Your program currently operates with a four-star license, issued September 20, 2025. The facilities compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent as of June 5, 2026. The license was posted, and the restrictions were in compliance. A walk-through of the home was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and capacities to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. During today’s visit it was discovered that the background checks for G. McDonald, M. McDonald, M. McDonald, T. Hancock, and G. Miles are expired. The following violations were cited during today’s inspection. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The qualification letters for G. McDonald, M. McDonald, M. McDonald, T. Hancock, and G. Miles were expired and a new application was not completed. G.S. 110-90.2 & .2703(n) & (o) 1831 A valid qualification letter was not on file and available for review at the facility. A valid qualification letter is not on file for G. McDonald, M. McDonald, M. McDonald, T. Hancock, and G. Miles. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The provider's roster in the ABCMS portal is not update to date with all employees. G.S. 110-90.2 & .2703(r) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 19, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Your program currently operates with a four-star license, issued September 20. 2022, earning 7 points in the education component, 2 points in the program standards component meeting minimum requirements, completing a self-study and self-assessment using FCCERS and 1 quality point for limiting infant capacity to no more than two children under one year of age. The last annual compliance visit was conducted September 10, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of September 4, 2025. During today’s visit, I observed children in the indoor learning environments. Children were participating in general personal care routines and free play activities. Teachers were seen walking around the room engaging with children during play. I found supervision, ratios, and interactions to be in compliance. The following were also inspected during today’s visit: program requirements, equipment, furnishings, staff/child ratios, supervision, medication, medication administration, outdoor learning environments, and program records including staff records and children records. The following was cited during today’s inspection. Violation Number Comment Rule 908 Health questionnaire was not completed annually. This was corrected during the visit by the provider filling out a new questionnaire. .1703(a)(1) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Technical Assistance: It is important to complete all required provider documents in the required timeframes. I would recommend reviewing your provider file twice a year to ensure all documents are completed in the required timeframes. Mrs. McDonald, operator, assisted me with the visit. Your program currently operates with a four-star license, issued September 20, 2022, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for meeting the programmatic option. The compliance history was reviewed with the operator. The program’s compliance history was ninety six percent as of March 19, 2025. The four star license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. The children were observed having free play, preparing for and eating lunch, toileting and napping. Lunch consisted of ham sandwich, French fries, baked beans, blueberries, cole slaw and milk. Supervision and staff interaction were adequate. There was one violation documented today. Violation Number Comment Rule 709 Equipment and toys were not in good repair and developmentally appropriate. The toilet lid was cracked and broken. 10 A NCAC 09.1720(a)(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. The Child Care Commission is currently in the process of drafting proposed child care rule language to begin the rulemaking process for implementing the new requirements. The rulemaking process will include publishing of proposed revised rules as well as a public hearing and public comment period. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. The visit summary was discussed with Mrs. McDonald and a copy was left at the facility.
