Applicable childcare requirements were discussed with you, Latasha Johnson, Operator. During this visit I monitored the indoor and outdoor areas approved for child care. One (1) child was present. The child was observed actively engaging with the caregiver and participating in various free-choice centers, including fine motor activities, books, blocks, and dramatic play. Positive and developmentally appropriate interactions between the caregiver and child and adequate supervision were observed during this visit. Their three-star license was posted and in compliance with all restrictions noted on the license. In addition, the operator’s file, program records, and all children files were monitored. According to the menu the children will be served baked chicken, pasta, green beans, orange slices, and milk. The operator does participate in the CACFP. Overall enrollment and attendance information were collected. The attached Family Child Care Enrollment Worksheet reflects this information. At the conclusion of the visit, the visit summary was reviewed with you, and you were provided with an opportunity to ask questions, seek clarification, and request technical assistance. You were also provided with a copy of this visit summary for your records. Violation Number Comment Rule 908 Health questionnaire was not completed annually. This was not completed annually. The operator completed the health questionnaire during the visit so, it has been corrected. .1703(a)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The operator hasn't created a roster in ABCMS within the required timeframe. G.S. 110-90.2 & .2703(r) Two (2) violations were cited during this visit. All violations cited must be corrected immediately. One (1) violation was corrected during this visit. A signed and dated letter of compliance must be submitted to me no later than May 12, 2026, clearly detailing how each violation has been corrected. You may email your compliance letter to me at leisa.benson@dhhs.nc.gov. Failure to submit your compliance letter by the stated deadline or failure to contact me regarding a revised timeline may result in a follow-up visit, and violations may be cited. If you are unable to meet the required deadline of May 12, 2026, you must contact me prior to that date with a proposed timeline for completing the required corrections. In some cases, this time frame may be extended. Important Notice: If your compliance letter states that corrections have been made and it is later determined they were not, this will be considered falsification of information, and an administrative action may be taken against your license. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit may negatively affect your compliance score. As of today, your compliance history is 99%. Your compliance history was reviewed with you, and you were also given a copy to keep for your records. Your three-star rated license will not expire until November 15, 2027. However, because it was evaluated using the previous star rated license application process you are requesting to have your rated license reevaluated using the current application process which is QRIS because you may earn a higher star license based on your education. I reviewed the three different pathway options with you, and you stated that you will go with pathway #2-Classroom and Instructional Quality for FCCH. During this visit I received your application to apply for a star rated license. I discussed with you that I can’t process your application until you have corrected the violation noted above and you submit all completed forms required for pathway #2. Reminders: Children that attend NCPRK or head start are not considered school-agers based on the DCDEE definition of school-agers. You can access this information by reviewing Chapter 9 Child Care Rules Effective July 1, 2025. You can access the rules at: www.ncchildcare.nc.gov. Based on your education you are required to complete at least eight (8) in-service training hours annually. As of today’s visit, you have completed seven (7) in-service training hours. You should complete all required in-service training hours by May 2026. Health questionaries are required to be completed annually. You completed this and placed it in your file during this visit so, it was corrected during this visit. The ABCMS roster should be completed once you transition from a temporary license to a star rated license. You have not completed this. I provided you with technical assistance on how to create this roster and I also provided you with a step-by-step guide on how to create the roster. Technical assistance: You were in compliance with this, however, per your request, I provided you with technical assistance on how to complete the On-Going Training Documentation so you can easily track your training hours from year to year. BENSON Operation Type: Family CC Home Case Number: Visit Date: 7/9/2024 Number Present: 0 Completed Date: 7/9/2024 Age: From 0 To 0 Total Minutes: 195 Time In: 08:30 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to the first Temporary Time Period visit. This program currently operates with a temporary license effective May 15, 2024, to November 14, 2024. Mrs. Latasha Johnson, Operator was present during today’s visit. All indoor and outdoor spaces designated for child care were monitored during this visit. According to Mrs. Johnson she has four (4) children enrolled; however, they were not present. I reviewed the daily sign in/out sheet