This facility has a one-star license and the compliance history percentage prior to this visit was 89%. Upon arrival, I was greeted by Ms. Whitney Moore, Assistant Director. I shared the reason for the visit and Ms. Moore assisted me with today’s visit. The following were monitored during this visit: supervision, staff/child ratio, safety, adequate/approved space, program records, new staff records, license posted, permit restrictions, discipline and storage of hazardous products and medications. I observed children engaged in story time, art, special group activity and the individual needs of the infants were being met by the teacher. Medication is currently administered at this facility. The storage of the diaper creams was monitored. The last sanitation inspection was on 3/25/25 with zero (0) demerits and a superior classification. The last fire inspection was conducted on 10/14/25. There were two (2) new staff members hired since the facility’s last AC visit on 5/6/25. I monitored the personnel and medical file for the new staff members. A staff and training worksheet was provided by administration and a copy was included in today’s visit summary packet. The violations cited during the visit were discussed with Ms. Moore and documented in the visit summary. The following violations were observed during the visit: Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. In space #4, a staff/child ratio sheet was not posted for the school age group. .0713(a)(10), (c) & (f)(3); .2818(e) 415 A current schedule was not posted for each group of children for reference. In space #4, a schedule was not posted for the school age children. GS 110-91(12);.0508(a) 428 A current activity plan was not posted for each group of children for reference. In space #4, an activity plan was not posted for the school age group. GS 110-91(12); .0508(a) 536 Formula and other beverages which require refrigeration were not identified for each child or properly refrigerated. In space #1, there was a child's formula bottle in the refrigerator without a date on the bottle. 15A NCAC 18A .2804(d) Because all cited violations were corrected during the visit, a compliance letter is not required to be submitted to the DCDEE. Technical Assistance/Resources: • During the visit, I asked Ms. Moore if she would like to discuss information regarding the QRIS system. This facility has a one-star license and the compliance history percentage prior to this visit was 95%. Upon arrival, I was greeted by Ms. Jessica Griffin, Administrator. I shared the reason for the visit. Ms. Griffin assisted me with today’s visit. The following were monitored during this visit: supervision, staff/child ratio, safety, adequate/approved space, program records, new staff records, license posted, permit restrictions, discipline and storage of hazardous products and medications. There were a total of twenty-four (24) children present during the visit. I observed children engaged in choice activities, table activity with the teacher and the individual needs of the infants and young toddlers were being met by the teacher. Medication is currently administered at this facility. I monitored the storage of the diaper creams which was in compliance with NC Child Care Rules. The last sanitation inspection was on 5/21/24 with zero (0) demerits and an approved classification. The last fire inspection was conducted on 10/2/24. There were two (2) new staff members hired since the facility’s last Annual Compliance visit on 5/8/24. The staff files and medical files were monitored for compliance with NC Child Care rules. I completed a staff and training worksheet during today’s visit for the two (2) files. The violations cited during the visit were discussed with Ms. Griffin and documented in the visit summary. The following violations were observed during the visit: Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. In space #3, there was a child three years of age in the same group with children one and two years of age. 10A NCAC 09 .0713(a)(6) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Staff member, J.B. did not complete the required hours of orientation during the first two weeks of employment. .1101(a)(b) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. Staff member J. B. did not complete the Shaken Baby policy review prior to working with children. .0608(d)(1-4) All violations documented above must be corrected immediately. A letter needs to be sent to me stating how each violation was corrected and how compliance will be maintained in the future. Please include the violation number, statement of compliance, license ID and your signature in the letter. Your compliance letter must be received, via email before or by November 21, 2024. If needed, send your letter to: Stephanie Palmer PO Box 481335 Charlotte, NC 28269 Technical Assistance/Resources: • During the visit, I reminded Ms. Griffin about the hold harmless status for licensed facilities until the new QRIS is implemented. • During the visit, I discussed with Ms. Griffin the testing requirements and assistance provided through Clean Classrooms for Carolina Kids. • During the visit, I reminded Ms. Griffin regarding the requirement for approved fire inspection reports to be submitted to the DCDEE within one week of completion. • During the visit, Ms. Griffin asked about the training requirements for a facility driver. I shared that if the driver provides direct care for the children, ie. Another staff member is not in the vehicle with the driver, he/she will need to complete the health and safety trainings and the on-going trainings. The on-going training hours can be prorated if the driver works less than 40 hours per week. Please see rule .1103 (d) on page 40 of the NC Child Care Rules. Also, please refer to pages 38-40 for rules regarding the health and safety trainings as well as the on-going training hours. • During the visit, I discussed with Ms. Griffin rule .0713 (a) (6) on pages 25-26 of the NC Child Care Rules regarding children ages 12 months-24 months not being in the same group as children ages 3 and older.
