This family childcare home began operating on 10/18/24. The compliance history percentage for this family childcare home is 98% for the previous 18-month period. This family childcare home currently holds a four-star rated license. Upon arrival, I was greeted by Ms. Lateka Smith, owner/operator. I shared the reason for the visit. Ms. Smith assisted me with today’s visit. I used the “Annual Compliance Monitoring Checklist for Family Child Care Homes” as a monitoring tool during today’s visit. I monitored supervision, capacity, group size, indoor spaces, safety, program records and permit requirements. Observations: Two (2) children were present during the visit. The children were engaged in choice activities, snack time and outdoor play time. Positive interactions were observed between Ms. Smith and the children. Transportation is provided. The interior and exterior of the vehicle was monitored. The registration is valid through 12/31/25. The insurance is valid from 9/18/25 through 3/18/26. Medication is not being administered. Four (4) children are enrolled in the family childcare home. There was a file for each child enrolled and the files were reviewed. The operator’s file was monitored as well. The family childcare home’s EPR plan was not available during today’s visit. The following documents were given to you prior to today’s visit: -VERIFICATION OF REQUIRED INFORMATION FOR OPERATOR AND ADDITIONAL CAREGIVERS. -The OPERATOR’S STATEMENTS OF RESPONSIBILITY. The required documents were received by the child care consultant on 8/19/25 and 8/25/25 via email. A copy of the computer-generated visit summary was given to you along with a copy of the enrollment/staff/child worksheet and the “Annual Compliance Monitoring Checklist for Family Child Care Homes”. The violations observed during the visit were discussed with Ms. Smith at the end of the visit and documented in the visit summary. The following violations were observed: Violation Number Comment Rule 709 Equipment and toys were not in good repair and developmentally appropriate. The backdoor used to access the outdoor play area had a window with broken glass. 10 A NCAC 09.1720(a)(7) 908 Health questionnaire was not completed annually. Health questionnaire was not available during the visit. .1703(a)(1) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child's health assessment was missing from the file. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. Emergency information with the children's photograph were not available. .1723(13) 1856 The operator did not complete the Emergency Preparedness and Response in Child Care training within the required timeframe and/or documentation of completion of the training was not on file. The operator did not complete the EPR training as required. .1714(b) 1857 The operator did not develop an Emergency Preparedness and Response Plan within four months of completing the Emergency Preparedness and Response in Child Care training and/or the Plan was not completed on the template provided by the Division. The operator did not develop the EPR plan as required. .1714(c) All violations documented above must be corrected immediately. A letter of compliance needs to be sent to me stating how each violation was corrected and how compliance will be maintained in the future. Please include the violation number, statement of compliance, license ID and your signature in the letter. Your compliance letter must be received, via email or mail by October 30, 2025. I encouraged Ms. Smith to review the new QRIS system. • During the visit, I emailed EPR Plan information to Ms. Smith. • During the visit, I provided an updated copy of the Summary of the Law poster. • During the visit, I provided a transportation permission form. • During the visit, I provided a health questionnaire form for Ms. Smith to complete. • During the visit, Ms. Smith asked for clarification on screen time. I shared that children under 3 years of age may not have screen time. If screen time is provided for children ages 3 and up, a documentation of the screen time must be kept and limited to 30 minutes per day and not to exceed 2 ½ hours per week. Name of Operation: Tat's Corner Daycare Facility ID: 84000302 Consultant: RASHEKA WHEELER Operation Type: Family CC Home Case Number: Visit Date: 12/3/2024 Number Present: 1 Completed Date: 12/3/2024 Age: From 2 To 2 Total Minutes: 90 Time In: 10:00 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor for compliance with applicable child care requirements for a first temporary time period visit. You, Lateka Smith, Operator, assisted me with the visit today. You have one (1) enrolled child that live in the home. I used the Family Child Care Home Item Number Listing (DCD 0375) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. During the visit, I monitored the indoor and outdoor environment, staff and children's records, outdoor inspection checklist, fire drill reports and required posted items. When I arrived, I greeted the provider and the child at the front door. I stated the purpose of today’s visit and began my monitoring visit. The child was engaged in free choice play throughout the entire designed space. The fire drill and playground inspections were conducted but was not documented on the required form. Please make sure you are documenting these expectations monthly. I recommend posting near the front door to help with the monthly reminder. You last completed CPR on 2/22/2024, and will expire on 2/22/26 and First Aid, 2/22/24 and will expire on 2/22/2026. The ITS SIDS certification was received on 2/8/23 and will expire on 2/8/26. Indoor Learning Environment: The child care room contained dramatic play materials, blocks materials, manipulative materials, books, musical instruments, art supplies, and games. The area was set up and accessible for the child to engage in varies play opportunities. Breakfast today consisted of grits, eggs and whole milk. The provider stated she is in the process of completing all necessary paperwork to be a part of the Food Program. