Allah Murchison, operator, assisted me with the visit. Your program currently operates with a three-star license, issued January 25, 2024, earning 4 points in the education component, 2 points in the program standards component (meeting enhanced space) and 1 quality point for meeting the education option. The last annual compliance visit was conducted March 25, 2025. The sanitation inspection was completed March 5, 2026 with a “Superior” classification. The last fire inspection was conducted October 26, 2025 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety four percent as of March 16, 2026. You visited each indoor and outdoor space with me. Children were observed having story time, free play in centers, toileting and preparing for lunch. Lunch was observed and consisted of chicken nuggets, green beans, mixed vegetables and milk. Violation Number Comment Rule 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. One staff person did not have medical information separate from other personnel information. .0701(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The allegations are as follows: There are concerns that – Appropriate ratios are not maintained. Diaper bags were stored on the floor of the toilet room in the room for infants. Mouthable toys are washed in the toilet room sink. All items were substantiated. There were eleven violations documented today and all were verified as corrected during the visit. Potentially hazardous foods are not properly stored. (formula) Staff is not following safe sleep practices. An infant was observed sleeping in a bouncer while a bottle was left in their mouth/laying on their chest without any teacher supervision. An infant was observed sleeping for an extended period in a bouncer, with their head in a way that looked as if it could make it difficult for the child to breathe. A refrigerator was placed in the room for infants, adjacent to the food prep handwashing sink. The unit appears to impede egress to the back door. Medications are not properly stored. Sanitizer and cleaners are not available as required. Broken glass was found on the play area. I spoke with Allah Murchison, Administrator, regarding the allegations. She stated the allegations were confirmed prior to the visit and she has corrected all the items. Ms. Murchison stated she has completed the following- 1. She purchased cubbies for the children’s belongings in the classroom for infant/toddlers. 2. Staff/child ratios are monitored and if assistance is needed staff have been instructed to call for help from the cook. 3. Staff have been reminded to ask for assistance if they are busy with other children and another child needs assistance. 4. Staff have been reminded infants/toddlers must be held or placed in an appropriate chair or high chair for meals. 5. A refrigerator was placed in the classroom for infants/toddlers so staff can refrigerate food and bottles. 6. A plastic bin marked “dirty toys” has been placed in the classroom for infants/ toddlers and staff have been reminded to place dirty or mouthed toys in the bin. 7. One staff person is responsible for daily playground checks. 8. Any medications/cleaning products are locked in the staff bathroom. Any refrigerated medications are placed in a lock box in the refrigerator. 9. During the visit, I provided the DCDEE sleep charts to Ms. Murchison for easier documentation. We reviewed them with staff. She stated she has reminded staff to place the infants on their backs when they are placed in the cribs for nap time. 10. During the visit, Ms. Murchison and I rearranged the classroom for infants/toddlers so the fire evacuation crib can fit through the exit door. 11. Ms. Murchison stated she has appointed the cook to make sure there is disinfectant and sanitizer mixed and available for use daily. All allegations were substantiated based on observations made by a collaborating agency. There were eleven violations documented today. Two teachers were present with seven infants and two children that were one year of age. GS 110-91(7);.0713(a-d) 491 Caregiver did not respond at the earliest opportunity to an infant or toddler’s physical and emotional needs. A child was observed sleeping in a bouncer for an extended period of time, with his/her head turned in such a manner the child could have had difficulty breathing. Also, a child fell face forward and was bleeding from the nose and the child was not tended to after the fall.1. During the visit today, children asleep were placed in the cribs. One child who fell asleep in a bouncy seat was placed in a crib within a couple minutes of falling asleep. 2. The operator and staff stated they were not aware of a child falling and bleeding. .0511(b)(1) 532 All children were not held or placed in feeding chairs or other appropriate apparatus to be fed. An infant was placed in a bouncy chair and given a bottle of milk. The child fell asleep with the bottle on its’ chest and remained in the seat for an extended period of time. During the visit, the children were observed being held for feeding or in high chairs. 10A NCAC 09 .0902(b) 536 Formula and other beverages which require refrigeration were not identified for each child or properly refrigerated. On two previous occasions, food sent from home, including infant formula, was not placed in refrigeration. The operator stated a parent had asked for her child’s formula to not be refrigerated. During today's visit, all foods and bottles were placed in a refrigerator in the classroom for infants/toddlers or in the refrigerator in the kitchen. 15A NCAC 18A .2804(d) 616 Toys and other mouth-contact surfaces were not cleaned and sanitized daily when used or cleaned more frequently if visibly dirty. On August 9, 2024, dirty mouthable toys were cleaned in the sink in the bathroom in the classroom for infants. During today's visit, staff placed dirty toys in a basket and they were taken to the kitchen for cleaning. .2822(a)(1-4) 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. Broken glass was found on the playground. During today's visit, no glass was observed on the outdoor play area. 15A NCAC 18A .2832(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Medications were observed unlocked in a first aid kit sitting on a cart accessible to children in the classroom for preschool children. During today's visit, the medications were observed locked in storage. 