The allegations are as follows: There are concerns that there are violations of child care requirements regarding discipline. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. One file for one new staff was reviewed, and no violations were found in that file. Additionally, I observed the indoor and outdoor spaces used by the children. I observed the teachers actively supervising and interacting with the children in the classrooms. I observed the children participating in free play, teacher-directed activities, group times, personal care routines and transitions. Today’s breakfast menu consisted of sausage biscuits, bananas and milk. Based on information provided on May 12, 2026, regarding concerns of the use of inappropriate discipline, a child was allegedly spanked and yelled at while outdoors. During my interview with the administrator, Jessie Kelley, she stated that she has not noticed any behavior issues in any of the classes nor has she had any concerns with discipline. She stated she can also hear the interactions between the staff and children throughout the center during the day and she has not noticed any issues. She also stated that there have not been any parents to come and express any concerns to her. She stated that both she and another teacher have children or grandchildren in the center and sometimes it is hard for those children to understand why things have to go a certain way or why they can’t have their way in all situations. She stated that during those times those children may cry or become upset but that they will talk with the children and help them to calm down. During my interview with Teacher A, she stated that she has not had any major discipline or behavior issues in her class. She stated that she does have a grandchild in her class and that there are times when that child cannot understand why they can’t have or do certain things that they may be allowed to do at home. She stated that she sometimes has to redirect the child to another area or activity because the child wants to sit near her or in her lap a lot of the times and when they can’t they cry a lot. According to her this happens both indoors and outdoors. She stated that she will try to explain to the child that she is the teacher at school and that she has to spend time with and play with the other children in the class too. She stated that she has not had any parents say anything about any concerns about discipline in the class. I spoke with Teacher B and she stated that she does have a couple of children in that class who sometimes will hit the other children. She stated that she will redirect the child or have them come sit with her for a minute and talk with them about the behavior and how they should treat their friends. She stated that she will go over the class rules. She also stated that any behaviors that she may have in that class from time to time are ones that she considers to be age appropriate. During my observation in the space used by the two- and three-year-olds, one child hit another child and Teacher B called the child over to sit at the table away from the group. When the child was asked why did they hit the other child, that child began to cry. The teacher spoke with the child about why they shouldn’t hit their friends and then sent the child back over to the group where they were waiting to go outside. In addition to my observation in this space and listening to all the language and interactions as well as in the spaces used by the one-year-olds and the preschool children and infants, discipline was found to be positive and adequate. I also reviewed the facility’s discipline policy and attendance sheets. Based on the above information, this allegation was unsubstantiated. The following violations were cited during today’s visit: Violation Number Comment Rule 316 Children under one year of age were not kept separate from children two years and older. Upon my arrival to the facility, space #4 had a two(2) month old and an eight(8) month old grouped with twenty one (21) other children who ranged in ages from two(2) to five (5). 10A NCAC 09 .0713(a)(5) 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. Upon my arrival, four(4) one-year-old children were grouped with fourteen (14) children ranging in age from three(3) to five (5). 10A NCAC 09 .0713(a)(6) 1245 For each child, there was not at least 30 sq. ft. inside space per the total licensed capacity and 100 sq. ft. outside space for each child using the outdoor learning environment at any one time. Upon my arrival, Space #4 used by the preschool children with a capacity of 15 had 23 children present in that space. 10A NCAC 09 .2809(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 3, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. An extra day was given because the facility is closed due to the upcoming Holiday. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Name of Operation: Jessie's Little Blessings Facility ID: 63000562 Consultant: TURKESHIA MCCORMICK Operation Type: Center Case Number: Visit Date: 4/8/2025 Number Present: 26 Completed Date: 4/8/2025 Age: From 0 To 5 Total Minutes: 210 Time In: 12:30 PM Time Out: 04:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's visit was to monitor your program for compliance with all applicable child care requirements during the first temporary time period visit (TTP). Jessie Kelley, administrator/owner assisted me with the visit. This facility was issued a temporary license on March 3, 2025 with the following restrictions: First and second shift and children under 2 ½ in rooms with direct exits only. Due to Hold Harmless the facility still meets four points in the education component, two points in the program component and one quality point for complying with the programmatic option. The facility’s last sanitation inspection was conducted on February 18, 2025, and the last fire inspection was conducted on December 17, 2024. I reviewed the North Carolina Secretary of State’s website and Jessielittleblessingsdaycare LLC. was listed as current-active. I monitored all indoor spaces, the outdoor learning environments, all staff six (6) files and three (3) children’s files, and other required program records were monitored. I observed the children in the spaces used by the toddlers and preschoolers during naptime, transitioning from nap to snack and personal care routines such as handwashing and toileting. I observed the children in the space used by the infants during naptime, tummy time, teacher directed activities and diapering and handwashing. The required posted information and materials were present in each classroom. The children were adequately supervised. Afternoon snack was observed and consisted of cinnamon rolls and milk. The following violations were cited during today’s visit: Violation Number Comment Rule 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. C. Route who was working in the infant room, has not completed the training within the required 90 days of employment. .1102(f) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. C. Route began employment on 1/30/2025 but did not sign the acknowledgement statement until 2/12/2025. .0608(d)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 23, 2025, (two extra days were given due to the facility being closed two days for the upcoming Holiday) I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. If you email the compliance letter, it must be sent from the email address registered with the DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical assistance was offered on the importance of following the center’s safe sleep policy. The purpose of a safe sleep policy is to reduce the risk of Sudden Infant Death Syndrome or SIDS and other sleep related accidents. Infants under the age of one must be visually checked every fifteen (15) minutes and it must be documented properly on the individual sleep charts. The Center for Disease Control (CDC) and the U.S. DHHS recommends that infants be given their own sleep space that is blanket free of soft bedding, pillows, bumper pads and toys. The American Association Pediatrics or AAP recommends that devices such as strollers, swings, infant carrier, car seats and inclined seats not be used for regular sleep routines. The safe sleep policy states that infants be placed in their cribs on their back to sleep. For children who can rollover on their own, I recommend placing a statement over their cribs stating that they can do so. Technical assistance was also offered on maintaining staff files. To ensure that the required documentation is completed by the required deadlines, consider creating a system where the files are checked periodically or when new staff are hired, they are checked weekly for the first six (6) weeks and afterwards monthly until they have completed their first ninety (90) days of employment. As a reminder, C. Durham will need to complete orientation on adequate supervision and prevention of infectious diseases by the close of business today. Be sure to document it's completion on the orientation sheet and place in their file. C. Route will need to complete CPR/First Aid training and ITS-SIDS trainings.
