This facility operates with a Two (2) Star License issued January 24, 2019, earning two (2) points in Program Standards, two (2) points in Staff Education, and one (1) quality point. The last sanitation inspection was completed on October 12, 2023 earning a “Superior” classification. The last fire inspection was completed on August 28, 2024 and was approved for day time and night time care. The center's last annual compliance visit was on September 28, 2023. C. Paul, Director was present and available to assist. Thirty-seven (37) children age ranging three (3) to five (5) years of age were present. Children were observed in outdoor play, and teacher directed activities All staff interacted with children in a positive and nurturing manner. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records were monitored. One (1) violation for children’s records/ medication was documented and two (2) violations for staff records were documented. Any violations observed today were discussed with you and documented on the visit summary reviewed with you at the end of the visit. The following violations were documented on today’s visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Written permission/authorization to administer medication, from the parent, was not on file for review. No medication has been administered 10A NCAC 09 .0803(1)(a & b) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The EMC plan review was not documented for staff records monitored; M. Dilday and A. Winslow 10A NCAC 09 .0802(a) 1041 Prior to employment a Criminal Background Check was not completed. Criminal Background check was not on file for M. Dilday G.S. 110-90.2(b) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply for your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before September 12, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. A copy of the compliance history score was printed and left with you at the conclusion of the visit. TECHNICAL ASSISTANCE/ CONSULTATION STAFF RECORD KEEPING The two staff records monitored, for M. Dilday and A. Winslow, did not have documentation on file of the review of the Emergency Medical Care plan (EMC). There was no Criminal Background Check (CBC) on file for M. Dilday. Administrator was advised M. Dilday could not be at the facility without a current CBC. CHILDREN’S RECORD KEEPING/MEDICATION One child did not have a Permission to Administer Medication form on file. Any medication kept at the facility is required to have a Permission to Administer Medication form signed by the parent to accompany the medical action plan. This facility operates with a Two (2) Star License issued January 24, 2019, earning two (2) points in Program Standards, two (2) points in Staff Education, and one (1) quality point. The last sanitation inspection was completed on October 26, 2022 earning a “Superior” classification. The last fire inspection was completed on August 16, 2023 and was approved for day time and night time care. The center's last annual compliance visit was on October 25, 2022. C. Paul, Director was present and available to assist. Forty-eight (48) children age ranging three (3) to five (5) years of age were present. Children were observed in free choice play and teacher directed activities, and snack time. All staff interacted with children in a positive manner. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility. A computer-generated visit summary was reviewed and left with you at the conclusion of the visit. The following violations of childcare requirements were documented. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. Two (2) children's files were missing documentation of the Summary of NC Child Care Laws GS 110-102 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One (1) staff member did not have documentation of receiving a TB skin test before the first day of employment .0701(a) 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. Two (2) children's files were missing documentation of receipt of the facility's Operational policies 10A NCAC 09 .0514(b) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Two (2) children's files were missing documentation of the receipt of the smoking and tobacco restriction policy .0604(j) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply for your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before October 12, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is accurate documentation. If any information provided in the letter is not accurate, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. COMPLIANCE HISTORY Prior to today's visit, this facility has maintained an 18-month compliance score of 92%. The compliance history score was printed, and you were given a copy at the conclusion of the visit. TECHNICAL ASSISTANCE/ CONSULTATION STAFF RECORD KEEPING One (1) staff member did not have documentation of having a TB skin test, on file for review. You stated the staff member has had a test but the documentation was misplaced. All staff are required to have a TB skin test on file prior to the date of hire. CHILDREN’S RECORD KEEPING Two (2) children did not have documentation of receipt of the Summary of NC Child Care Law, receipt of the facilities operational policies, and receipt of the tobacco and smoking restriction policy, on file for review. All documents are required to be on file at the child’s enrollment date.