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. Transportation: The provider desires participate in transportation. During today’s visit, I monitored her vehicle during today’s visit and approved it for use. A vehicle is on the premises for emergency use only. Records: One (1) child’s file for the live in child was monitored during today’s visit. We spoke about having packets ready for perspective families if interest in enrolling into her license. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. License Status: Currently, you hold a temporary child care license. Restrictions on your license include daytime care for first and second shift. There is a maximum of 5 preschool children at any time. Your temporary license will expire on April 18, 2025. Rated License: We discussed the two-component rated license process. You stated that you are interested in getting the FCCERS assessment completed and have already contact Stanly County Partnership for Children. You have been visited by Gaye Honeycutt, EC Specialist. I stated, I will email you the application and the 3-month study on January 3, 2025, and will require the information to be completed and sent to me by January 31, 2025. You will need to create a WORKS account to send in your most recent education received. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidy. Health and Safety Trainings: The Child Care Development Block Grant (CCDBG), also called the Child Care Development Fund (CCDF), is a federal grant which provides funding for child care subsidies for low income working families and improvements in child care quality. The recent reauthorization of CCDBG, Child Care and Development Block Grant Act of 2014, brought for the first time in 18 years new state requirements. These requirements promote the health and safety of children in child care, create continuity of access to subsidies for low-income families, better educate parents and the general public about the child care choices available to them, and improve the overall quality of early care and education programs. States must have pre-service or orientation and on-going minimum health and safety training requirements in place for child care providers. The trainings must cover the following content areas: * Administration of medication * Appropriate precautions in transporting children * Building and physical premises safety * Pediatric First Aid and CPR appropriate for the ages of children in care * Prevention and control of infectious diseases, including immunizations * Prevention of and response to emergencies due to food and allergic reactions * Prevention of shaken baby syndrome and abusive head trauma * Prevention of SIDS and use of safe sleeping practices * Recognizing and Responding to child abuse and neglect * Handling and storage of hazardous materials and the appropriate disposal of bio-contaminants * Emergency preparedness and response planning for emergencies To help child care providers meet the new training requirements, the Division of Child Development and Early Education has provided these training on Moodle on our website providing FREE online training modules. Click on the Course Catalog tab at the top of the page on the website to login to your training. If you have obtained an individual NCID, then you can use it with you log into Moodle. You can also receive training from your resource and referral agency in Montgomery. You will have a year from your license date of October 18, 2024 to April 18, 2025 to complete the Health and Safety Trainings. You may care for no more than five (5) preschool children at any time. Your capacity must never exceed five (5) preschool children and 3 school-aged children. 2. Children must be visually supervised while awake at ALL times and able to be heard while at rest. You may never leave your home to go outside or behind closed doors from the children. 3. We discussed making sure her kitchen is clean and orderly to maintain good sanitary habits while caring for children. During the visit, I observed dirty dishes in the sink, and unclean stove top. The following violations were cited today. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The monthly outdoor check was not completed for the month of November 2024. 10A NCAC .1721(e)(5)(A-F) 716 Electrical outlets not in use were not covered. An electrical outlet was observed uncovered near the front door, accessible to the child enrolled at the program. 10A NCAC .1719(a)(27) 1853 The operator did not conduct a monthly fire drill. The provider completed a fire drill but did not document it on the fire drill form. .1719(a)(15) & .1721( e)(2) It is important for licensed providers to be knowledgeable of all of the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It's the provider's responsibility to be in compliance with all of the applicable laws and rules.
