Shaniqua Graham, Administrator, assisted me with the visit. Your program currently operates with a three-star license, issued July 15, 2020, earning 5 points in the education component, 2 points in the program standards component (meeting enhanced space and ratios) and 1 quality point for meeting the programmatic option. The last annual compliance visit was conducted December 10, 2024. The sanitation inspection was completed November 24, 2025 with a “Superior” classification. The last fire inspection was conducted January 6, 2025 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent as of December 8, 2025. The NC Secretary of State website was reviewed on December 8, 2025 and Save the Children was listed as current- active. You visited each indoor and outdoor space with me. Children were observed playing in centers, playing with kinetic sand with staff, toileting, eating lunch and napping. Lunch was observed and consisted of fish fillet sandwich, tater tots, peaches and milk. One violation was documented and corrected during the visit - Violation Number Comment Rule 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Six staff files did not have medical information stored separately from other documents. The administrator corrected this during the visit. .0701(d) Comments: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. QRIS Head Start Pathway- The Request for Accreditation and Head Start Licensure Pathway form was completed today to transition the facility to the new QRIS Pathway system. The facility submitted a copy of the verification of head start designation today. The facility will increase to five stars based on these requirements. I interviewed Ms. Baker, Interim Administrator, regarding the allegations. Ms. Baker stated she was not aware of the incident until the evening of the incident. The next morning she viewed the video and observed the staff hit the child on the shoulder. I was given permission to view the video of the incident. It clearly showed the staff person hit the child's shoulder with her hand after he stepped in juice that was spilled on the floor. Staff A, who witnessed the incident, stated the child stepped in the juice and the staff person hit the child on his right shoulder and asked him why he stepped in the juice. Staff B, who also witnessed the incident, stated the staff person did not hit the child. She stated the staff person grabbed the child's arm and gently pulled him away from the juice. Based on interviews and video observation, this complaint is substantiated. Violation Number Comment Rule 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. A three year old child was hit on his shoulder by a staff member after he stepped in spilled juice. .1803(a)(1) Ms. Baker stated the staff person has been suspended and an internal investigation is ongoing. She stated once the investigation is completed, staff will receive training on Safeguarding Children and active supervision and appropriate discipline of children. Comments: Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. On or before May 21, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. A toilet bowl brush was sitting in the bathroom for preschool children. It was removed and made inaccessible during the visit. .2820(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Technical Assistance- Ms. Washington asked for guidance concerning children with behavioral issues. The Partnership for Children and Families in Cumberland County has a Behavioral Specialist who is available to assist Moore County providers. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated.
