They shall have a physical assessment, including a tuberculosis status assessment by a licensed physician or registered professional nurse. A physical assessment report, signed by a licensed physician or registered professional nurse, shall be on file at the time of employment or within thirty (30) days following employment.

Infants and toddlers shall have safe toys. Toys, parts of toys or other materials small enough to be swallowed shall not be used.

Requirements for indoor play equipment and materials were not met. Feeding equipment was not in good condition as evidenced by safety straps were missing on the feeding table.

It shall be well-maintained and free of hazards such as poisonous plants, broken glass, barbed wire, open wells, rocks and other debris, and shall have good drainage.

The facility's outdoor space was not safe for children's activities as evidenced by the presence of the black plastic ground cover is exposed creating a tripping hazard.

5 CSR 25-300.080 · 5 CSR 25-300.080 Sanitation Requirements (2) (A) states: Toilets, urinals and hand washing sinks shall be clean and odor free.

The bathroom was not clean and odor free as evidenced by urine and/or excrement odor was detected in that strong smell of urine in the 3 and 4 year old room and in the hall girls bathroom.
