This facility has a five-star license and the compliance history percentage prior to this visit was 96%. Upon arrival, I was greeted by Ms. Carli Whitley, administrator. I shared the reason for the visit and Ms. Whitley assisted me with today’s visit. The following were monitored during this visit: supervision, staff/child ratio, safety, adequate/approved space, program records, new staff records, license posted, permit restrictions, discipline and storage of hazardous products and medications. I observed children engaged in circle time, outdoor play time on the playgrounds and choice activities. Medication is currently administered at this facility. The storage of the diaper creams was monitored. The site monitoring tool was monitored for the one (1) NC PreK classroom. The classroom was monitored by the director on 10/23/25. An on-site visit was made by the NC Pre-K Coordinator on 1/8/26. The last sanitation inspection was on 11/12/25 with zero (0) demerits and a superior classification. The last fire inspection was conducted on 2/10/26. There were two (2) new staff members hired since the facility’s last AC visit on 9/25/25. I monitored the personnel and medical files for the new staff members. A staff and training worksheet was completed by the administrator. The violation cited during the visit was discussed with Ms. Whitley and documented in the visit summary. The following violation was observed during the visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The rubber border on the preschool playground has ripped sections. G.S. 110-91(6); .0601(b) All violations documented above must be corrected immediately. A letter needs to be sent to me stating how each violation was corrected and how compliance will be maintained in the future. A QRIS conversation form was given to Ms. Whitley summarizing what I discussed with her regarding pathway options and requirements. • During today’s visit, I offered two (2) NC Foundations of Early Learning and Development books for Ms. Whitley to share with staff and use as a resource for planning age appropriate activities for the children enrolled. This facility’s last annual compliance visit was on October 1, 2024. The compliance history percentage for the 18-month period is 98%. The facility currently holds a five-star license. Upon arrival, I was greeted by Ms. Carli Whitley, administrator. I shared the reason for the visit. Ms. Whitley assisted me with today’s visit. The Secretary of State’s website was monitored and Love-n-Care of Albemarle, Inc. is current and active. I used the “Annual Compliance Monitoring Checklist for Child Care Centers” as a monitoring tool during today’s visit. I observed postings, indoor and outdoor spaces, safety and program management. Observations: I observed the children engaged in breakfast time, choice activities and outdoor play time. Two (2)s staff file and six (6) children’s files were monitored during the visit for compliance with NC Childcare Rules. There was one (1) new staff member hired since the facility’s last RU visit on 5/1/25. Three (3) of the six (6) children’s files monitored were for children enrolled in the NC Pre-K program. The child care consultant made notes on the staff and training worksheets and initialed where notes were made on the documents completed by the administrator. The Emergency Preparedness and Response Plan was reviewed and was last updated on the template by the administrator on September 5, 2025. Medication is currently administered at this facility. The storage of the diaper cream was monitored along with the parent authorization forms. Transportation is provided at this facility. The interior and exterior of the van was monitored during the visit. The registration is valid through 8/31/26. The insurance coverage provided by West Bend is valid from 5/11/25 to 5/11/26. The last sanitation inspection was conducted on 4/9/25 with zero (0) demerits and a superior classification. The last fire inspection was conducted on 2/11/25. A copy of the computer-generated visit summary and enrollment worksheet were provided during the visit along with a copy of the “AC Monitoring Checklist for Child Care Centers”. The violations observed during the visit were discussed with Ms. Whitley and documented in the visit summary. The following violations were observed: Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. In space #4, there was a broken plastic knob with sharp edges on the toy kitchen. .0601(c) 807 A safe indoor and outdoor environment was not provided for the children. A wasp nest was located behind a light near the equipment storage area in the outdoor learning environment. 10A NCAC 09 .0601(a) Because the violations were corrected during today’s visit, a compliance letter is not required to be submitted. Technical Assistance/Resources: • During the visit, I discussed the new QRIS system and Pathways to the stars with Ms. Whitley. I also shared that during the facility’s Routine Unannounced visit, we would discuss the pathway chosen by the facility owner and/or administrator and share required documents. I also shared that if Ms. Whitley is ready to move forward prior to the RU visit to contact me to begin the process. • During the visit, I discussed the filing of staff medical and personnel files with Ms. Whitley. Please see rule .0701 (d) on page 23 of the NC Childcare Rules which states (d) Staff medical statements, proof of a tuberculosis test or screening, and completed health questionnaires shall be included in the staff member's medical file, which must be maintained separately from the staff member's individual personnel file in the center.