and four (4) children were signed in/out when present. Program records, operator file, and children files were reviewed during today’s visit. Their last fire inspection was conducted on January 10, 2024. They are required to have an annual fire inspection. You requested to be approved for transportation. During the visit I monitored the following: Vehicle that will be used for transportation: Ford Explorer. Current Driver’s License for Lastasha Johnson. The vehicle is in good repair. Once I receive the following information I can approve you for transportation: Expiration date of Vehicle Registration Date of last inspection Fire extinguisher secured Fire Aid Kit secured Emergency information and attached picture for each child transported, on the vehicle Proof of liability insurance No smoking sign Transportation Roster At the completion of the visit, the Visit Summary was reviewed with Mrs. Johnson and a computerized copy was given to her. Violation Number Comment Rule 1718 The written plan of care was not given and explained to parents of children in care on or before the first day the child attended the home. Parents did not sign a statement acknowledging the receipt and explanation of the plan. Parents did not give written permission for their child to be transported by the operator for specific routine tasks that are included on the written schedule. The written plan of care for one child enrolled was not signed by the parent acknowledging receipt and explanation of the plan. .1712(e )(6) The violation/s documented above must be corrected immediately. A signed and dated letter of compliance must be mailed to me by July 23, 2024, describing how and when the violation/s were corrected. You should email the compliance letter to leisa.benson@dhhs.nc.gov . If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by July 23, 2024, you must contact me with a purposed timeline of the corrections. In some cases, this timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. As of today’s, visit your compliance history is 97%. Rated License Assessment: In order to maintain or be eligible for subsidy payments at the end of the temporary time period, the facility must transition to a three to five star rated license. An application for a star rated license was given to you during today’s visit. If you decide to apply for a star rated license you should submit your application to me by July 23, 2024. I reviewed the Assessment for a Two Component Through Five Star Rated License Form with you and I also gave you a copy. Education Standards: You submitted your most current education information to WORKS. You are qualified as a lead teacher with fifteen (15) additional semester hours in ECE. You have a two (2) year degree in Business Administration. If you receive additional education you should submit it to WORKS as soon as possible to be evaluated. Prompt submission of this information is imperative to allow adequate time for processing. Program Standards: You could receive two (2) points in program standards without the ERS assessment; however, to receive more than two (2) points in program standards, you will need to have an Environment Rating Scale (ERS) assessment and score at least a 4.0. Be sure to visit www.ncrlap.org to familiarize yourself with the FCCH assessment. I recommend that you contact the Moore County Partnership for Children for technical assistance if you choose to have the ERS assessment. You stated that you do not want the ERS assessment because you feel you need longer than six (6) months to prepare for it. If you decide later to have the ERS assessment you will need to notify me by September 1, 2024, to allow enough time for the ERS assessment to be completed before your temporary license expires; a temporary license cannot be extended. Quality Bonus Point: While you may meet more than one (1) possible quality point, you can only earn a maximum of one (1) quality bonus point. Total points and Star Rated License Information: The total points received in both education and program standards and the quality bonus point will determine the star license issued to your FCCH. To receive a three-star license you must earn at least seven (7) points. To receive a four-star license, you must earn at least ten (10) points. To receive a five-star license you must earn at least thirteen (13) points. Rated License Information: In order to earn two (2) or more points in Program Standards for the rated license a three-month self- study on the FCCERS must be completed. You should start the three-month self-study immediately and it should be completed by October 1, 2024. This could include, but is not limited to, the following: Self-study/assessment using the FCCERS score sheets List of low scoring items with a plan to improve quality Develop a timeline of making improvements or enhancements to the child care environment Documentation of on-going observations and follow up using the ERS Improvement Plan Self-study assessment based on information provided by a TA Specialist, or a mock assessment completed by the local resource and referral agency. The FCCERS book may be purchased through various retail outlets or some resource and referral agencies. In addition, you may go to www.ncrlap.org for information on the FCCERS process and helpful information. Also, if you meet the requirements for a 4-5 star rated license and plan to provide care to 4-year old’s you will be required to have a curriculum that has been approved by DCDEE.