15A NCAC 18A .2820(d) 871 Center staff did not comply with the safe sleep policy. Staff were observed placing infants to sleep on their stomachs. One child was observed with a loose fitting sheet bunched up around the his/her mouth while sleeping on his/her stomach. Several infants had other items such as toys in the cribs while they slept. During today's visit, children were observed being placed on their backs for sleeping and there were no other items in the cribs with the children while they were napping. 10A NCAC 09 .0606(a) 882 The evacuation crib or other approved device was not able to fit through the exit door. -A refrigerator and some large toys were placed beside the exit door and prohibited the evacuation crib from exiting the classroom during fire drills. During the visit today, we moved the toys to another storage place and moved the position of another crib which allowed for the evacuation crib to fit through the space to the exit door. .0604(r) 9995 A violation was found for which there is no item number. Sanitation requirement 15A NCAC 18A .2812(i)(j). An approved disinfectant shall be provided for cleaning purposes and a sanitizing solution shall be provided for cleaning purposes. There were no sanitizer of disinfectant available at the facility on several occasions. During today's visit, sanitizer and disinfectant was observed in the facility. Technical Assistance- 1. Storage space must be available for children's belongings to keep cross contamination to a minimum and secure a health environment for children. 2. Staff/child ratios must be maintained for the safety of all children. If staff are not present, accommodations must be made to ensure staff/child ratios are maintained. 3. Caregivers must respond the the infants needs at the earliest opportunity. If available, an extra staff person must be called into the classroom to assist with caregiving. Response to infant needs is vital to the health and safety of the children. 4. Children must be held or placed in appropriate feeding apparatus such as a high chair to prevent the choking or strangulation during feeding times. 5. All food and formulas that require refrigeration must be refrigerated to prevent food borne illnesses. 6. Dirty toys and mouthable toys must be cleaned to prevent cross contamination of illnesses among children and the spread of germs. 7. All areas, including the outdoor play area must be checked daily before the children go outside for any debris, trash, such as broken glass, to ensure safety of the children in the area they are playing. 8. Medications must be made inaccessible to children at all times to prevent consumption and possible reactions and side effects to medications that are ingested by children other than the ones it is prescribed for. 9. Toys, bottles, pacifiers, loose fitting sheets/blankets must not be placed in the crib with sleeping children. These items could contribute to SIDS deaths or other health complications. 10. Always ensure exits are not blocked and there is enough room to exit the facility with the evacuation crib. Blocked exit doors could contribute to injuries and even death in the event of a fire. 11. Disinfectant and sanitizer must always be available for cleaning and sanitizing to prevent cross contamination, food borne pathogens an eliminate germs to prevent illnesses. An infant was observed sleeping in a bouncer while a bottle was left in their mouth/laying on their chest without any teacher supervision. An infant was observed sleeping for an extended period in a bouncer, with their head in a way that looked as if it could make it difficult for the child to breathe. A refrigerator was placed in the room for infants, adjacent to the food prep handwashing sink. The unit appears to impede egress to the back door. Medications are not properly stored. Storage space must be available for children's belongings to keep cross contamination to a minimum and secure a health environment for children. 2. Staff/child ratios must be maintained for the safety of all children. If staff are not present, accommodations must be made to ensure staff/child ratios are maintained. 3. Caregivers must respond the the infants needs at the earliest opportunity. If available, an extra staff person must be called into the classroom to assist with caregiving. Response to infant needs is vital to the health and safety of the children. 4. Children must be held or placed in appropriate feeding apparatus such as a high chair to prevent the choking or strangulation during feeding times. 5. All food and formulas that require refrigeration must be refrigerated to prevent food borne illnesses. 6. Dirty toys and mouthable toys must be cleaned to prevent cross contamination of illnesses among children and the spread of germs. 7. All areas, including the outdoor play area must be checked daily before the children go outside for any debris, trash, such as broken glass, to ensure safety of the children in the area they are playing. 8. Medications must be made inaccessible to children at all times to prevent consumption and possible reactions and side effects to medications that are ingested by children other than the ones it is prescribed for. 9. Toys, bottles, pacifiers, loose fitting sheets/blankets must not be placed in the crib with sleeping children. These items could contribute to SIDS deaths or other health complications. 10. Always ensure exits are not blocked and there is enough room to exit the facility with the evacuation crib. Blocked exit doors could contribute to injuries and even death in the event of a fire. 11. Disinfectant and sanitizer must always be available for cleaning and sanitizing to prevent cross contamination, food borne pathogens an eliminate germs to prevent illnesses. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. All violations were verified as corrected. No compliance letter is required. However, a follow up visit is required due to the staff/child ratio violations. A computer generated visit summary was used today due to internet issues.
