that you contact the local inspections department to determine what would be required to obtain an approved inspection. A follow up visit may be conducted in the near future to verify compliance with violations documented during the visit. In addition, this would allow more space to provide developmentally appropriate activities for the older one-year olds, while providing protected space for non-mobile children. We discussed that only rooms where direct exits are present can be used for children under two and one-half years of age. We discussed that Space #3 could be used for infant care, Space #1 for children one and two years of age, and Space # 2 could be used for children three and four years of age. Space #4 could also be used for children over two- and one-half years of age. Other options, such as creating a direct exit in Space #2 or #4 were also discussed. A follow-up visit may be conducted in the near future to verify compliance with violations documented during the visit. The facility’s Temporary License was issued October 6, 2025 and is valid through April 6, 2026. Restrictions on the permit include a first shift capacity of 38 children, ages 0 to 12 years old, with the total capacity including all children and adults. The permit also lists that all children under 2 ½ years old are cared for in rooms with direct exits. Restrictions were in compliance today. T. Tippit, Administrator was present when I arrived. The owner representative, who was being used as a staff member during today’s visit, was in the parking lot when I arrived. We walked together through the gate and into the facility. Ms. Tippit was notified of my arrival and the purpose of today’s visit. Two classrooms are operating with 24 children enrolled and 18 children present. During today’s visit, infants and children one year of age were observed playing with materials in the bucket seat table, on the floor, being held by the caregivers, or engaged in routine care. The group of children two to four years of age were observed cleaning up from free play and transitioning into group time. Spaces #2 and #4 are not in use at this time. According to the North Carolina Secretary of State website, your corporation, Tippitoes Early Education Developmental Center, L.L.C., is current and active. Your last fire drill was completed November 4, 2025 at 8:00am. The log reflected it took 10 minutes to evacuate 10 children and 5 adults. The most recent shelter-in-place drill was conducted on November 4, 2025 at 9:00am, and the last outdoor inspection was completed December 1, 2025. As this is a change of ownership a new building, fire and sanitation inspections must be completed by April 6, 2026. As the legal entity owns other licensed child care programs in North Carolina, you are not required to complete the Prelicensing Workshop. You provided me with the approved fire inspection today. This inspection was conducted on November 13, 2025. You also provided me with zoning approval today. The sanitation inspection and building inspections have not been received. I reminded you that the Environmental Health Section requires a plan review, and you stated you had spoken with the Environmental Health Specialist. I also reminded you that in some jurisdictions in Johnston County the building inspection must be completed by an architect or engineer. I asked that you contact the local inspections department to determine what would be required to obtain an approved inspection. Failure to obtain these inspections prior to April 6, 2026 may result in administrative action against your license. I monitored your classroom environments, outdoor playgrounds, and some program records during today’s visit. The visit was concluded before the exit interview was completed with the administrator. However, the violations were reviewed and technical assistance provided as the walkthrough was being conducted. I used the North Carolina Child Care Requirements and Compliance Listing as basic monitoring tools during the visit. Staff and children’s records were not monitored today. The following violations of child care requirements were observed today: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan in Space #1 was not dated. GS 110-91(12); .0508(a) 501 Meals/snacks did not comply with the Meal Patterns for Children in Child Care Programs. The menu for today listed peanut butter and jelly sandwich, apple slices, and milk to be served for lunch. 10A NCAC 09 .0901(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu was not posted in the food preparation area where easily seen by the cook. 10A NCAC 09 .0901(b) 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence) These plans were not posted. 10A NCAC 09 .0902(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. An aerosol can of Lysol was observed on top of the paper towel dispenser in the bathroom adjacent to Space #1. The door to the laundry room was observed to be unlocked. This room contained a gallon bottle of bleach, fabric softener, and several cans of paint. The hot water heater is also located in this room. .2820(b) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. The facility was not accurately documenting checking on sleeping infants. An electronic software program was being used, however the check observed for one child did not accurately reflect the time the nap began, the checks conducted during the nap, and when the nap ended. .0606(g) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. Cups of Dunkin Coffee and a box of doughnut holes were observed on a low shelf in Space #3. .0901(i) All violations must be corrected immediately. A Notice of Compliance is issued to a program that is owned and operated by a church, synagogue, or school of religious charter. Tippitoes Early Education Developmental Center, L.L.C., is not listed with the Secretary of State as either of these. You asked how to become a church. I referred you to the Secretary of State for information about various types of organizational structures. Technical Assistance and Consultation: During today’s visit you stated your own school-age children attend the center. I reminded you that as a Limited Liability Company owns the facility, your own children would count as enrolled children, and must meet all applicable child care requirements, including