The facility's outdoor space was not safe for children's activities as evidenced by there is a black cord/wire that is coming out of the wall of the building that is on the infant and toddler playground side. The black cord/wire is approximately 3-4 feet and is looped and loose in the infant and toddler playground. The provider will need to provide evidence that the long cord/wire is not present in the infant and toddler playground. The provider may need to box and cover the cord or may need to contact the provider of the cord (cable or phone company) to come and bury the wire or remove the wire.
