This facility currently operates with a Five Star license issued on January 10, 2018, earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point. The last sanitation inspection was completed on October 3, 2025, earning a “superior” classification. The last fire inspection was completed on September 4, 2025 and approved for day time care only. The last annual compliance visit was made March 11, 2025. R. Barnes, Lead Teacher/Pre-K Coordinator was present and available to assist. There were sixty-eight (68) children present. The children present were playing in free choice play, interacting with the assigned staff, and outdoor play. Daily routine transitions such as preparing for lunch, toileting and hand washing were also observed. All off site forms were reviewed and there was no update off site form for staff, nor children. This center is owned and operated by Hertford County Public Schools. I verified the owner and program contact information is correct as documented with DCDEE. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. A computer- generated visit summary reviewed and left with you at the end of the visit. The following violations were documented today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The annual fire inspection was completed on 9/5/24 but was not submitted to the consultant. 10A NCAC 09 .0304(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Six (6) containers of Clorox wipes were observed in an unlocked cabinet in Space 15. .2820(b) 1043 All staff records, except financial records, were not made available for review. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Classroom capacity for all NC Pre K classrooms is 18 students, on the most current floor plan for the facility. There were twenty (20) students enrolled and nineteen students present in Space 18 and there were nineteen (19) students enrolled, but only sixteen present today in Space 12. According to the rule, there can be twenty (20) students enrolled in the space, however there has to be a request made to the consultant and prior approval to accommodate that number of students. STAFF RECORD KEEPING Recognizing and Responding to Suspicions of Child Maltreatment training was not on file for A. Fain within 90 days of hire. Bunch did not have employment application, TB Skin test, health questionnaire, emergency information, child care criminal background qualification letter, orientation, documentation of review of EMC plan, documentation of review of EPR plan, CPR/First Aid training documentation, receipt of shaken baby and abusive head trauma policy, and recognizing and responding to child maltreatment training on file fore review. L. Williams, substitute in Space 18, has a criminal background check that expired 6/26/2024. Any staff who has contact with the children in the licensed spaces are required to have a NC Childcare Criminal Background check, as well as other required documentation. PROGRAM RECORD KEEPING The offsite records form was not current for transportation and children’s records. The last dated transportation for was 9/6/2024. And the children’s off site record form was not on file. STORAGE OF HAZARDOUS ITEMS In Space 15, six (6) containers of Clorox wipes were observed in an unlocked storage cabinet. This was corrected during the visit by locking the storage cabinet. ANNUAL INSPECTIONS The facility’s fire inspection was completed on 9/4/2025, however it was not submitted to the consultant within the required one (1) week of completion. A copy of the fire inspection was obtained during the visit. QRIS/STAR RATED LICENSE We discussed the different pathway options for the new QRIS system. You decided to complete Pathway 1-Classroom and Instructional Quality. We completed an in-depth review of the forms specific to Pathway 1 and you were given the opportunity to ask questions. This facility currently operates with a Five Star license issued on January 10, 2018, earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point. The last sanitation inspection was completed on October 7, 2024, earning a “superior” classification. The last fire inspection was completed on October 10,2024 and approved for day time care only. The last annual compliance visit was made April, 17,2024. R. Barnes, Lead Teacher/Pre-K Coordinator was present and available to assist. There were fifty-one (51) children present. The children present were playing in free choice play, interacting with the assigned staff, and outdoor play. Daily routine transitions such as preparing for lunch, toileting and hand washing were also observed. All off site forms were reviewed and found to be in compliance. This center is owned and operated by Hertford County Public Schools. I verified the owner and program contact information is correct as documented with DCDEE. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records, and all program records were monitored. All children and program records were found to be in compliance. Three (3) violations were documented for staff record keeping/ training. A computer generated visit summary was reviewed and left with you at the conclusion of the visit. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy five percent. These trainings are to be completed every five (5) years before the expiration of the last trainings. EMERGENCY RELOCATION As of today the emergency relocation for the four (4) NC Pre K classrooms is no longer needed. The children had relocated back to the original classrooms. Rooms, 12,15,17,18 are all furnished and equipped with everything needed to ensure the safety and education of the children. All required posted materials are posted and visible for both spaces, including current activity plans, daily schedules, and ratio charts. The fenced outdoor area was observed and found to be compliant. RESOURCES LEAD IN WATER/PAINT AND ASBESTOS Public schools and licensed child care facilities in North Carolina are eligible to participate in the funding mechanisms to cover the costs of testing for lead in water and inspecting for asbestos and lead-based paint hazards beyond the May 1, 2024 deadline listed in 10A NCAC 41C .1002(a). Due to the recent impacts of Hurricane Helene and other natural disasters, this deadline has been revised statewide, and facilities that have not yet enrolled should complete the online process by May 31, 2025. The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket costs. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement and will allow facilities to participate in the funding mechanisms. Documentation of EMC plan review, EPR plan review, Shaken Baby and Abusive Head Trauma Syndrome policy, Emergency Information, Health Questionnaire, Staff’s education, Recognizing and Responding to Suspicions of Child Maltreatment training, CPR and First Aid training, Criminal Background check, medical report, and TB skin test were not on file, for review, during the visit. You stated the staff had all this information; however, it could not be monitored during the visit. Emergency information, health questionnaire, shaken baby policy, EMC review and EPR review documentation were all updated during the visit. V. Ghee had not completed the Health and Safety trainings nor the Recognizing and Responding to Suspicions of Child Maltreatment training before it expired. These trainings are to be completed every five (5) years before the expiration of the last trainings. EMERGENCY RELOCATION As of today the emergency relocation for the four (4) NC Pre K classrooms is no longer needed. The children had relocated back to the original classrooms. Rooms, 12,15,17,18 are all furnished and equipped with everything needed to ensure the safety and education of the