Currently this program operates with a Three Star license issued on January 2, 2023 earning five (5) points in staff education, two (2) points in program standards, and one (1) quality point. The last sanitation inspection was completed on November 5, 2025, earning a “Superior” classification. The last fire inspection was completed on September 4, 2025 and approved for day time and night time care. The last annual compliance visit was conducted on April 1, 2025. D. Wilder, Owner/Director, was present and available to assist. Twenty-eight (28) children ranging in ages zero (0) to four (4) were present. Children were observed in teacher directed activities, free play activities, handwashing and toileting routines, diapering routines, and lunch. Lunch consisted of beef and noodles, corn, fruit cocktail, and milk. All interactions between the teachers and the children were warm and nurturing. I used the Annual Compliance Monitoring Checklist and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent (10%) of staff records and ten percent (10%) of children’s records and all program records were reviewed today. All children’s records and program records were found to be in compliance. One (1) violation was documented for staff record keeping. One (1) violation was documented for medication, and one (1) violation was documented for equipment not being in good repair. Any violations observed were discussed with you and documented on the computer-generated visit summary left with you at the conclusion of the visit. No following violations were documented today. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. The toilet used by the enrolled boys had a visible crack in it. .0601(c) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Medication was still stored at the center after the medication authorization expired, for two (2) children in the infant/toddler space. .0803(12) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. Annual staff evaluation was not completed for T. Wilder, and C. Cherry before the previous one expired. 10A NCAC 09 .0514(f) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply for your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before March 17, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. I used a checklist as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to your program. A computer generated visit summary was reviewed and left with you at the conclusion of the visit. The following violations of child care requirements were documented today.
