Gayle Rodriguez, operator, assisted me with the visit. Your program currently operates with a three-star license, issued February 27,2023, earning 4 points in the education component, 2 points in the program standards component and 1 quality point for meeting the programmatic option. The program’s compliance history was ninety five percent as of January 14, 2026. You visited each indoor and outdoor space with me. Children were observed preparing for and resting. Lunch consisted of chicken nuggets, mixed fruit, green beans and milk. The following one violation was documented- Violation Number Comment Rule 1409 Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. A lower cabinet door in the kitchen was broken off the hinges and allowed children access under the cabinets. 10A NCAC 09 .1719 (a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 11, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Children were observed playing in centers, preparing for and eating lunch and having gross motor activities with the provider. Lunch was observed and consisted of hamburger patty with gravy, mashed potatoes, green beans, bread slice and milk. The provider has completed the lead/asbestos analysis in November 2022. The following one violation was documented- Violation Number Comment Rule 816 Each child's hands were not washed before and after eating. The children did not wash hands before or after eating. .1725(a)(7)(B) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Gayle Rodriguez, operator, assisted me with the visit. Your program currently operates with a three star license issued September 17, 2019. Restrictions are daytime and overnight, ages zero to twelve years of age, maximum of five preschool children at any time, children play in fenced in yard, no more than two infants under age one per shift. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent as of August 13, 2024. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Supervision and staff interaction were appropriate for the children in care. Children were observed having free play, toileting and preparing for and eating lunch. Lunch consisted of baked ham, macaroni and cheese, sweet potatoes, green beans and milk. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. There were two violations documented and discussed today. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. A outdoor play area check had not been completed since July 2024. 10A NCAC .1721(e)(5)(A-F) 1853 The operator did not conduct a monthly fire drill. A fire drill had not been conducted since July 2024. .1719(a)(15) & .1721( e)(2) On or before August 28, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance- The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. The rulemaking process will include publishing of proposed revised rules as well as a public hearing and public comment period. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. Mrs. Rodriguez stated she has not competed the asbestos part of the testing from the NC Clean Classrooms for Kids. I reminded her to complete it by January 1, 2025. Technical Assistance- Mrs. Rodriguez is interested in becoming a center and finding a suitable building. I encouraged her to have the building inspected by building, fire, and sanitation before making a purchase or renting the building. I also suggested she register for the Pre-Licensing Workshop for Centers. She can register for and find information on the DCDEE website. Two violations were observed today. Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. The provider's First Aid certification expired May 2023. .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification.