children. All required posted materials are posted and visible for both spaces, including current activity plans, daily schedules, and ratio charts. The fenced outdoor area was observed and found to be compliant. RESOURCES LEAD IN WATER/PAINT AND ASBESTOS Public schools and licensed child care facilities in North Carolina are eligible to participate in the funding mechanisms to cover the costs of testing for lead in water and inspecting for asbestos and lead-based paint hazards beyond the May 1, 2024 deadline listed in 10A NCAC 41C .1002(a). Due to the recent impacts of Hurricane Helene and other natural disasters, this deadline has been revised statewide, and facilities that have not yet enrolled should complete the online process by May 31, 2025. The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket costs. This facility currently operates with a Five Star license issued on January 10, 2018, earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point. This NC PreK program is operated under Hertford County Public Schools and housed at Bearfield Primary School. The last annual compliance visit was made April 17, 2024. The Principal, O. Stinson, was available and assisted with the visit today. The facility has a capacity to serve one hundred twenty-six (126) children and is currently serving seventy-two (72) NC PreK children. An email was received from the NC Pre K Coordinator for Hertford County Public Schools, S. Horton, on Friday, January 24, 2025, to explain that the children in the NC PreK program would not have access to hot water for 4 to 6 weeks. The facility’s heating unit began the leak, causing it to no longer function properly. The email was to inquire about the options available moving forward. Upon gaining clarification, that the area the NC PreK classrooms are housed on was the only area in the school where there was no hot water, S. Horton was advised that the children are required by sanitation standards to have access to hot water, and they would have to ensure they are meeting that requirement. I also explained that a request could be made for an emergency relocation of the children, within the school or at an alternate location. The explanation instructed Ms. Horton to notify me of the decision and then an observation and approval of the space would need to be completed. A final email was sent by Ms. Horton, after hours, stating that the children would be relocated on Monday, January 27, 2025. During a phone conversation with B. Pate, NC PreK Supervisor, on January 27, 2025, it was discussed that the local Environmental Health inspector would need to be contacted to ensure the facility is operating in compliance with the sanitation guidelines and rules. According to Principal Stinson, the unit cannot be repaired and will have to be completely replaced. He was also told this process may take up to 4 to 6 weeks, leaving the children without access to hot water or proper heating. When asked, the relocation of the children happened today when the students arrived. However, the spaces the children were moved to, within the school, were not approved before the relocation took place. A violation was documented today for the children utilizing unlicensed spaces without approval. Any violation(s) documented may impact the compliance history score. I did not review records today but continue to keep all staff and children’s records in the proper location in those temporary spaces. Maintain applicable program records on file as well. Ensure all staff have current health questionnaires and emergency information on file. All posted items will still have to be posted and can be in a central location. Continue to keep current sign in/sign out sheets, and attendance records in each space. We discussed the Emergency Medical Care plan should still be posted, as well as staff/ratio charts. Schedules and activity plans for each space. Also, post the Temporary Emergency Child Care form that was issued today. CHILD CARE APPLICATION AND INSPECTIONS A new child care application and applicable inspections may be required depending on how long you will be operating at the new location. I will continue to remain in contact with you regarding the status of the repairs/renovations, etc. We discussed contacting the local Environmental Health Specialist regarding the inaccessibility of hot water for the children. It may be required for you to obtain signed statements from the local sanitation inspector stating there are no hazards, and this location is safe for temporary occupancy. The Division of Child Development and Early Education’s procedures for programs in emergency situations was shared with you during the visit today and a copy of the procedures was left with you. A Temporary Emergency Child Care form was issued today, January 27,2024. Temporary emergency care is allowable for up to 90 days. We discussed that contact should be maintained to ensure updates are shared concerning the progress of the maintenance for the heating unit. This facility currently operates with a Five Star license issued on January 10, 2018, earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point. The last sanitation inspection was completed on October 16, 2023, earning a “superior” classification. The last fire inspection was completed on October 26, 2023 and approved for day time care only. The last annual compliance visit was made May 30, 2023. R. Barnes, Lead Teacher/Pre-K Coordinator was present and available to assist. There were fifty-seven (57) children present. The children present were playing in free choice play, interacting with the assigned staff, and outdoor play. Daily routine transitions such as preparing for lunch, toileting and hand washing were also observed. All off site forms were reviewed and found to be in compliance. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records were monitored. All staff records monitored were found to be in compliance. Three (3) violations were documented for children’s record keeping. The following violations of child care requirements were documented today. Violation Number Comment Rule 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Emergency Medical care information was not on file for K. Whitley. .0802(c) 1314 Emergency information did not name childs health care professional. A health care professional was not listed in the emergency information for K. Whitley .0802(c)(2) 1317 Authorization for emergency medical care information was not signed by child's parent. Parent did not sign the authorization for emergency medical care information for K. Whitley .0802(d) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 1, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is accurate documentation. If any information provided in the letter is not accurate, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. TECHNICAL ASSISTANCE/ CONSULTATION CHILDREN’S RECORD KEEPING The reviewed record for K. Whitley did not have completed Emergency Medical Care Information on file for review. The record was missing the preferred health care professional, and the parent’s signature authorizing emergency medical care for the child. All emergency medical care information is required to be completed on or before the child’ first day of enrollment and signed by the parent, authorizing the facility to take action in an emergency situation. LEAD IN WATER/PAINT AND ASBESTOS The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs must complete the application summary for their program regarding lead paint and asbestos by May 1, 2024. Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Most of you have completed your initial testing and now it is time for your three-year retest.