those for supervision and records, as all other enrolled children. We discussed that many items are required to be posted where parents can view them. You stated today that parents use two entrances. I encouraged you to post the required information at both entrances. I also notified you that these required items, including the child care license, must be posted uncovered, so all parts of the document are visible to parents and visitors. We discussed that you are using an electronic software system for some of your recordkeeping. Two of the items being kept electronically are children’s attendance and time of arrival and departure documentation. We discussed the implications of having only electronic records in the event of an emergency or evacuation and internet connections are not available or are not stable. We also discussed that all staff may not have access to the software at all times, or that staff needed to access the information may not be available to do so. Child care rule .2318(2) stated that records may be maintained in a paper format or an electronic format, provided that all required signatures are preserved in a paper format, PDF, or other used graphic format. Please send me a compliance letter (written statement) describing how and when the above violations were corrected. The compliance letter must be received no later than December 31, 2025. The compliance letter should be sent to me via email, from the email address on file with the Division as the center’s official email address. Contact me if you have questions regarding correction of the violations documented during today’s visit. If you are unable to correct all violations within the specified timeframe, send a compliance letter regarding violations that were corrected and a specific plan to correct the remaining violations. An extension will need to be requested from management. During today’s visit you asked about operating under a Notice of Compliance rather than a Child Care License. A Notice of Compliance is issued to a program that is owned and operated by a church, synagogue, or school of religious charter. Tippitoes Early Education Developmental Center, L.L.C., is not listed with the Secretary of State as either of these. You asked how to become a church. I referred you to the Secretary of State for information about various types of organizational structures. Technical Assistance and Consultation: During today’s visit you stated your own school-age children attend the center. I reminded you that as a Limited Liability Company owns the facility, your own children would count as enrolled children, and must meet all applicable child care requirements, including those for supervision and records, as all other enrolled children. We discussed that many items are required to be posted where parents can view them. You stated today that parents use two entrances. I encouraged you to post the required information at both entrances. I also notified you that these required items, including the child care license, must be posted uncovered, so all parts of the document are visible to parents and visitors. We discussed that you are using an electronic software system for some of your recordkeeping. Two of the items being kept electronically are children’s attendance and time of arrival and departure documentation. We discussed the implications of having only electronic records in the event of an emergency or evacuation and internet connections are not available or are not stable. We also discussed that all staff may not have access to the software at all times, or that staff needed to access the information may not be available to do so. Child care rule .2318(2) stated that records may be maintained in a paper format or an electronic format, provided that all required signatures are preserved in a paper format, PDF, or other used graphic format. Child Care Rule .0901(j) requires that staff model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns. We reviewed that coffee, snack cakes, and doughnuts were observed on a shelf in Classroom #3. These items do not meet nutritional standards and cannot be consumed while in the presence of children. I encouraged you to read this rule and to review it with your staff. I also encouraged you to put this requirement in your staff policies. We discussed the use of toddler bucket seat tables. In order to continue to be used, they should be fitted with footrests so the children’s legs and feet are supported. We discussed that the table may not be used as a confinement device, as was observed today. One child was observed sitting in the bucket seat with no materials or food. A second child was observed being placed in the seat with a book. While we were discussing the practice, and I was sharing the observations, the child was given a toy. Children should only use the table when eating, when participating in an activity, or while engaged in free play. I reviewed with you that child care rule .0713(a)(10) requires that the staff/child ratio applicable to a classroom be posted in that classroom in an area that parents are able to view at all times. I observed today that you have the staff/child ratio information posted in the hallways outside the classrooms. I asked that you move these inside the classroom. They must also be accurately completed, with the age of the youngest child to be cared for in the space, the applicable staff/child ratio, and the maximum number allowed either by space capacity or by group size. The box containing the applicable set of ratios chosen should also be checked. We also discussed that toys that have been mouthed by children should be taken out of circulation. Staff in Space #3 stated they used a dirty toy bucket, however it was currently in another area of the center when being disucssed. It was returned to the classroom during the visit. All over the counter medications, including diaper rash creams, should be labeled with the child’s name. Care should be taken to ensure that staff purses and bags are stored appropriately. They must be stored inaccessible to the children. If they contain medications or other hazardous products, they must be stored in locked storage. A follow up visit may be conducted in the near future to verify compliance with violations documented during the visit.
