Currently, this center located in a residence operates with a Four-Star License, issued November 8, 2019, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality Point. The last sanitation inspection was completed on February 26, 2025 with a “Superior” classification. The annual sanitation inspection has not been completed. The last fire inspection was completed on April 28, 2025, and the facility was approved for daytime and night care. The last annual compliance visit was completed on April 2, 2025. L. Ward, Owner/Operator was present and available to assist. Six (6) children age ranging one (1) to three (3) years of age were present. Children were engaged in free choice play and small group, teacher directed activities, eating lunch, and naptime. Lunch consisted of lima beans, green beans, peaches, whole wheat toast, and milk. All staff interacted with children in a positive, nurturing manner. I used the Annual Compliance Monitoring Checklist and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent (10%) of staff records and ten percent (10%) of children’s records and all program records were reviewed today. All staff records and children’s records were found to be in compliance. One (1) violation was documented for program record keeping/ annual inspections. Any violations observed were discussed with you and documented on the computer-generated visit summary left with you at the conclusion of the visit. The following violations were documented today. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply for your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Name of Operation: WARDS PRECIOUS DAY CARE Facility ID: 46000031 Consultant: MARQUITTA HOLLOMAN Operation Type: Center Case Number: 0825-176L Visit Date: 8/26/2025 Number Present: 9 Completed Date: 8/26/2025 Age: From 1 To 4 Total Minutes: 140 Time In: 11:40 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of my unannounced visit was to investigate a complaint report regarding allegations of childcare requirements. The allegation report was reviewed with L. Ward, Owner. You were given the opportunity to discuss the allegations. Nine (9) children were present ages one (1) to four (4) years old. FINDINGS/OBSERVATIONS Required regulatory components for a complaint visit were monitored. A walkthrough of the facility was conducted, and attendance and staff/child ratios were observed. When asked if there were any recent concerns from parents, L. Ward stated there had been communication from a parent alleging her daughter had a cotton craft ball stuck up her nose from daycare, and sent a picture via text on 5/21/2025. The parent was told that there was no craft activity on that day and the child would not have gotten the cotton craft ball from the daycare. The provider stated those particular craft balls they did not use that day and they are not accessible to the children otherwise. According to P. Cherry, Lead Teacher she is never left alone with the children outside of the ratios. We discussed the ratios for the age group and that the ratios must be followed for the youngest child in the group. Ms. Cherry and Ms. Ward stated the children’s diapers are checked and/or changed at least every two (2) hours and as needed. We discussed children being put into the high chair and how often. You stated children are put into the high chair during feeding times, and sometimes to remove them from the space when they are having a meltdown, if you feel they may hurt the other children or themselves. We discussed that this practice was inappropriate and I suggested you have a designated area for “time out” to remove the children from the space and situation and allow them to calm down. Based on observations and information provided, one of the allegations regarding violations of child care requirements were confirmed. TECHNICAL ASSISTANCE/ CONSULTATION MONTHLY INSPECTIONS The facility did not have documentation of a completed playground inspection for the months of October and November 2024. The inspection was completed during the visit and documented. REMINDERS CPR and First Aid Training for D. Cherry will expire January 16, 2025. To ensure to remain in compliance, please contact the proper trainer to schedule the training before it expires. STAR RATED LICENSE Hold harmless has been extended until the new QRIS is implemented (SB 425). Providers in Cohort 1 are not required to go any further with a rated license assessment unless they want to, and providers in Cohort 2 do not need to start their preparation year unless they want to. This should be the providers’ choice only. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
